Copies to put in Yellow Folder (filed in or near the Cumulative file-may vary depending on
school expectations):
-Notice of Proposed Meeting - form E *
-Referral and Placement form & Accommodations Page (grade clusters) - form C, pg 1 & 2 *
-State Testing Participation form (pg 30) *
-State Assessment Accommodations/LEP Accommodations Checklist (if applicable) *
-Aspire+;; EXPLORE/PLAN, & IEP/ACCESS
-Copy of Title III Letter-form D (2 pages or back/front) *
-Permission to Exit-form G (if applicable)
-Academic Performance- form AP (if applicable)
-Successful Performance Letter-form H (if applicable)
-Denial to Participate-form F (if applicable)
-MODEL Scoring Sheets/Testing Document (if applicable)
NOTE: the above forms should be grouped together for each year, paper clipped together, then filed chronologically
with the most recent on top in the yellow folder

-Teacher report of ACCESS results (original)* - NEEDS TO BE PLACED WITH OTHER STATE TESTING DOCUMENTATION
-Home Language Survey (every child in school has one in CUM file, not just ESL students)
Copies to Send to ESL Office after ELL Committee Meeting
-Denial to Participate/ Title III Parental Waiver (if applicable)
Copies to give Parents:
-Referral and Placement Form & Accommodations page (pg. 2 in Spanish if needed)
-State Testing Participation & Accommodations form
-Title III Letter (make every attempt to hand deliver—then send home w/ student in a sealed envelope,
addressed to the parents by the 10th or 30th day, respectively)

-Academic Performance (if applicable)
-Successful Performance Letter (if applicable)
-Denial to Participate (if applicable)
-Parent copy of ACCESS score reports (make every attempt to hand deliver—then send home w/ student in a
sealed envelope, addressed to the parents by the 10th or 30th day, respectively)
Copies to give Teachers of each ELL student:
-Referral and Placement Form/Accommodations Page
-Monitoring Form (to be filled out each grading period)
-“Can Do” Descriptors &/or Student Level Organizer
-Do not give ACCESS Teacher reports to Teachers—should remain in Cumulative file
