Listed below are details to know BEFORE you complete the online AOA Fellowships Application. Once you begin the online application, you will be able to save and return to the application, however, you will not be able to go back to previously completed sections. The preferred browser for completing the AOA Fellowships Application is Google Chrome. You will need complete the following sections when applying online. You may upload your responses to each question, or you can type in the answers where applicable: How you heard about this traveling fellowship CV in the requested format (see below) – Must be uploaded Describe your areas of special interests in orthopaedics (research, clinical interests, other – in 200 words or less) Briefly, describe your future career plans and goals (in 200 words or less): Please provide a personal statement describing what sets you apart as a candidate for this Traveling Fellowship. (250 word limit) A recent digital photo – Must be uploaded A total of two (2) sponsors must send letters of recommendation to the American Orthopaedic Association on your behalf. One sponsor should be either your chair or division chief (or the head of your practice group), and the other should be an orthopaedic surgeon who has been familiar with your work for at least the past three years. Letters can be sent separately, or downloaded at the time of completing the online application. Note: You must have both (2) letters to upload when completing the online application, otherwise please check the box that your letters will be mailed or emailed. The online application with all attachments outlined above must be completed and received by January 31, 2016. Incomplete applications, or those received after that date will not be considered. Complete your curriculum vitae on a separate attachment, according to the format listed below with the headings in the following order. If there is a section that does not apply to you (e.g. Military Service dates) this section should be omitted from your CV and proceed to the next section. NOTE: Your CV must be attached to the online application at the time the application is being completed. 1 Name of applicant. 2. Education/Post Graduate Education (List Residency). List the College or University you graduated from including date, as well as Post-Graduate Education and the dates. 3. Additional Education or Fellowship. List type of Education or Fellowship, the name of Director and the dates. 4. Date of ABOS certification. 5. List Military Service dates (if applicable). Please list Branch of service/Rank/Duties. 6. Professional Employment/Teaching Affiliations. Please list in chronological order the Academic Title, Teaching Responsibilities and the dates. 7. Special Awards and Honors: List special awards and honors received while in medical school, residency, fellowship or following the completion of your educational program. 8. Membership/Leadership Positions: List membership or leadership positions you have held on National, Regional or Local committees or in professional organizations. 9. Peer-reviewed Articles (Published and In Press): List articles published in peer-reviewed journals. Include the title, all authors, journal, and year of publication. Please underline your name and capitalize the name of the journal. You may also list articles in press but not those that are submitted only; please indicate whether they are published or in press. 10. Non-Peer-Reviewed Articles: List articles published in non-peer-reviewed journals (include internet related articles here). 11. Textbooks and Textbook Chapters: List textbooks or chapters in textbooks that you have authored or coauthored. Identify title, publisher, all authors, and the year. 12. Other Media: List movies, sound slide programs, exhibits, audio tapes, and video tapes which you have developed or co-developed. List the scientific meetings where each has been presented. 13. National Scientific Presentations: List scientific presentations that you gave at national meetings (include title of paper, all authors, organization, location and date). 14. Regional and Local Scientific Presentations: List scientific presentations that you gave at regional and local meetings (include title of paper, all authors, organization, location and date). 15. Peer-Reviewed Research Grants: List peer-reviewed research grants you have received (i.e. NIH, OREF), and the amount and source of each grant. List your role on grant (PI, co-PI, investigator). 16. Non-Peer Reviewed Research Grants: List non-peer-reviewed research grants you have received (i.e. Industry), and the amount and source of each grant. List your role on grant (PI, co-PI, investigator). 17.
