BREEDE VALLEY MUNICIPALITY
SUPPLY CHAIN MANAGEMENT
In terms of section 111 and 112 of the Local Government: Municipal Finance Management Act, 2003 (Act 56 of 2003) each municipality
must have and implement a supply chain management policy which gives effect to the provisions of the Act. The policy must be fair,
equitable, transparent, competitive and cost-effective and must comply with the Municipal Supply Chain Management Regulations as
published under General Notice 868 in Government Gazette 27636 of 30 May 2005.
In terms of paragraph 14.1 (a) of the Supply Chain Management Policy the municipality must keep a list of accredited prospective
providers of goods and services that must be used for the procurement requirements.
All interested and prospective providers of municipal goods and/or services, who have not been listed during the past six
months, are hereby invited to apply or re-apply for evaluation and listing as accredited prospective providers of the Breede
Valley Municipality.
The application forms and listing criteria are available either from the Supply Chain Management Unit, Mr. Ivin Witbooi, Stofberg Huis,
23 Baring Street, Worcester; or can be requested in writing from The Municipal Manager, Supply Chain Management Unit,
Private Bag X3046, Worcester, 6849; or can be requested by e-mail from iwitbooi@bvm.gov.za / cmatthys@bvm.gov.za; or requested
telephonically at 023 348 2795 (Mr. I Witbooi or Ms. Chantal Matthys); or downloaded from the website: www.bvm.gov.za
Duly completed and signed forms, with the required documentation, must be submitted to the above mentioned office or mailed to the
above mentioned postal address. Faxed and e-mailed forms and documentation are not acceptable and will be rejected.

VOORSIENINGSKANAAL BESTUUR
In terme van artikels 111 en 112 van die Plaaslike Regering: Munisipale Finansiële Bestuurswet, 2003 (Wet 56 van 2003) moet elke
munisipaliteit beskik oor ʼn Voorsieningskanaalbestuursbeleid en moet sodanige beleid ooreenkomstig die bepalings van die Wet
geïmplimenteer word. Die beleid moet billik, regverdig, deursigtig, mededingend en koste-effektief wees en moet ook voldoen aan die
Munisipale Voorsieningskanaalbestuursregulasies soos afgekondig per Algemene Kennisgewing 868 in Staatskoerant 27636 van
30 Mei 2005.
In terme van paragraaf 14.1 (a) van die Voorsieningskanaalbestuursbeleid moet die munisipaliteit ʼn amptelike lys van geakkrediteerde
waarskynlike verskaffers van goedere en dienste byhou wat vir verkrygingsvereistes gebruik moet word.
Alle voornemende en belangstellende verskaffers van munisipale goedere en/of dienste, wat nie gedurende die afgelope 6
maande gelys is nie, word hiermee genooi om aansoek of heraansoek te doen vir evaluering en om gelys te word as
geakkrediteerde waarskynlike verskaffers van die Breede Vallei Munisipaliteit.
Die aansoekvorms, asook die kriteria waaraan voldoen moet word, is beskikbaar by die Voorsieningskanaalbestuurseenheid, Mnr. Ivin
Witbooi, Stofberg Huis, Baringstraat 23, Worcester of kan op skriftelike versoek verkry word van die Munisipale Bestuurder, Privaatsak

Die volledig voltooide en getekende vorms, tesame met die vereiste dokumentasie, kan by bovermelde kantoor ingedien word of per pos
na bogemelde posadres gestuur word. Vorms en dokumentasie wat per faks of e-pos gestuur word is nie aanvaarbaar nie en sal
verwerp word.
