There are some plans that consider VCU Medical Center to be “out-of-network,” such as out-of-state Medicaid plans and most Medicare Advantage plans. If your insurance plan considers VCU Medical Center to be outside of their network, you may be responsible to pay the total amount for your appointment or service. Please be aware that health insurance plans do not pay for all health care services — even those that your health care providers and you feel are necessary. You will be billed the total amount for any appointment or service that your insurance plan does not cover. Some insurance plans, such as Medicare, require that you sign a special form if your plan is not likely to pay for a specific service or item. This form is your notification that you may be responsible to pay the total amount. Before each appointment, please contact your health insurance plan to find out: If VCU Medical Center is part of your insurance plan’s network If your appointment or service will be covered by your insurance plan If your appointment or service requires a referral from a primary care physician The amount you may need to pay Financial Assistance by VCU Medical Center – From www.vcuhealth.org Financial Assistance programs include: Discounts for self-pay patients are available with additional discounts for prompt payments State-Sponsored Indigent Care Program provides free care to patients with income below the Federal Poverty Level and a slide scale discount for patients with incomes less than 200% of the Federal Poverty Level Virginia Coordinated Care Program provides added care coordination for indigent patients in certain localities Medicaid and FAMIS programs, which are state insurance programs that assist with payments for medical care if an individual meets certain criteria Documents that may be required: Paycheck receipts (your last three pay stubs) Child support verification Unemployment verification Social Security income verification Current bank statement
