Messaging Community of Practice (LabMCoP) has reviewed these and in light of the discussion, that the long term goal is to migrate this vocabulary from HL7 to an external coding system (SNOMED CT is proposed). The following recommendations were made: For the following terms used in HL70070, but missing from HL70487, where no SNOMED CT code exist: Table0070 Table0070 Table0487 Table0487 SPM.4 SPM.4 term Code Value Code Value DOSE Dose med leave out or substance RT Route of leave out medicine XXX To be leave out specified in another part of the message UMED Unknown medicine UMED Unknown medicine LNA Line arterial LNA Line arterial LNV Line venous LNV Line venous 122552005 Arterial blood specimen (specimen) 122555007 Venous blood specimen (specimen) DOSE and RT are not considered specimen, XXX is not appropriate, as the SPM segment should be the source of truth for this information and also where in the message is not indicated with this Page 10 of 20 code, leaving ambiguity. UMED was considered a valid specimen type for forensic and possibly chemistry testing. LNA and LNV, even though they have a related SNOMED CT term, would require the additional use of the SPM-7 field in addition to the SPM-4 to properly differentiate to other specimen types. Since not all systems currently support more than one field for specimen, the decision was made to add them to HL70487. For the following terms used in HL70070, but missing from HL70487, where a single SNOMED CT code exists that can be used instead: Table0070 Code AMN Table0070 Value Amniotic fluid Table0487 Code /Value use SNOMED CT SPM.4 SPM.4 term 119373006 Amniotic fluid specimen (specimen) BDY Whole body Bile fluid use SNOMED CT 91000009106 carcass specimen use SNOMED CT 119341000 Bile specimen (specimen) BLD Whole blood use SNOMED CT 258580003 Whole blood sample (specimen) BLDA Blood arterial use SNOMED CT 122552005 Arterial blood specimen (specimen) BLDCO Cord blood use SNOMED CT 122556008 Cord blood specimen (specimen) BLDV Blood venous use SNOMED CT 122555007 Venous blood specimen (specimen) BPH Basophils use SNOMED CT 119357000 CDM Cardiac muscle use SNOMED CT 119377007 Basophil specimen (specimen) myocardial specimen (specimen) CNL Cannula use SNOMED CT 119308003 CSF Cerebral spinal fluid use SNOMED CT 258450006 CUR Curettage use SNOMED CT 258419009 Curettings (specimen) CVM Cervical mucus use SNOMED CT 122577006 Cervical mucus specimen (specimen) CVX Cervix use SNOMED CT 119395005 Specimen from uterine cervix (specimen) DIAF Dialysis fluid use SNOMED CT 119360007 Dialysis fluid specimen (specimen) DUFL Duodenal fluid use SNOMED CT 122574004 Duodenal fluid specimen (specimen) EAR Ear use SNOMED CT 309165001 Ear sample (specimen) ENDC Endocardiu m use SNOMED CT 119378002 Endocardial specimen (specimen) BIFL Cannula specimen (specimen) Cerebrospinal fluid sample (specimen) Page 11 of 20 ENDM Endometriu m use SNOMED CT 119396006 Specimen from endometrium (specimen) EOS Eosinophils use SNOMED CT 119356009 Eosinophil specimen (specimen) EYE Eye use SNOMED CT 128164002 Tissue specimen from eye (specimen) FIB Fibroblasts use SNOMED CT 119333000 Fibroblast specimen (specimen) GEN Genital use SNOMED CT 119344008 Specimen from genital system (specimen) GENC Genital cervix use SNOMED CT 119395005 Specimen from uterine cervix (specimen) GENL Genital lochia use SNOMED CT 122579009 Genital lochia specimen (specimen) HAR Hair use SNOMED CT 119326000 Hair specimen (specimen) ISLT Isolate use SNOMED CT 119303007 Microbial isolate specimen (specimen) LYM Lymphocyt es use SNOMED CT 119353001 Lymphocyte specimen (specimen) MAC Macrophag es use SNOMED CT 119352006 Macrophage specimen (specimen) MAR Marrow use SNOMED CT 119359002 Bone marrow specimen (specimen) MAR Marrow use SNOMED CT 119359002 MEC Meconium use SNOMED CT 119340004 Bone marrow specimen (specimen) Meconium specimen (specimen) MILK Breast milk use SNOMED CT 119328004 mother's milk specimen (specimen) MLK Milk use SNOMED CT 119321005 Milk specimen (specimen) NAIL Nail use SNOMED CT 119327009 Nail specimen (specimen) NOS Nose (nasal passage) Pancreatic fluid use SNOMED CT 119388001 Specimen from internal nose (specimen) use SNOMED CT 119343002 Pancreatic fluid specimen (specimen) PAT Patient use SNOMED CT 91000009106 carcass specimen PLC Placenta use SNOMED CT 119403008 Specimen from placenta (specimen) PLR Pleural fluid (thoracente sis fld) use SNOMED CT 418564007 Pleural fluid specimen (specimen) PAFL Page 12 of 20 PMN Polymorph onuclear neutrophils Erythrocyte s use SNOMED CT 119355008 use SNOMED CT 119351004 SMN Seminal fluid use SNOMED CT 119347001 Seminal fluid specimen (specimen) SWT Sweat use SNOMED CT 122569007 TEAR Tears use SNOMED CT 122594008 Sweat specimen (specimen) Tears specimen (specimen) THRB Thrombocy te (platelet) use SNOMED CT 119358005 Platelet specimen (specimen) TISG Tissue gall bladder use SNOMED CT 122656001 Tissue specimen from gall bladder (specimen) TLGI Tissue large intestine Tissue lung use SNOMED CT 122643008 Tissue specimen from large intestine (specimen) use SNOMED CT 127458004 Specimen from lung (specimen) Tissue small intestine Ulcer use SNOMED CT 122638001 Tissue specimen from small intestine (specimen) use SNOMED CT 119369008 Specimen from ulcer (specimen) UMB Umbilical blood use SNOMED CT 122556008 Cord blood specimen (specimen) URNS Urine sediment use SNOMED CT 122567009 Urine sediment specimen (specimen) URTH Urethra use SNOMED CT 119393003 Specimen from urethra (specimen) USUB Unknown substance use SNOMED CT 119324002 Specimen of unknown material (specimen) WBC Leukocytes use SNOMED CT 122584003 Leukocyte specimen from patient (specimen) RBC TLNG TSMI ULC Polymorphonuclear neutrophil specimen (specimen) Erythrocyte specimen (specimen) As a next step we will reach out to the HL7 Terminology Authority to get clarity on the process of migrating HL7 tables to external coding systems, but that is outside this CR. 2 OPEN ISSUES How best to indicate to use SNOMED CT for concepts that are not officially in the table, but that folks are used to seeing in HL70070. After reaching out to HL7 Vocab WG and the HTA to resolve how best to indicate use of SNOMED CT for missing terms, besides the one we agreed to add, this was the recommendation received (paraphrased from the emails by Jim Case, Ted Klein and Rob McClure): … the immediate solution is to get the old terms that are NEEDED added to table 487. This is because the plan to address a migration to snomed needs to be developed ; users do note like mixed code system value sets ;there is no publication or transition plan for the tables yet. A stopgap would solve Page 13 of 20 the immediate problem with the least amount of pin for implementing. The plan to gap-fill the new tables with HL7 codes from the 0070 table as a stop-gap solution until we have a finalized approach for transitioning to SCT-only value sets and/or multi-code system value sets (that would have the resulting expansion sets described in tables such as 487/488.) Heather, this puts the ball back in HTA’s court to help HL7 clarify such policies.
