SENIOR SCHOLARSHIP PROGRAM APPLICATION
ELIGIBILITY REQUIREMENTS AND POLICY* All eligibility requirements are subject to the approval of the scholarship committee.*


Applicant must be a graduating senior attending college as a Freshman in either the Fall 2015 or Spring
2016 Semester. Scholarship, if awarded, must be used prior to August 2016 Fall Semester.
Applicant must be the dependent of an officer or enlisted military member (active duty, guard/reserve
assigned to Whiteman AFB, POW/MIA, retired, or deceased), residing in the local area, and graduating
from a high school within 50 miles of Whiteman AFB or home-schooled.

THE FOLLOWING INFORMATION MUST ACCOMPANY THIS APPLICATION:
(1) Completed and Signed Application
(2) A complete copy of high school transcripts signed and certified by counselors, to include grades to date,
current class standing, and ACT/SAT scores.
(3) Signed Statement of Understanding (attached).
(4) An essay. Essay must be 300 - 500 words typed / 12 point font and in response to the topic listed below.
In your opinion, what is the most important characteristic (for example, honesty, intelligence, a sense
of humor) that a person can have to be successful in life? Use specific reasons and examples from
your experience as a military dependent to explain your answer. When you write your answer, you
are not limited to the examples listed in the question.
(5) A valid military ID is required for receipt of this scholarship. The winner will be required to present a
valid ID to the scholarship committee for verification before the award can be presented.
(6) A letter of recommendation from an adult that is not a relative.
(7) A self-addressed stamped envelope.

SENIOR SCHOLARSHIP PROGRAM APPLICATION
Guidelines for Scholarship Application – High School Senior
1. The student application process is judged on the following criteria: Academics, Extracurricular,
Community Service/ Volunteer Activities, Honors/ Awards, Leadership, Employment, and Essay.
Please be complete when you fill out your application, as there are no interviews. This is what you
will be judged upon.
2. Academics are based on GPA and difficulty of classes taken. All applications MUST BE accompanied
by an Official Transcript. Transcript must be sealed by School Official and received by Scholarship
Committee unopened.
3. The essay question is judged based on content, grammar, spelling, and overall impression. Make sure
to completely answer the question asked! It is also important to have someone else proofread your
essay. Essays should be double spaced (300-500 words typed) in 12-point font, and presented on a
separate sheet of paper.
4. You may include a photocopy of any award or recognition that supports your achievements and awards
listed e.g. photocopy of Dean’s Listing, certificates etc.
5. Please include a brief biography for the Awards Luncheon and Article in Base paper in the event you are
a scholarship recipient e.g. hometown, college planned to attend, degree seeking etc.
6. Please submit all requirements in a 9x12 envelope or larger. Please keep all documents in order. Please
remember to include a self-addressed stamped envelope.

SENIOR STATEMENT OF UNDERSTANDING
All applications must be postmarked for return prior to and received by the Whiteman Spouses Club no later than
Saturday, March 14, 2015. No late applications will be accepted. Winning applicants will be notified and presented
their award at an official event on April 25, 2015.

If for some reason a selected applicant cannot accept the scholarship (i.e. receives a full scholarship from
another source, or does not enroll in school) the funds revert to Whiteman Spouses’ Club and will be awarded
to an alternate applicant (if one is available), or the funds will be held over for award during the next
scholarship awards process the following season.
2.
Funds will be paid directly to the school where the student is enrolled, to be applied towards the student’s
account for tuition, fees, or other bona fide academic expenses. It is the responsibility of the student to
provide Whiteman Spouses’ Club with proof of enrollment and the address to which the funds should be
sent.
3.
Scholarship award checks will be sent out on or about August 1, 2015. Funds must be used before June 30,
2016, or they will revert back to the issuing organization. Requests for extensions of this deadline must be
made in writing to the scholarship committee.
4.
All submitted materials become the property of the scholarship committee and will be retained for one full
year from the date of selection. All sponsor information will be kept confidential and used for no other
purpose. At the end of that year, all applications and supporting documents will be destroyed.
5.
Selection as a scholarship recipient automatically gives Whiteman Spouses’ Club permission to use the
student’s name and photograph in any publicity releases.
I hereby authorize the Whiteman Spouses’ Club scholarship committee and an independent selection panel of
judges to examine the application that I have submitted. I understand that this application and supporting
documents will be kept in strictest confidence and is required for purposes of scholarship consideration. I
certify that the information contained herein is accurate to the best of my knowledge and that my essay is
entirely of my own effort. I also acknowledge that I have read and understand the policies and provisions as
outlined on the application form and in the Statement of Understanding.
