C. INSTRUCTIONS FOR SELECTING STUDENT RECORDS
The list of 10% of your school’s Special Education Student Records (IEPs): Your DVR Team Lead will
share the list with you approximately one week prior to your DVR.
 Organize all sped files and cumulative records prior to the DVR
 Provide entire cum and green folder (DO NOT PHOTOCOPY IEP)
Provide a current initial assessment report written within the past 12 months by each Resource Specialist
Teacher at your school (if applicable)
Select 2 Section 504 Plans of students:
 Provide 504 Plans
 Provide students’ ID numbers (the ID number is not documented on the plan)
 Provide students’ schedules of classes
