20 week transfer
assessment process
starts at this meeting.

ALL TRANSFER REVIEW EVENTS WILL BE HELD BY Friday 11th December 2015
Mid Oct – early March Using information
from pre-transfer, any
updated reports and
transfer meeting the
draft plan will be
drawn up and sent to
parents for review and
preference of
placement.
If identified that
updated advice is
required from other
professionals this will
be sought by the LA
before the draft is
written.
If it is decided a
transfer to an EHC Plan
is not required,
parents will be notified
and given right of
appeal.
ALL PLANS WILL BE FINALISED BY 31ST MARCH 2016
