Canine Boarding Admission Form

Boarders need to be dropped off and picked up between the hours
of 7:30 a.m. and 6:00 p.m, Monday-Friday or between 8:30 a.m.
and noon on Saturday. We do not have Sunday pick-up or dropoff times.

Canine Boarding Rate: $21 per night. Payment is expected in full at time of your pet’s discharge. We
accept cash, personal check, MasterCard, Visa, Discover and American Express.

Please understand that bathing by kennel staff is very basic- consisting of a warm-water bath, drying,
and a brief (5-10 minute) brush out.
Occasionally, some pets develop diarrhea due to stress while boarding. This condition is not
uncommon and usually responds to minimal medical treatment. There is a fee for the
medication dispensed. Should your pet have this problem, would you like us to begin
treatment or would you like a phone call first?

In the event of a serious medical condition, you will be contacted. If we are unable to reach
you, your pet will be treated at Doctor’s discretion until you can be reached.
Please note: For the best interest of all pets under our care, all patients will be examined for
external parasites (fleas, lice, and ticks) while here. If parasites are found the pet must be
treated for them to prevent spread to other patients. Our staff will explain if any additional
follow up care if necessary.
North St. Paul Animal Hospital Boarding Policy- “We treat your pet as if it were our own”

All dogs will be walked three times daily.

Fresh water will be provided daily.

Clean bedding will be provided daily by hospital as it can be washed and sanitized.
I, the undersigned owner or designated agent, hereby authorize North St. Paul Animal Hospital (hereinafter
“Hospital”) to board my pet during the dates listed above. I also hereby authorize the Hospital to perform the
services indicated above while my pet is boarding. The Hospital will attempt to notify owner if the pet
becomes ill while boarding. If owner does not inform the Hospital otherwise regarding measures to be taken,
or if the state of the animal’s health reasonably demands quick action in the opinion of the veterinarian, the
Hospital will administer medical and/or surgical treatment as needed until the owner can be notified.
As the owner of said animal, I realize that I am responsible for boarding fees, and any associated costs, and for
the payment of services listed above and that they are to be paid in full at the time the animal is discharged. If
I do not pick up the animal within five (5) days of the scheduled pick-up date, the Hospital will assume the
animal is abandoned. If the animal is abandoned, the Hospital is authorized to remedy the abandonment as
prescribed by law. I further understand that abandonment DOES NOT release me of my financial obligation
for services rendered, fees associated with abandonment, collection action, and/or legal services.
I also understand and acknowledge that the Hospital is not staffed 24 hours a day.
