Welcome to Karon McConnell who has recently joined Hartwood Healthcare as the Practice Pharmacist.
As a pharmacist prescriber, Karon’s role involves prescribing repeat medication, dealing with prescription
requests and completing medication reviews. Her role is to support GPs in ensuring patients are taking the
most appropriate medication and to ensure patients attend for regular reviews of their long term
conditions.
MEDICATION REVIEWS
A full medication review should be carried out routinely, at least annually, on all patients.
The medication review is an opportunity for the doctor to review the appropriateness of the patient’s
drug treatment.
A more frequent review may be more appropriate depending on the patient’s clinical condition. If a
patient fails to attend for a medication review, then the standard policy is to issue only one month
extra as an emergency prescription, with a highlighted message on the right hand side of the
prescription inviting the patient to attend for review.
Would you like to have your say about your surgery…...What works well? What
needs improving?
We are looking for patients to be actively involved in our patient group. If you are
interested please speak to one of our receptionists.
Would you like to be a volunteer?
Hartwood Healthcare, together with RSVP (Retired and Senior
Volunteer Program), is creating a patient befriending scheme to help
combat feelings of loneliness and isolation in elderly people.
People are living longer and over half of 75-year olds live alone and
have few friends or relatives nearby.
Our volunteers may visit the less mobile and elderly patients for a cup
of tea and a chat whether it be regularly or on an adhoc basis.
If you are interested in finding out more about becoming a volunteer
please pick up a leaflet from reception.
SELF CARE
Hartwood Healthcare and Hillview Family Practice are holding a Self Care week
between 16th and 22nd November where information will be available on how to take
care of yourself over the winter months.
Colds and Flu are caused by a virus, so antibiotics do not work. You may have a
cough, runny nose and general aches that can last a week. Drink plenty of fluids and
take pain relief.
Diarrhoea and Sickness is normally caused by viruses and you should take small
sips of water to replace fluids. Take extra care with small children.
