CONTEXT
Crossroads Care co-ordinates and delivers a service which empowers and supports carers to
achieve resilience in their caring role. We have received funding to deliver an innovative new
project in East Hertfordshire that will enable people living with dementia to live well in their own
home. This will be achieved through adjustments to their home environment to ensure it is as
supportive as possible in creating independence in a safe and healthy way.
Such adjustments may involve improved lighting, painting internal walls/doors using appropriate
colour differentiation, using colour differentiation for objects creating distinction and definition for
everyday household items, label cupboards and objects with pictures and words so that they
can be identified, removing visual clutter and distractions such as carpets with floral patterns.
JOB PURPOSE

To process referrals and liaise with The Alzheimer’s society and EMDASS and ensure
the continuation of a constant client base.

Co-ordinate the assessment of needs and delivery of home adaptations to carers home
across East Hertfordshire.

MAIN RESPONSIBILITIES









Maintain a referrals process through Herts Help.
Process the client data and create a database saving key statistics for KPIs on each
client, Age, DOB, Gender, Length of time since diagnosis etc.
Ensuring the Evaluation via the Warwick and Edinburgh Mental Health Wellbeing Scales
(WEMWBS) criteria is undertaken on the secondary contact two weeks subsequent to
the home visits.
Evaluation and analysis of the information taken on the WEMWBS evaluation
Using the KPIs to determine the statistical value of the project.
Signposting clients to appropriate agencies to ensure their ongoing needs are met i.e.
Home Help, Shopping etc.
Liaise with colleagues and 3rd parties such as Alzheimer’s Society and EMDASS to
support carers and their homes where adaptations would benefit them to live well with
dementia.
Liaise with colleagues and 3rd parties, including trades people to ensure assessments of
needs are carried out and adaptations are carried out.
Liaise with volunteers to deliver some of the ongoing needs of families in the home i.e.
gardening, home help, voluntary sitting services.

Criminal convictions
 This appointment is subject to a satisfactory enhanced disclosure from the
Disclosure and Barring Service (DBS). Where appropriate this check will also show
whether a person is barred from working with vulnerable adults and / or children.
Because of the nature of this work you need to provide information about any
criminal record you have, spent or unspent. This includes cautions, reprimands and
warnings.

Amendments to the Exceptions Order 1975 (2013) provide that certain spent
convictions and cautions are ‘protected’ and are not subject to disclosure to
employers, and cannot be taken into account. Guidance on criteria on the filtering
