At Loudoun Ob-Gyn, we are always striving to find ways
to keep our patients informed and healthy and improve
our service to you and our community. With this in mind,
we are pleased to bring you our first newsletter to update
our patients on exciting developments in our practice and
in the field of Obstetrics and Gynecology. Many women
get in the habit of seeing their Ob-Gyn for their prenatal
care and annual exams, and don’t have time to think
about the other services we offer. Maybe you’ve been
putting off a discussion with your doctor, thinking your
symptoms are “a normal part of being a woman.” Read on
to see how the latest advances in our field may improve
your health and quality of life, and are probably easier
than you think!

Are you bothered by heavy menstrual flow?
Chances are, we can help.
Heavy periods are a common symptom for women,
and one that a lot of women have learned to live
with. As doctors, our patients frequently tell us
that they arrange their lives around their cycles,
because they are afraid their period could cause
embarrassment. Some women cancel activities,
need to stay near a restroom, and even carry a
change of clothes with them during their periods.
Have you ever been told you are anemic, or do you
feel tired all the time—excessive menstrual flow
could be the reason.
Fortunately, women have many choices today for
managing heavy menstrual flow. First of all, it is
important to perform an evaluation for any specific
cause of heavy flow, such a fibroids or polyps. This
can usually be done with an ultrasound and in inoffice endometrial biopsy if necessary.
After a work up, our doctors will discuss what
options might work best for you. Your health
history, desire and need for contraception, and
future childbearing plans all factor into this
decision. Hormonal options have always been
popular and quite effective, but there is now a nonhormonal medication that effectively lessens
uterine blood flow, which you take only during
your cycle. If fibroids in the uterine lining are the
problem, these can often be removed in an

outpatient procedure with a small camera inserted
into the uterus. And for those who want a
permanent solution and have completed their
family, an endometrial ablation is a 2 minute
procedure that can be done in our office! We offer
this procedure in our Lansdowne location, with an
on-site anesthesiologist to ensure your comfort and
safety. Patients enjoy the convenience and
personal atmosphere of an in-office procedure.
A generation ago, women often had hysterectomies
for heavy bleeding. And sometimes that is still a
suitable solution. However, women now have many
less invasive alternatives, offering excellent results
in an office or outpatient setting with minimal
downtime.

Do you have a smart phone? Try out an
app called Period Tracker. It’s a free and
easy-to-use tool for tracking your period.
It will calculate the length of your cycle
and when the next one is due. You can
also record your symptoms, amount of
flow, and monitor your fertile days!

Incontinence slow you down!
Incontinence is a topic women sometimes feel awkward discussing
with their doctors. However, we’d love to hear about your
incontinence, because we’d like to help!
Stress incontinence is leakage of urine that occurs when there is
physical pressure on the bladder. Women may curtail their activities
because they leak, finding they can no longer enjoy their run or
tennis game without wearing an uncomfortable pad. Mommies are
sometimes reluctant to jump around with their kids or break out in
laughter because they’ll wet themselves. Allergy sufferers dread the
cough or sneeze that causes urinary leakage. If you are having
bothersome frequent episodes of urinary leakage, you don’t have to
live with it!
We offer in-office urodynamic testing, in which we will test your
bladder’s capacity and objectively measure how much pressure on
your bladder causes leakage. This will help us diagnose other
possible types of incontinence, such as urge incontinence, which is
caused by an overactive bladder muscle. A physical exam will
determine if the bladder has prolapsed, or fallen, and may need a
corrective “lift,” called an anterior repair. If stress incontinence is
indeed the problem, a minimally invasive procedure is now available
in which a mesh sling is inserted through a small incision in the
vagina to support the urethra. Patients can often go home the same
day and have only mild discomfort. Talk to our doctors if you want
more information on this procedure, called a Transobturator Tape.

This is not your mother’s
hysterectomy!
If you’ve ever heard about hysterectomy, you may think of a
prolonged hospital stay. You may think you’ll be out of work for 4
weeks. You may think it means your ovaries will be taken out.
These scenarios were common a generation ago, even 10 years
ago. But newer surgical techniques mean hysterectomy can be
performed with less pain, less blood loss, shorter hospital stay,
and quicker recovery and return to your normal life.
Our physicians have kept current with these advances in the
field of gynecology, and we offer these minimally invasive
options. Previously, large fibroids meant the patient had to have
a large abdominal incision. Currently, newer instruments allow
us to break up a large, bulky uterus into small pieces and remove
it through an eleven millimeter incision! In the past, a small
uterus could be removed through the vagina, but this required
removal of the cervix and didn’t allow for a thorough inspection of
the pelvis and ovaries. However, some women may want to leave
the cervix in place, and nowadays we are more frequently
removing just the upper portion of the uterus when there are no
problems with the cervix. Likewise, we are less apt to remove the
ovaries, even in a perimenopausal or menopausal woman, as it is
thought they continue to function for a time after menopause.
Our patients have been quite pleased with their minimally
invasive hysterectomies, usually going home the day after
surgery and returning to work in two weeks or less.

Summer will be here before you know it, so think
about calling soon to make an appointment for your
daughter while she’s home from school!
We often get asked, “When should my daughter start
seeing a gynecologist?” Did you know, the American
College of Obstetricians and Gynecologists
recommend young women have their first visit at
age 15. This visit does not necessarily include a
pelvic exam, but we will talk to her about her body,
healthy habits, and sexuality when appropriate. We
will tailor the visit to fit her maturity level, and
mom is welcome to be present for the appointment.

Loudoun Ob/Gyn is pleased to announce our
newest addition, Dr. Jennifer Hickman. Dr.
Hickman joined us in September and is accepting
new patients. She comes to us from Charlotte,
North Carolina, where she was in private practice
after completing her residency at Carolinas
Medical Center. She relocated here for her
husband’s job and has three adorable little girls.
We are very happy to have her with us, and know
you will enjoy getting to know her.
We also are pleased to announce our new office
manager, Christina Ortiz. If you have any officerelated concerns or billing questions, please call
or speak to Christina and she will be happy to
assist you.
For your convenience, we now offer early morning
appointments on Tuesday and Thursday, starting
at 8 am.

by Washingtonian magazine. He also has

Inova Loudoun Hospital.
