Municipal and Community Affairs Regional Youth Sport Events Contribution Program 2012 – 2013 APPLICATION FORM Introduction The Department of Municipal and Community Affairs (MACA) recognizes the importance of regional youth sport events. The intent of the Regional Youth Sport Event Contribution Program is to strengthen local programming, support athletes and coaches in smaller communities, and improve youth participation in physical, leading to positive lifestyles. MACA encourages sport and recreation organizations, community governments and school authorities to organize and host regional youth sport events. The maximum amount of funding available to any one event is $50,000 per fiscal year. Applications must target a minimum of ten (10) youth and a minimum of two (2) communities in addition to the host community. They will be weighted to favour events that target an underrepresented segment of the population. Further details on eligibility can be found in the Regional Youth Sport Events Contribution Policy. Applicants are encouraged to work with representatives of the Department of Municipal and Community Affairs during the application phase to ensure their applications comply with all funding requirements. Contact information can be found at the end of this application form. Your application for a financial contribution will be reviewed and assessed on the basis of the following information. The deadline for this year’s application is April 30th, 2012. Terms and Conditions MACA will send a notice of approval or rejection to the contact provided. The financial contribution will be issued as a contribution transfer agreement. By signing this application form you acknowledge that you were made aware of the following terms and conditions that will be part of the written agreement. The event or project will comply with the provisions set out in the Regional Youth Sport Event Contribution Policy. The applicant has comprehensive liability insurance no lower than $2,000,000 in place. MACA encourages applicants to take event photos and submit photos with the report. In order to do so permission must be obtained for the GNWT to use these images to promote physical activity and healthy life styles. MACA can provide consent forms to the applicant. The summary report template required will include evaluation criteria that the applicant is required to fill out. This reporting template will be included in the contribution agreement. Applications will be weighted to favour events that target underrepresented segments of the population. Funding allocation will be prioritized in favour of those projects which assist rural and remote communities to improve their own sport and recreation services. The funding must be accounted for by sending the following documents to MACA no later than 60 days after the event takes place: 1. a summary report detailing how the funding was utilized using the template provided by MACA; and 2. a financial report showing all revenues and expenses related to the event. Copies of original receipts must be retained on file should the Department chose to audit the project at a later date. Budget The following expenses are eligible: travel and costs incidental to travel; accommodations and meals; rental of sport and recreation venues; premises suitable for sleeping quarters; repairs or purchase of sport equipment or training tools to a maximum of $5,000 per year; and communications including advertising.
