JOB PURPOSE










To provide specialist clinical technical services, professional advice and direct support to practices in order to achieve a pattern of medicine usage across the CCG that
maximises cost effectiveness and quality.
To liaise with general practitioners, secondary, intermediate, social care and community pharmacists and carers to improve the pharmaceutical care of patients.
To develop the role of the medicines management technician to maximise support
to practices to improve patient care in relation to prescribing.
Provide professional specialist medicines management advice to GPs, practice
nurses, community pharmacists, community nurses and other health professionals
to promote the safe, effective and high quality, cost effective use of medicines in all
prescribing decisions.
To identify, develop and promote quality improvements in medicines management
across community pharmacy & GP practices ensuring that best practice is adopted
Provide support for the medicines management technician(s)/facilitator(s) to deliver
the medicines management goals of the CCG
To contribute to the development and implementation of the CCG prescribing strategy and medicines management initiatives across the CCG
To encourage better pharmaceutical understanding throughout the primary health
care team, including training of GP’s practice nurses, practice staff, attached staff,
community pharmacists and pharmacy staff
Undertake risk assessment and management and ensure compliance with medicines
legislation
Liaise with prescribing colleagues primary and secondary care to ensure consistency
with District-wide prescribing strategies

To contribute to the development & implementation of new medicines management
initiatives and services within GP practices, community pharmacies and community
providers
The post holder will support the Service Director and their team to ensure that all
workstream of programmes are planned and managed effectively and take the lead
in assisting in their successful delivery.

Overview of Responsibilities
• Undertake regular prescribing audits and prescribing reviews under the management
of the CCG pharmacists to analyse patterns of prescribing at CCG and practice level in order
to inform GP practices and the CCG of prescribing trends. Utilise this information to
implement any necessary prescribing switches/changes to deliver cost-effective
prescribing
• Analyse prescription data to produce reports, audit practice prescribing procedures
such as repeat prescribing and interpret audit information in order to advise practices on
improving prescribing policies and procedures

Implement other initiatives to reduce waste
• Analyse and interpret specific prescribing information highlight potential prescribing
issues and to update practice clinical systems, liaising at the interface with secondary care,
the local community hospital, local pharmacies and nursing and residential homes

Determine accuracy of both hospital and GP prescriptions with regard to medication

• Provide independent information to patients with regard to medicines management
and through assessment initiate further support/input from other healthcare professionals
where appropriate
• Make independent decisions regarding prescribing issues in accordance with CCG
guidelines where there is a range of possible options which impact on the prescribing
budget
• Work with the prescribing team to develop medicines guidelines and policies and
deliver training and education on prescribing and therapeutics in general practice
• Implement procedural and service changes relating to prescribing to GP practice staff
to ensure best practice is adopted in accordance with local guidelines and legislation
• As part of the prescribing team ensure the provision and distribution of information
to the primary healthcare team including GPs, practice managers, practice and district
nurses, local community pharmacists and their patients
• To link into local clinical governance structures to report on risk management issues
relating to the prescribing process
• To establish and maintain an effective working relationship with practice staff and
other healthcare professionals in order to influence prescribing behaviour according to
local CCG policy
• To support the primary care pharmacists in the planning and delivery of medicines
management services across the CCG.
• To work within the full range of technical pharmacy and codes of practice and
protocols in order to maximise pharmaceutical support to practices.

Data Protection

a range of policies for the use of computer equipment and computer generated
information. These policies detail the employee’s legal obligations and include references to
current legislation. Copies of the Policy on the Protection and use of Personal Information
and other Information Technology can be found in the PSS’s Policies and Procedures
Manual.

Risk Management
You will be responsible for adopting the Risk Management Culture and ensuring that you
identify and assess all risks to your systems, processes and environment and report such
risks for inclusion within the PSS’s Risk Register. You will also attend mandatory and
statutory training, report all incidents/accidents including near misses and report unsafe
occurrences as laid down within PSS’s Incidents and Accidents Policy.

No Smoking Policy
PSS is a ‘no smoking organisation’ therefore staff are not permitted to smoke whilst on duty.
All PSS staff are expected to recognise their role as ambassadors for a healthy lifestyle. As
such, PSS staff should not smoke whilst in uniform, in NHS vehicles or on PSS or other health
care premises.
Other Duties
There may be a requirement to undertake other duties as may reasonably be required to
support PSS. These may be based at other PSS managed locations.
