FCCMC SCHOLARSHIP AWARD 2015
Purpose:
To provide scholarships to ten (10) deserving high school graduating seniors who
plan to continue their education by attending a college/university or other post
secondary educational institution. The scholarships are in the amount of $1000.
Criteria
1. Applicant must be a graduating high school senior in the year the scholarship is
awarded.
2. Applicant must have a minimum GPA of 2.5 to be considered for the award.
3. Applicant must show evidence of community and church involvement.
Process
(Please ensure that application is completed in its entirety).
Applicants must submit the following items:
1. Completed application form (please type or print legibly, using black ink).
2. A brief biographical sketch of applicant
3. Completed essay, using one of the five questions on separate sheet of paper. (Adhere
to the guidelines within the application).
4. At least one letter of recommendation from choice of high school teachers,
administrators, counselors, pastor, employers or individuals with significant
knowledge of applicant.
5. An official school transcript or ensure completion of School Counselor
Authentication/Evaluation part of application.
6. Make sure that all required signatures are on application.
7. ALL application packets must be postmarked NLT, Friday, April 17 2015, and
submitted through the U.S. Postal Service. ABSOLUTELY NO EXCEPTIONS.
8. Please mail or submit to person listed on the application.
9. Scholarships will be awarded upon the Council’s receipt of documentation of
enrollment. The award will be made to the student, not the institution.
10. Scholarship Certificate Award will be presented at the Scholarship Awards Event to
be held on Monday, May 18, 2015.
NOTICE
All recipients are asked to attend the Scholarship Awards Banquet where they will
receive a Scholarship Award certificate. The actual money will be awarded upon
verification of enrollment in college. Student must be enrolled by second semester.
All recipients will be notified by May 8, 2014.
