Department of Occupational Therapy & Occupational Science Policy for Repeating Fieldwork Policy A student who receives a “U” in Level II Fieldwork (FW) may be given permission to take the course a second time. Level II Fieldwork must be completed within 24 months of fulfillment of didactic course work. Enrollment in Level II Fieldwork is contingent upon demonstrated evidence of goal attainment as determined by faculty teaching the independent study/directed readings course. Procedure Those students who received a grade of “U” because of performance or professional difficulties during the fieldwork will be required to complete the FW remediation process and an independent study course and receive a grade of B or better prior to receiving permission to repeat the Level II fieldwork experience. A. Remediation Plan – The student will: 1. Meet with the Fieldwork Administrator, assigned fieldwork faculty, and/or other faculty deemed relevant by the Department Chairperson within 2 weeks of termination of the fieldwork experience to review the situation and plan strategies to improve performance. 2. Complete reflection including areas of personal and professional responsibility. 3. Develop and submit documents a. Write self-assessment integrating reflection of personal and professional responsibility with feedback from the fieldwork educator and fieldwork faculty. b. Review and discuss self-assessment with assigned fieldwork faculty member before moving forward to create the remediation plan. c. Create Remediation Plan based on the self-assessment to include the following requirements in consultation with assigned independent study faculty: 1.) Measurable goals for remediating the difficulties 2.) Implementation strategies such as self-study, laboratory experiences (e.g., role playing, skills practice). 3.) A Level I Fieldwork experience to address performance and professional difficulties. 4.) Student signature and date 5.) Signature page for the following faculty and staff to sign giving their approval of the plan: the assigned independent study faculty, Fieldwork Administrator, and Department Chairperson. Signatures must be dated and obtained in this order. 4. Adhere to timelines a. The student must submit the remediation plan to and receive approval from the Chairperson within 6 months of the fieldwork termination. b. The Fieldwork Administrator forwards the approved plan to the faculty assigned to oversee the implementation of the remediation plan. Independent Study Students are required to enroll in an Independent Study Course after the remediation plan has been approved by Chairperson. Remediation courses are only available to students in minimister and summer sessions. 1. The remediation plan will become the basis of a 3-credit Independent Study Course. (Independent Study will be completed as OCTH 318 or OCTH 692 according to undergraduate or graduate status). 2. The student will meet with the Independent Study faculty member to develop a syllabus including an appropriate grading scheme, timeframe, all activities, and student responsibility for completion following the Remediation Independent Study template. Level I FW experience included in a remediation plan is graded as satisfactory or unsatisfactory. The Independent Study Course is graded according to letter grade. 3. As with any other course, students are expected to meet assignments and course requirements and due dates. 4. All portions of the course must be completed within 6 months of remediation plan approval in order to be scheduled for a Level II Fieldwork. C. Alternate Plan of Study Student must meet with his/her advisor to develop an Alternate Plan of Study for remaining coursework and submit to the Executive Committee.
