Job Summary:
The LPN Shift Supervisor is responsible for overseeing all care delivery and coordination of care and
services for residents on their assigned shift. In the absence of the Resident Care Director, assumes
supervisory responsibility for the Resident Care staff and department. The LPN Shift Supervisor will
promote hospitality and deliver a high level of customer service at all times.
Duties and Responsibilities include but are not limited to the following:
Resident Care
1. Complete all move in paper work and assessments on new residents and those re-admitted
from the hospital, rehab or other settings.
2. Receive and process all physician and ancillary orders.
3. Coordinate the ordering and receipt of medications from the pharmacy.
4. Complete rounds each shift on new residents and those identified on the communication log or
via verbal report with condition changes or clinical concerns.
5. Observe residents for any mental or physical changes in condition, identifies the need for
medical attention, takes appropriate action including notifying the Resident Care Director,
family and physician. Assists with PRN medications based on observed need.
6. Assists with the identification of all at risk residents (falls, elopement, skin impairment) and
provides necessary, timely interventions under the direction of the Resident Care Director.
7. Follow up with resident’s physician and family regarding changes in condition or need for
medical evaluation or treatment.
8. Assist with the completion and update of resident service plans and level of care assessments as
assigned.
9. Assist with the medication administration process as needed and assigned.
10. Perform ongoing audits of the medication cart as assigned.
11. Ensure written daily assignments of resident care and environmental tasks are completed with
lead care manager.
12. Ensures all Resident Care staff carries pagers or scout phones and respond to resident calls for
assistance in a timely manner. Assists with answering calls as necessary.
13. Maintains Resident’s Rights while providing care.
14. Follows Standard Precautions policy using proper protective equipment and hand washing as
needed.
15. Assist with the delivery of resident care and treatments as needed.
Documentation
1. Complete incident reports and any other required documentation for resident or employee
injuries, per community policy.
2. Records vital signs and weights at time of move in, monthly and more frequently if required by
physician orders.
3. Update resident’s condition at least monthly in resident’s chart.
4. Record necessary information in resident chart related to changes in condition as these changes
occur. Document necessary resident information per state regulatory requirements.
5. May complete initial move in progress note for resident’s health record.
6. Keeps necessary documentation related to vital signs, weights and blood glucose readings and
provides this information to resident’s physician as requested.
7. Prepares and sends necessary documentation for residents going to health provider
appointments outside of the community.
8. Documents appropriate discussions with the resident and family in the progress notes.
9. Maintain medication refrigerator temperature log.
10. Assist with completion of laboratory requisition slips per community policy and state guideline.
11. Complete monthly re-cap of physician orders and MAR’s.
Communication
1. Communicate any resident change of condition to Resident Care Director or to family and
physician if necessary in the absence of the RCD. Document changes in progress notes and
communication log.
2. Record relevant information in the Communication Log/shift report at the end of each shift.
3. Provide verbal report to oncoming shift including relevant resident care information.
4. Communicate resident concerns to other departments within the community.
5. Provides oversight and coordination in the absence of the Resident Care Director and other
department heads and communicates concerns as needed.
6. Maintains confidentiality of resident information in all interactions.
7. Assists with receptionist duties as needed; forwards calls to various departments and takes
messages for follow-up.
8. Assists third party providers with obtaining pertinent information needed to provide services to
community residents.
Supervisory Responsibilities
1. Promote a team concept with care staff in all areas of the community.
2. Provide RA Task Sheet assignments to Resident Assistants at the start of each shift. Adjust
assignments as needed based on residents needs and number of care staff on duty.
3. Lead change of shift report in the absence of the Resident Care Director.
4. Provide oversight and coordination during emergency situations in the absence of the Resident
Care Director in accordance with community policies.
5. Provide oversight for the emergency response system in the absence of the Resident Care
Director.
6. Provide conflict resolution with situations involving staff members.
7. Ensure that all work assignments are completed according to schedule and that job duties are
carried out in a safe and effective manner.
8. Follows community policy for reporting job related injuries informing the Resident Care Director
immediately.
9. Assists supervisor by providing input in performance appraisals of Proxy Caregivers and Resident
Assistants.
10. Assists in the orientation and training of staff including training on new policies and procedures
as directed by the Resident Care Director.
11. Addresses disciplinary problems immediately. Informs the Resident Care Director and completes
disciplinary action notices per community policy with direction from the Resident Care Director.
12. Follows community policy for ensuring adequate number of staff is scheduled for on-coming
shift and documents staff absences per policy. Keeps the Resident Care Director informed of
staffing issues.
Customer Service
1. Provides a 30 second commercial to prospective residents during initial tour of community.
2. Introduces self to new residents and families at the time of move in.
3. Utilizes proper customer service skills during telephone conversations with families, physicians
and other outside parties.
4. Spends time each day interacting with residents and families as necessary to identify any areas
of concern, alleviate anxiety and to orient to current environment.
5. Address any conflicts that arise from staff, residents or families in the absence of the Resident
Care Director.
6. Promote a team concept with Care Staff in all areas of the community.
7. Use the Positive Physical Approach at all times.
Essential Knowledge and Abilities
1. Ability to read, write and speak English.
2. Working knowledge of basic resident care procedures used to provide physical and social care to
residents.
3. Working knowledge of safe work practices and procedures to include fire safety, first aid,
universal precautions, appropriate body mechanics, etc.
4. Working knowledge of Resident Rights and state regulations.
5. Ability to follow written or oral instructions and communicate effectively with residents and
staff
6. Ability to carry out responsibilities according to company or state guidelines and with limited or
no supervision.
7. Ability to lift up to 50 pounds and the ability to safely lift or transfer a resident.
8. Ability to function as a member of a team.
9. Ability to demonstrate outstanding customer service skills
10. Ability to attend required in-service training and staff meetings and maintain required
certifications according to state regulations.
11. Ability to stay beyond scheduled shift in the event of emergency or inclement weather.
12. Ability to provide a working contact telephone number at all times
Training and Experience
1. Graduation from accredited LPN program.
2. Current LPN license in state of community residence in good standing.
3. Six to twelve months experience in related field is preferred.
4. Supervisory experience or training course completion preferred.
