LORI LAMB LEGACY GRANT Deadline for submission is: April 1st, 2014. Purpose of the Scholarship The Lori Lamb Legacy Grant was established 2011 in memory of Lori to recognize an outstanding MHS senior and to motivate all students toward their best academic ability and athletic participation for intellectual and physical fitness. Each spring a student will be selected from eligible applicants for a scholarship for university tuition beginning with the fall semester. The scholarship will be awarded based on the following application and scholarship guidelines. Scholarship Requirements The Lori Lamb Legacy Grant will be awarded to graduating seniors meeting the following requirements: 1. The student must be a graduating senior of Macksville High School and plan to attend one of the below mentioned Kansas 4-year Universities in pursuit of a Bachelor’s Degree. Kansas Universities are as follows: Kansas State University, Kansas University, Fort Hays State University, Emporia State University, Pittsburg State University and Wichita State University. Accredited 4year state colleges and universities in other states may be considered by the selection committee upon request by the applicant. 2. Graduating senior must have a grade point average of 3.0 or better on a 4-point scale. Additionally, Math, Science, and English classes must be a minimum GPA of 3.0 or better (each subject) during high school. 3. Graduating senior must score at least a 23 on the ACT or its equivalent on the SAT. 4. Graduating senior must have participated in at least one MHS athletic program for 3 out of 4 high school years. 5. A recommendation letter from a coach of a sport in which applicant participated must be supplied. The letter should assess the applicant’s sportsmanship, discipline, unselfishness, team support, hard work, and general character as an athlete. 6. Favorable character recommendation letters from at least three teachers from Macksville High School whose classes’ applicant attended must be submitted. 7. Graduating senior must submit a written essay including the following information: the course of study student is pursuing and reasons for choosing that course of study; reasons for college selection; strengths the applicant believes he or she possesses; the areas the applicant hopes to improve and how he or she thinks that can be done; whether extracurricular activities and/or community service will be a part of their college experience; how getting the degree they seek will serve their goals as an adult; and any other information the applicant wishes to include that they feel would assist the selection committee in making their decision. *No mandatory format or length is required, however the essay must be the applicant’s own work and will be a significant part of the selection process. 8. Signature on Personal Attestation Form attesting to certain requisite behaviors and authorizing use of specific items being provided by applicant. “Unlawful acts” shall mean a knowing action in violation of a law, civil or criminal, as well as declining or refusing to correct an unlawful status or condition. Renewal of Scholarship Requirements The Lori Lamb Legacy Grant is intended for an academic year of two semesters and the requirements below pertain to each of the semesters an applicant receives the grant. A MHS senior who is selected to receive the grant may receive grants for a total of eight consecutive semesters of undergraduate study leading to a Bachelor’s Degree or pre-graduate program such as pre-law, pre-medicine, or other such graduate degree with designated under-graduate programs, provided that the following requirements are met. Grants will be between $1,000.00 and $3,000.00, yearly, depending on fund support and advancement. 1. Recipients must maintain a cumulative 3.0 college grade point average or better. And, a 3.0 or better term GPA for the preceding semester is required for renewal for the coming semester. Consecutive semester renewal is required to remain eligible for subsequent semesters. The student is required to forward a copy of his or her semester grades to the SCCF office each semester. 2. Recipients must maintain a full time academic load. 3. If the student fails to meet the GPA requirements, character requirements or withdraws from college, the scholarship will be terminated without further opportunity to re-establish eligibility. 4. Renewal applications expectations: a. Updated picture b. Personal report updating progress in school and activities that you have been involved in. c. Report due to SCCF Office no later than November 15th. 5. Subsequent year scholarship awards are dependent upon maintaining high standards of academic excellence and exemplary character. Determinations of qualifications are within the discretion of the selection committee and the South Central Community Foundation.
