the last year (in 2014) with the
programs at Support & Housing –
Halton. Your feedback is important to
us and will be used, along with the
feedback of others, to help us make
improvements to our programs where
needed.

You can do this survey either by completing this form or by going online and completing the
web version of the survey, whichever you find easier. Either way, your answers will remain
anonymous. Only the researcher doing the analysis of the survey will see your answers. She will
be developing a report that summarizes the responses from everyone who completes the
survey. There is no identifying information on the survey so, while she may use quotes from
some of the surveys in her report, at no point will any individuals be identified.
If you choose to do this paper version of the survey rather than doing it on-line, when you have
completed it please place it in the envelope it comes with, seal the envelope and either give it
to your Support Coordinator or put it in the mail. NO POSTAGE IS REQUIRED (it is prepaid.)
Envelopes containing surveys will be given to the researcher unopened and the paper copies of
the surveys will be destroyed after the researcher has completed the analysis of everyone’s
responses.
If you are doing the survey on-line, once you have completed it and clicked on “Done”, there is
nothing else you need to do.
