CONCURRENT DISORDERS SPECIALIST- CASE MANAGER Strengthening Communities in Scarborough PERMANENT FULL-TIME EXTERNAL POSTING The Canadian Mental Health Association Toronto Branch is a non- profit, community- based mental health agency providing a wide range of services across Toronto. The Strengthening Communities in Scarborough team (SCS) is a new intensive case management team to serve individuals with complex mental health and addiction needs from a recovery- based philosophy. This team is a multidisciplinary team which will provide individualized supports in the community to increase mental health and physical well- being and housing tenure. The aim will be to improve the experience of individuals with complex mental health and addictions issues in accessing housing support, and their utilization of appropriate services across the broader health and social services system. This project will include immediate, short-term and long-term supports based on client needs. CMHA Toronto is seeking a Case Manager- Concurrent Disorders Specialist for this team. As a member of this team, you will be providing support for diverse people with mental health and addictions issues according to their individual needs. The CD Specialist will provide community- based support and linkages to other services. The CD Specialist may also be asked to provide individual and group counseling and consultation on concurrent disorders to other CMHA programs. KEY RESPONSBILITIES: Orientation to program and agency Client-centered engagement, service provision and interventions Skills development and goal planning Providing resources, information and advocacy Maintaining a trusting, respectful and collaborative working relationship Monitoring, evaluation and follow-up Individual and group addictions counseling and therapy Consultation to other staff on addictions- related issues QUALIFICATIONS: Minimum Bachelors level education in Social Work/Social Science & Addictions Training 2 years of direct service experience in community mental health and addictions Experience in working with diverse communities Ability to demonstrate a non-homophobic and anti-racism approach Knowledge and working experience with Intensive Case Management and Housing First Model and recovery-oriented practice Working knowledge of the OCAN assessment tool Familiarity with Motivational Interviewing Excellent organizational skills and the ability to work independently in a dynamic team environment Fluency in a second language is an asset Primarily day hours with occasional evenings A valid Ontario driver’s license is required.
