Instructions Concerning Use of this Form
Applicant will complete and deliver this form, prior to construction, to the Carbon
County Commissioners Office or the County Road Foreman serving in the area in which
the Approach Permit is requested.
Application must include a plot plan or plan and profile of the approach and the $100
fee. The undersigned applicant shall bear all costs and expenses associated with the
construction, maintenance, and/or removal of the irrigation pipe, and reclamation of the
Road Right-of-way following completion of such activities.
