1. INTRODUCTION
This is a replacement post to support the specialist diabetes service and to share clinical
commitments with a Consultant colleague in South Cumbria.
The successful candidate will have the option to take part in endocrinology and acute
medical services at Furness General Hospital.
The posts will be employed by CPFT with collaborative agreement with University Hospitals
of Morecambe Bay NHS Foundation Trust (UHMBFT) for the Endocrinology and General
Medicine components.
The successful candidate will be a flexible, innovative individual with high professional
standards, strong interpersonal skills and a track record as a team player. Pro-active
clinicians with a special interest are welcomed and the structure of the posts is designed to
support this.
Applicants who are only able to work part-time are welcome to apply. Their job plan would
be modified in agreement with the Clinical Director.

Our community services are supported by hospital beds and rehabilitation services from our
nine community hospitals, community beds in acute hospitals, and mental health and
learning disability inpatient units. This system of care is part of the Cumbrian commissioners
‘Closer to Home’ strategy which aims to reduce reliance on acute hospitals by building
services that cross organizational boundaries.

4. THE SERVICE
Cumbria Diabetes represents an integration of the entire multidisciplinary specialist team
with responsibility for the quality of all diabetes services in Cumbria. The service started in
2011 and has made significant progress since that time. We aim to improve the quality of
diabetes care by delivering patient pathways with structured education programmes to
support self-management, by supporting and training of primary care teams and providing
excellent specialist diabetes clinical services in both in and out-patient settings.
A key aim of Cumbria Diabetes is to facilitate and encourage self-management by people
with diabetes. Structured patient education programmes (DAFNE and DESMOND) are
embedded in our pathways of care and we are training both primary care and specialist staff
to undertake holistic care planning. We are the largest DESMOND centre in the UK and the
DESMOND team won a national award in 2014. We have a GP Lead within our service that
drives the delivery of DESMOND in Cumbria.
We provide speciality services including pregnancy, young person/ transition service and an
insulin pump service.
A Cumbria wide retinal screening service operates throughout Cumbria at a variety of
locations. The retinal screening service is also the service provider for North Lancashire and
Blackpool.
The Cumbria Diabetes MDT operates as a North and South based team. We currently have
four substantive Diabetes and Endocrine consultant posts across Cumbria. Two posts are
based in North and South Cumbria respectively.
The Cumbria wide team meet 2-3 times per year to review overall service provision and
share good practice.
South Cumbria Diabetes Team
The South Cumbria Diabetes team operates across the South Lakes and Furness localities
of Cumbria. The team bases are Westmorland General Hospital (WGH) and Furness
General Hospital (FGH).
The multidisciplinary team members are as follows:


5.6 WTE diabetes specialist nurses (includes 0.2 WTE MODY link nurse for county)

1.6 WTE diabetes specialist dietitians
Current activities include:

Providing support and training to GP practices. A rolling programme of training is run
with pharma support on a 4 monthly basis.

Multidisciplinary diabetes clinics for people with Type 1, insulin pump, complex
issues related to glycaemia or complications and for people with Type 2 where new
therapies are being considered.
Pregnancy and diabetes clinics jointly provided with Obstetric colleagues in the relevant
Acute Trust at CIC, WCH and FGH.
The majority of specialist nurses and all specialist dietitians are DAFNE trainers and
courses are run from all team bases across Cumbria.
DESMOND courses are delivered by many of the DSN’s and dietitians. We also have a
number of primary care trainers for DESMOND.
North Cumbria Diabetes Team
The North Cumbria Diabetes team operates across both the East (Carlisle and Eden) and
West (Copeland and Allerdale) localities of Cumbria. The team bases are at Cumberland
Infirmary (CIC), Carlisle and West Cumberland Hospital (WCH), Whitehaven.
The multidisciplinary team members are as follows:



4.95 WTE diabetes specialist nurses (2.95 WTE CIC and 2 WTE WCH)

2 WTE diabetes specialist dietitians (1 WTE per team base)
University Hospital of Morecambe Bay NHS FT (UHMBFT)
UHMBFT has two hospital sites within Cumbria, FGH and Westmorland General (WGH).
Acute medical admissions and in-patients only occur at FGH.
FGH is a district general hospital which opened in 1984 and is situated in a green field site
on the edge of Barrow-in-Furness. The hospital provides acute and continuing care services
to a population of approximately 140,000 in the Furness peninsula and the surrounding part
of South Cumbria
It has departments in accident and emergency, general medicine, elderly medicine, general
surgery, orthopaedic surgery, urology, gynaecology, obstetrics and paediatrics. There is an
ITU, medical HDU and surgical PPU. The Oncology unit offers day case and inpatient
chemotherapy for a range of haematological malignancies and solid tumours.
There are regular consultative clinics in neurology, cardiology, plastic surgery (specialty based),
radiotherapy, nephrology, immunology, clinical genetics and paediatrics, with visiting regional
paediatric specialists in neurology, nephrology, endocrinology, cardiology and paediatric
surgery.
FGH and WGH have well-equipped postgraduate centres.

5. THE POST
The post is a replacement post and will involve specialist diabetes clinics and delivery of
training to primary care staff. There is the option to undertake endocrinology clinics and a
shared responsibility to GIM in-patients and the on-call rota at FGH.
We offer speciality diabetes clinics including pregnancy, pump and a paediatric transition
service. The clinics take place at both FGH and Westmorland General Hospital, Kendal
(WGH). We work collaboratively with the diabetes podiatrist and would like to establish a
designated diabetes foot service for in and out-patients.
We are willing to negotiate clinics dependent upon the specialist interests of the successful
candidate.
New patient appointments will be for 30 minutes and follow up patients 15-20 minutes
dependent upon the clinic.
A key role of Cumbria Diabetes is the support and training of primary care teams. This post
has a responsibility to the South Lakes locality area of GP practices. Job plans include 1 PA
to support this activity. There would also be the opportunity to be trained to delivery DAFNE
doctor sessions on our local DAFNE courses.
The MDT provides diabetes care to in-patients at FGH. This could include direct review of
patients and/ or training sessions for Acute trust staff.
Surgical support to the foot service is provided by the Lancashire Teaching Hospitals NHS
Foundation Trust Vascular team. They visit FGH on a weekly basis and are contactable at all
times at Royal Preston Hospital.
The Renal service to South Cumbria is provided by visiting nephrologists from Royal Preston
Hospital. They undertake weekly out-patient clinics at FGH. A Renal nursing team are based
at FGH to support patients on CAPD and with other issues related to chronic kidney disease.
There is one Endocrine clinic per week at FGH and there is the option to share this
commitment with the current substantive post holder. This is a general clinic with tertiary
referrals usually made to Royal Preston Hospital.
There is the option to have shared responsibility for general medicine (GIM) commitments
including in-patient work and an on call. The GIM responsibilities take place at Furness
General Hospital. The on call rota is a 1 in 11 and this post would potentially share a slot
with the current substantive consultant (approx. 1:22).
The Diabetes team currently supports in-patients on the Complex and Coronary Care Unit
(CCCU) which is an 8 bedded unit. There is a Cardiology Consultant of the week model and
appropriate patients are managed solely by Cardiology. There is collaborative working with
ICU to step patients between level 2 and 3 care. A small number of additional patients can
occur on the ITU, medical oncology (Coniston suite) and outlying wards. We have a junior
team including a Specialist Registrar in Diabetes/ Endocrinology (North West deanery), CT1
and F1 to support the in-patient work.

Key responsibilities






To provide leadership to the South multidisciplinary specialist diabetes teams in
collaboration with Consultant colleagues.
To further develop a flexible and responsive service to deliver high quality care for
patients.
Provide leadership within the diabetes MDT meetings and engage with the clinical
governance agenda.
Ensure close collaboration with all relevant professionals and organisations in
providing a comprehensive and seamless diabetes service across organisational
boundaries.
The post holders will be required to develop, maintain and manage effective
relationships with a range of stakeholders internally and externally, across all
disciplines and including service users, carers and the media.
Facilitate innovation, evaluation, audit and research within the field of diabetes.
Other Duties:



So far as is consistent with the proper discharge of the above duties the post holder
undertakes to deputise from time to time for absent colleagues.
The post holder undertakes, exceptionally, to perform additional duties in occasional
emergencies and unforeseen circumstances.
The post holder undertakes, exceptionally, to be available for such irregular
commitments outside normally rostered duties as are essential for continuity of
patient care.
There is scope for development of specialist interests that match the aspirations and skills of
potential candidates.
Management Role:






The post holder would be expected to take up a leadership role within the scope of
their clinical practice, to uphold the tenets of safe and effective practice within clinical
governance frameworks.
Additionally, the post holder would be encouraged to pursue special interests, as
appropriate to the role in a leadership capacity.
The post holder is expected to report poor performance in colleagues within Trust
Policies and Procedures.
The post holder is expected to review their job plan at least annually with the clinical
director to reflect both service and career development.
Appraisal and revalidation to be held according to Royal College of Physicians, Trust
and Department of Health Guidelines.
Post-holders will be supported in development of research and it is expected that
increasing academic opportunities will become available with the development of the
Cumbria, Lancashire Medical School.

Accountability, management and colleagues
The post holder will be accountable to the Clinical Director for Cumbria Diabetes. The post
holder will work closely with Consultant colleagues, the DSN, dietetic and GP Leads and the
Cumbria Diabetes senior service manager.
Colleagues will include the Multi-disciplinary diabetes care team, staff from the relevant
Acute Trust, primary care, adult social care and third sector.
Administrative Support
Post-holders will have administrative support to include medical secretary resource, access
to a computer with full internet connection and dictation equipment. Designated office space
will be available.

All posts will be 10 PA with the possibility of additional PAs when required. Travelling time
will be taken into account when travelling across sites. Candidates will be offered the choice
of reduced commitments within a 10 PA plan, or an additional session of travel will be
allocated.
Any clinic activity will be based upon 3.5 hours face to face patient contact and 0.5 hours for
clinical related administration.

The Consultant will participate in the provision of cover on a shared basis with colleagues for
periods of sickness, annual leave and study leave, except for prolonged absence due to
sickness where locum cover may be necessary.
7. CPD/RESEARCH
The Trust supports requirements for continuing professional development as laid down by
the Royal Colleges and is committed to providing time and financial support for these
activities.
The links can be tailored to support both the service in Cumbria and personal development
of our consultants. Consultants in Diabetes/ Endocrinology in Cumbria are welcome to
participate on the full range of CME/CPD activities. If they wish, can undertake research,
audit, can be involved in student teaching or other forms of CPD.
Cumbria Partnership NHS Foundation Trust has a very active research department that is
able to support recruitment of patients for clinical studies.
Total time allocated to CPD / research / highly specialist clinics will be agreed with the
Clinical Director.
Cumbria Partnership NHS Foundation Trust provides mentorship and action learning sets for
new consultants and post holders, especially those appointed from posts other than NHS,
consultants are expected to participate.
All doctors in Cumbria Partnership NHS Foundation Trust are expected to involve
themselves in a range of clinical governance activities and will be required to engage in the
enhanced appraisal / revalidation process. There will also be an expectation to perform audit
as part of the post holders role; this will be included within the SPA time
8. MEDICAL EDUCATION
Furness General Hospital as part of University Hospitals of Morecambe Bay NHS
Foundation Trust (UHMBFT) is part of the North West Deanery based in Manchester. There
is a medical school in Lancaster and opportunities to provide education to the year 4 medical
students. Training of postgraduate medical staff is viewed as essential by the Trust for the
development of high quality clinical services.
FGH and WGH have well-equipped postgraduate centres.

9. HEALTH AND SAFETY
Employees must be aware of the responsibilities placed upon them under the Health &
Safety at work Act (1974), to ensure that the agreed safety procedures are carried out to
maintain a safe environment for employees, visitors and patients.

10. RISK MANAGEMENT
All staff has a responsibility to report all clinical and non-clinical accidents, incidents or nearmisses promptly and to co-operate with any investigations undertaken.
Your specific responsibility for Risk Management will be clarified to you by your line manager
at your local induction.
You will be expected to familiarise yourself with the Trust’s Governance Strategy which
outlines the Management and Committee Structures and Procedures for the Governance of
the Trust’s activities.
You will have a duty to familiarise yourself with the relevant Policies and Procedures, i.e.:

11. REHABILITATION OF OFFENDERS
Because of the nature of this work this post is exempt from the provisions of Section 4(2) of
the Rehabilitation of Offenders Act 1974 (Exemptions) Order 1975. You are therefore not
entitled to withhold information about convictions which for any other purposes are “spent”
under the provisions of the Act and any failure to disclose such convictions could result in
disciplinary action by the Trust. Any information given will be kept in strict confidence and
only used in relation to the position to which the order applies.

12. MAIN CONDITIONS OF SERVICE




The post-holder will be appointed under the Terms and Conditions – Consultants
(England) 2003.
Outside employment may be agreed with the Chief Executive.
For pre-employment health purposes, as part of the Trust’s Occupational Health
Service, the successful candidate will be required to complete a health questionnaire.
This will be treated in the strictest confidence and only be seen by the Occupational
Health Department.
Horary Contracts will be in place to ensure access to Acute hospitals and hospices

a controlled manner subject to statutory requirements and agreed security and confidentiality
policies, procedures and guidelines e.g. Data Protection Act 1998, Freedom of Information
Act 2000, Caldicott Guidelines NHS Confidentiality Code of Conduct 2003, and professional
codes of conduct on confidentiality.
The post holder must maintain the confidentiality of information about service user staff and
organisational business in accordance with the Data Protection Act 1998 and Caldicott
principles.
It is likely that the post holder will be in contact at some time with a form of information
system, and therefore are responsible for implementing and maintaining data quality. The

attributable and that the record keeping is contemporaneous. It is essential that information

(Kendal and Barrow), junction 40 for Penrith and junctions 42-44 for Carlisle.
The historic city of Carlisle is the main centre of population for North Cumbria with smaller
communities in Penrith, Keswick, Brampton, Alston, Wigton and Kirkby Stephen. The city
has a wide range of facilities including the Lanes shopping centre, a unique Victorian market
and its pedestrian only area sets the scene for year round events including international food

The castle was founded in 1092, and the cathedral dates back to the 14th century; there is an

8. THE SERVICE
Cumbria Diabetes represents an integration of the entire multidisciplinary specialist team
with responsibility for the quality of all diabetes services in Cumbria. The service started in
2011 and has made significant progress since that time. We aim to improve the quality of
diabetes care by delivering patient pathways with structured education programmes to

support self-management, by supporting and training of primary care teams and providing
excellent specialist diabetes clinical services in both in and out-patient settings.
A key aim of Cumbria Diabetes is to facilitate and encourage self-management by people
with diabetes. Structured patient education programmes (DAFNE and DESMOND) are
embedded in our pathways of care and we are training both primary care and specialist staff
to undertake holistic care planning. We are the largest DESMOND centre in the UK and the
DESMOND team won a national award in 2014. We have a GP Lead within our service who
drives the delivery of DESMOND in Cumbria.
We provide speciality services including pregnancy, young person/ transition service and an
insulin pump service.
A Cumbria wide retinal screening service operates throughout Cumbria at a variety of
locations. The retinal screening service is also the service provider for North Lancashire and
Blackpool.
The Cumbria Diabetes MDT operates as a North and South based team. We currently have
four substantive Diabetes and Endocrine consultant posts across Cumbria. Two posts are
based in North and South Cumbria respectively.
The Cumbria wide team meet 2-3 times per year to review overall service provision and
share good practice.
This new post is in recognition of the service need in North Cumbria and is an exciting
opportunity to join a rapidly evolving service. The specific areas of the diabetes service
which require development are Diabetes Foot, transition care in collaboration with
paediatrics and in-patient care. There is also a need for additional capacity in the Endocrine
service.
North Cumbria Diabetes Team
The North Cumbria Diabetes team operates across both the East (Carlisle and Eden) and
West (Copeland and Allerdale) localities of Cumbria. The team bases are at Cumberland
Infirmary (CIC), Carlisle and West Cumberland Hospital (WCH), Whitehaven.
The multidisciplinary team members are as follows:
 2 WTE Consultants (1 WTE per team base)
 4.95 WTE diabetes specialist nurses (2.95 WTE CIC and 2 WTE WCH)
 2 WTE diabetes specialist dietitians (1 WTE per team base)
South Cumbria Diabetes Team
The South Cumbria Diabetes team operates across the South Lakes and Furness localities
of Cumbria. The team bases are Westmorland General Hospital (WGH) and Furness
General Hospital (FGH).
The multidisciplinary team members are as follows:
 2 WTE Consultants (includes the Clinical Director)
 5.6 WTE diabetes specialist nurses (includes 0.2 WTE MODY link nurse for county)
 1.6 WTE diabetes specialist dietitians

Current activities include:
 Providing support and training to GP practices. A rolling programme of training is run
with pharma support on a 4 monthly basis.
 Multidisciplinary diabetes clinics for people with Type 1, insulin pump, complex
issues related to glycaemia or complications and for people with Type 2 where new
therapies are being considered.
 Pregnancy and diabetes clinics jointly provided with Obstetric colleagues in the relevant
Acute Trust at CIC, WCH and FGH.
 The majority of specialist nurses and all specialist dietitians are DAFNE trainers and
courses are run from all team bases across Cumbria.
 DESMOND courses are delivered by many of the DSN’s and dietitians. We also have a
number of primary care trainers.
9. THE POST
This is a new post and is required to provide additional endocrine capacity in North Cumbria
and to support the development of diabetes foot, transition care and in-patient services.
Important roles for all Consultant posts include:
 Leadership and support to the North Cumbria diabetes team
 Specialist diabetes clinics and we would aim to align specialist areas to individual
interests
 Delivery of diabetes training to primary care staff
 Endocrinology clinics
 Participation in the GIM on-call rota at CIC
There may be the need to travel to West Cumbria to provide specialist diabetes clinics in the
interest of providing best quality services across North Cumbria.
New patient appointments will be for 30 minutes and follow up patients 15-20 minutes
dependent upon the clinic.
A key role of Cumbria Diabetes is the support and training of primary care teams. Job plans
include 1 PA to support this activity. There would also be the opportunity to be trained to
delivery DAFNE doctor sessions on our local DAFNE courses.
The MDT provides diabetes care to in-patients at CIC and WCH. In-patient referrals for
endocrinology are also made from the Acute Trust. An area for development is the delivery
of diabetes training sessions for Acute trust staff.
The Endocrine clinics are currently delivered at CIC. These are general clinics and the usual
tertiary referral centre is Royal Victoria Infirmary, Newcastle. A joint meeting with the Clinical
Biochemistry Consultant and Diabetes/ Endocrine Consultants occurs approximately every 6
weeks at CIC for case discussions.
General medicine on-call is at CIC. The rota is an Acute Physician model with a shift
frequency of approximately 1:12. The post-holder will have no responsibility for in-patient
beds on the medical wards.

North Cumbria University Hospital NHS Trust (NCUH)
The endocrine and GIM commitments within the post are NCUH services and are covered
by a service level agreement between CPFT and NCUH. NCUH has two hospital sites at
CIC and West Cumberland Hospital (WCH).
NCUH provides an extended range of “District General Hospital” services such as a full
range of cancer services (oncology, chemotherapy, nuclear medicine and radiotherapy),
maxillo-facial surgery and rehabilitation medicine. In order to ensure local access to key
services, paediatrics, maternity and A&E services are provided on both hospital sites.
Examples of recent developments and initiatives include:






Introduction of an out of hours Telestroke service delivered across 6 Acute
Trusts and eight hospital sites in Cumbria and Lancashire
Trauma Unit status for the Cumberland Infirmary, Carlisle
Development of specialist vascular services at the Cumberland Infirmary,
Carlisle
Development of Cumbria-wide interventional PCI service
Ambulatory Emergency Care Nurse Practitioner services have been
implemented on both sites
New Emergency Surgical Unit at the Cumberland Infirmary, Carlisle.
Key responsibilities
 To provide leadership to the North multidisciplinary specialist diabetes teams in
collaboration with Consultant colleagues.
 To further develop a flexible and responsive service to deliver high quality care for
patients.
 Provide leadership within the diabetes MDT meetings and engage with the clinical
governance agenda.
 Ensure close collaboration with all relevant professionals and organisations in
providing a comprehensive and seamless diabetes service across organisational
boundaries.
 The post holders will be required to develop, maintain and manage effective
relationships with a range of stakeholders internally and externally, across all
disciplines and including service users, carers and the media.
 Facilitate innovation, evaluation, audit and research within the field of diabetes.
Other Duties:
 So far as is consistent with the proper discharge of the above duties the post holder
undertakes to deputise from time to time for absent colleagues.
 The post holder undertakes, exceptionally, to perform additional duties in occasional
emergencies and unforeseen circumstances.
 The post holder undertakes, exceptionally, to be available for such irregular
commitments outside normally rostered duties as are essential for continuity of
patient care.
There is scope for development of specialist interests that match the aspirations and skills of
potential candidates.

director to reflect both service and career development.

increasing academic opportunities will become available with the development of the
Cumbria, Lancashire Medical School.

The post holder will be accountable to the Clinical Director for Cumbria Diabetes. The post
holder will work closely with Consultant colleagues, the DSN, dietetic and GP Leads and the
Cumbria Diabetes senior service manager.
Colleagues will include the Multi-disciplinary diabetes care team, staff from the relevant
Acute Trust, primary care, adult social care and third sector.
Administrative Support
Post-holders will have administrative support to include medical secretary resource, access
to a computer with full internet connection and dictation equipment. Designated office space
will be available.

All posts will be 10 PA with the possibility of additional PAs when required. Travelling time
will be taken into account when travelling across sites. Candidates will be offered the choice
of reduced commitments within a 10 PA plan, or an additional session of travel will be
allocated.
Any clinic activity will be based upon 3.5 hours face to face patient contact and 0.5 hours for
clinical related administration.
The Consultant will participate in the provision of cover on a shared basis with colleagues for
periods of sickness, annual leave and study leave, except for prolonged absence due to
sickness where locum cover may be necessary.
18. CPD/RESEARCH
The Trust supports requirements for continuing professional development as laid down by
the Royal Colleges and is committed to providing time and financial support for these
activities.
The links can be tailored to support both the service in Cumbria and personal development
of our consultants. Consultants in Diabetes/ Endocrinology in Cumbria are welcome to
participate on the full range of CME/CPD activities. If they wish, can undertake research,
audit, can be involved in student teaching or other forms of CPD.
Cumbria Partnership NHS Foundation Trust has a very active research department that is
able to support recruitment of patients for clinical studies.
Total time allocated to CPD / research / highly specialist clinics will be agreed with the
Clinical Director.
Cumbria Partnership NHS Foundation Trust provides mentorship and action learning sets for
new consultants and post holders, especially those appointed from posts other than NHS,
consultants are expected to participate.
All doctors in Cumbria Partnership NHS Foundation Trust are expected to involve
themselves in a range of clinical governance activities and will be required to engage in the
enhanced appraisal / revalidation process. There will also be an expectation to perform audit
as part of the post holders role; this will be included within the SPA time

19. MEDICAL EDUCATION
The North and West post-holders will work into North Cumbria University Hospitals Acute
Trust (NCUH) sites. NCUH is part of the Northern Deanery (www.northerndeanery.nhs.uk)
based in Newcastle. The South post-holder will work into Furness General Hospital which is
part of University Hospitals of Morecambe Bay NHS Foundation Trust (UHMBFT). UHMBFT
is part of the North West Deanery based in Manchester. There is a medical school in
Lancaster and opportunities to provide education to the year 4 medical students.
Training of postgraduate medical staff is viewed as essential by the Trust for the
development of high quality clinical services.
There are excellent library facilities on all Acute sites. There is a Medical Illustration Service
based at the Cumberland Infirmary, Carlisle.
20. HEALTH AND SAFETY
Employees must be aware of the responsibilities placed upon them under the Health &

Because of the nature of this work this post is exempt from the provisions of Section 4(2) of

The post-holder will be appointed under the Terms and Conditions – Consultants
