September 2010 To All Parents of Children with IEPs: This letter is to inform you of upcoming changes in the IEP process during the 2010-2011 school year. Beginning this school year all districts are directed by the Michigan Department of Education (MDE) to discontinue the use of a “signature page” to conclude the IEP process. In the past we have used this page to indicate approval or disapproval and for the district to officially offer a Free Appropriate Public Education, commonly referred to as FAPE. As we conduct IEPs this year we will use our “signature page” but will also use the recommended MDE form in tandem. This form is known as the “Notice for Provision of Services and Programs”. Beginning with IEPs in the 2011-2012 school year the “Notice for Provision of Services and Programs” will totally replace the “signature page” we have used for several years. Please understand our “signature page” has become obsolete and we can no longer use it after the 2010-2011 school year. The MDE form allows us to better serve students with IEPs and their parents. In short, we will conduct the IEP as part one and as part two we will sign a “Notice for Provision of Services and Programs” page (along with a signature page this year only). With the signature of the District Superintendent or Designee on the “Notice for Services and Programs” page, the school district is officially offering FAPE. I have attached a copy of the new MDE form “Notice for Provision of Services and Programs” for your review. You can also find this form along with a Quick Reference Guide on the OAISD website at: https://www.oaisd.org/oaisd/departments/specialneedseducation/specialeducatio nproceduresrequirements/section3iep/. Please do not hesitate to call me with any questions you might have as we make this transition in our special education forms. Thank you for your understanding and cooperation. Sincerely, Janette L. Cochran, Director of Student Services
