The Reflex Methodology of Treatment
Portland, OR – Many with chronic knee pain wonder if they can ever get their life back, be able to be active
again, and be able to walk around the block again. For many of Reflex’s hundreds of patients, the answer is
yes. Reflex has developed a treatment plan that works to keep a person’s pain under control, allowing for
strengthening, re-alignment of the knee and exercise. There are only two ways to deal with osteoarthritis
(OA) of the knee – replacement surgery or a treatment plan that manages pain. For those who want to
keep their knees and avoid surgery, the doctors at Reflex develop this complete plan for their patients.
Reflex has refined their treatment plan to include the following:








Initial evaluation (to diagnose and determine if Reflex treatments will be effective)
Series of 3 guided injections, each delivered a week a part (reduce inflammation, protect joint)
Off Loading KneeBrace fitting and adjustments for pain relief and proper joint alignment
Bionicare consultation (an electrical stimulation treatment)
2 month follow up appointment (monitoring pain levels and effectiveness)
Physical Therapy (rebuild knee joint and muscle strength, and correct compensation patterns)
6 Month follow up appointment
Second series of guided injections (if needed)
Initial Evaluation: These are typically an hour long. A Clinical Care Coordinator will take the patients
height, weight and blood pressure. Then they meet together with the physician where they discuss
patients symptoms/pain, medical history, and health goals. They also take X-rays to assess level of
osteoarthritis. The physician will come up with an individualized treatment plan for each patient’s
needs and health status. Next a Reflex specialist will fit an offloading knee brace on the patient to see
if it provides any immediate relief. Lastly the Insurance specialists go over the patients specific
benefits to discuss cost and coverage.

Joint Fluid Therapy (Guided Viscosupplementation): This treatment involves a series of 3 injections, 1 week
apart. Many Reflex patients see 6-12 months of significant pain relief with this procedure. The injections are
guided through X-ray – allowing for maximum therapy and maximum benefit. Reflex is one of the few that do
guided injections through x-ray imaging. A series of 3 injections will occur a week apart. The procedure is
simple and virtually painless.
They’ll seat you in the treatment chair where your knees are easily accessible to the fluoroscope. They
will cover your knee with a sterile drape then spray Ethyl-chloride to numb your skin where you will get
the injection. Next, they inject Lydocaine to numb the joint itself and Omnipaque is injected using
fluoroscopic guidance. Omnipaque creates a contrast on the x-ray so that the doctor can see exactly
where the needle and liquid are being injected into the body, ensuring 100% accuracy for your injection.
The fluid we introduce into the knee provides the same lubrication that your body naturally provides. Most
patients feel pain relief within a few days.
Offloading -Knee Bracing System: An offloading brace helps protect the damaged cartilage and decrease a
patient’s pain. It also helps maintain proper joint alignment, which is crucial to a healthy knee. Proper use of
the brace can help prevent several years of pain and suffering by slowing the degenerative process of the
damaged cartilage. If a knee brace provides immediate relief, one will be ordered for you. Once it comes into
the clinic we’ll schedule a fitting. The Reflex tech will teach the patient how to put on the knee brace correctly
so it is relieving pain, pressure, and helping re-teach the leg to move correctly. Patients come in for an
additional adjustment two weeks later, and at the follow up appointments. OR anytime the patient feels it is
necessary.
The BioniCare® Knee System: This treatment is intended for people with moderate and severe knee
osteoarthritis (OA). The device uses a unique technology for treating knee osteoarthritis and relieving its pain.
It delivers a patented low-level pulsed electrical signal to the knee joint that is not felt by the patient. If
Bionicare is prescribed we will send the prescription off to VQ Orthocare, the company we work with for
Bionicare. Once received their consultant meets with the patient at the Reflex office to go over how it works,
how and when it should be worn, and what to expect.
Health Supplements: Reflex introduces patient to supplements that are the highest grade available and the

Physical Therapy: Chronic knee pain results in altered gait mechanics, leading to an uneven load across the
joint, accelerating the degeneration of the inflamed cartilage. It is important to increase overall joint stability to
maintain long-term improved knee health. A qualified physical therapist will supplement the bracing system by
increasing muscle strength and overall joint stability.
