CANDID
Have you been to see doctor with bowel symptoms OR with lung symptoms lasting 3 weeks or more?
Dr Azim and Partners is taking part in a research study called CANDID and, if you are over 35, your GP or Nurse
Practitioner may ask if you would like to take part.
If you are invited to take part, you will have the study explained to you (including being given the contact details
of the study team) and given the opportunity ask any questions you may have before you make your decision
about whether or not you wish to be recruited to the study. You can decide today, or take the information home
and decide later. Even if you agree to participate you can change your mind at any time
New Did not Attend ( DNA ) Policy
Calling all Patients!
Patients who do not attend appointments for doctors, nurses or
HCAs are classed as DNA’s (Did Not Attend. Due to the high
numbers of missed appointments the surgery introduces a new
procedure.
The practice now has an active Patient
Participation Group called Friends@DNA who meet
regularly to raise points which might be improved
and to engage patients in events and activity
groups which may be of interest to them.
The “Friends” meet regularly at the Surgery and,
since their formation about a year ago, has
covered a lot of ground, in liaison with the doctors
and staff: there is always more to do!
Dates of “Friends” meetings and events are
posted on the Friends’ noticeboard in the waiting
Room, where the aims are also shown.
The next walks in Sunny Hill Park will be on
Sunday 11th January. The next meeting will be
held in the surgery on 7th January at 6pm.
Friends at DNA will also have coffee morning on
4th December.
The group welcome all-comers to join a meeting
and see what it’s about.
Patients who have failed to attend three or more
appointments cannot make advanced appointments for one
year. These patients will be able to book an
appointment on the day ONLY.
