Outreach Program A unique and innovative program designed to provide specialized service to adults with health needs. Who we are RH Healthcare is the organizational sponsor for the Disease Specific Outreach Program. The outreach program is part of cardiology Service. As part of the broader regionally based service, we are able to tap into the resources and expertise found within our services, We provide a continuum of specialty cardiac health services of clinical assessment and intervention, education and community development through: Consultation and Partnership We work collaboratively with our care partners in assessment, treatment and intervention phases of service. It is our goal to view the whole person, stabilize the presenting issues and enhance the quality of life for the senior and their family/caregivers Education and Service Development we are committed to deliver and participate in educational, learning and community development initiatives. Respect and Advocacy We respect the experience of the seniors and families living with cardiac illness and/or other health issue. We strive to provide input for positive change . Research and Evaluation We strive to be active contributors to the ongoing research, evaluation, discovery and translation of new knowledge in the delivery of effective and efficient health services for labor camp individuals Mandate & Funding The mandate of the Disease specific ((Outreach Program)) is to provide and enhance continuity of care for labor camp individuals with or with out health problems, by: Helping to identify, understand and support the needs of labor camp individuals with or with out health problems, Providing specialized, comprehensive individual assessments and collaboratively developing individual care/service plans to maximize functioning and quality of life; Establishing linkages, partnerships and alliances with local and regional community services, caregivers, the client and family; and Educating health and social service providers to build community and system capacity. Target Population We serve labor camp individuals regardless their age, nationality, and over residing in the regions of Dubai and Jabal Ali + some population like shopping mall, airport, University and hospitals. Our Model of Service The model of service for the Disease specific ((Outreach Program)) can be best described as a flexible approach; striving to understand the experience of labor camp individuals and others; sharing, and partnering with primary care, Hospital; appreciating the service and system limitations and strengths; optimizing the clinical abilities and resources internally and externally across our service partners and the system; positioning our service as a resource; and demonstrating a commitment to building capacity within ourselves and others Our team works in a trans-disciplinary manner with training in: Cardiology /medicine Nursing Social Work Dietician Education Responsibility for delivering primary health care, diagnostic and educational to the labor camp individuals and others; and Interventions can include health teaching, supportive counseling, Dietician, person-centered monitoring and tracking, advocacy for services and/or supports, Our Vision RH Disease specific ((Outreach Program)), a satellite program of RH (cardiology department, is a unique and innovative program dedicated to all the labor camp individuals and others and their caregivers. We serve labor camp individuals regardless their age, nationality, and over residing in the regions of Dubai and Jabal Ali We provide clinical/education outreach services to those labor camp individuals and others who cannot or will not seek traditional service e.g. going to a hospital/clinics. Included area Region of Halton, i.e. Burlington, Oakville, Milton, Acton and Georgetown Northwest Mississauga covering the area north of the QEW, west of Mavis Road and south of the 401 or as determined by postal codes: L5K, L5L, L5M, L5N, L5C, L5V, L5W In the Mississauga Halton LHIN, St. Joseph's Healthcare Hamilton and Trillium Health Centre share in providing geriatric mental health outreach services. Although each program has a defined geographic catchment area we are flexible and work together to best match the resources and meet the needs of the senior and their family/caregivers. What does outreach mean? Outreach means that we come to your home, whether in the community, retirement home or long term care, to do the assessments and follow up visits. Outreach services are for those high risk seniors who cannot (e.g. poor physical health, limited mobility, at risk for falls, limited or no family support etc.) or will not seek traditional or office-based services (e.g. senior may be very reluctant and lack insight into their problems due to illness). Outreach also includes providing education on seniors' mental health, addiction and/or behavioural issues to caregivers, healthcare providers and family members. Seniors with mental health difficulties often benefit greatly when they, their family members, caregivers and/or healthcare providers are well-informed about mental health and available services, treatments and support Explore this department Department Overview » Conditions Treated » Eligibility Requirements » Cardiac Outreach Program - An Overview The Outreach Program at St. Francis Hospital is dedicated to screening patients who don’t have access to care for heart disease, including coronary artery disease – a major cause of heart disease today – and its associated risk factors. Since 1989, our program has been dedicated to this mission. We specialize in risk factor assessment and the early detection of heart disease in underserved communities where patients may not have access to care. We also seek to educate communities through literature and referrals to community health programs. Be 18 years old or older. Not be currently (within the last 6 months) under the care of a physician or clinic. Not be covered by Medicare insurance. Not be covered by Medicaid insurance, or if have Medicaid, must not be currently seen on a regular basis in a clinic or by a private physician. 5. Not be taking prescription medication(s). 6. Not be able to afford the cost of a medical checkup, paid out of pocket (approximately $75 for the physical exam and $200-300 for lab work & EKG). 7. Not have been previously screened by the SFH Cardiac Outreach Program. Our mobile clinic evaluates eligible patients for heart disease and for the risk factors that cause heart disease. Once we identify certain conditions, further cardiac services are provided at St. Francis Hospital.
