As a rural family practice we pride ourselves on providing more than just medical treatment. Our two surgeries, on Hillyfields Way in Winscombe and Westfield Road in Banwell, are fully integrated into the surrounding communities, serving a total catchment area of 9000 patients. Our local population includes all age groups from young families, commuters and the Elderly. Our GP’s care for 3 nursing homes including St. Monica’s Trust Care Village in Sandford, which has a variety of housing including a dementia unit, nursing home and independent living houses. We also care for Winscombe Hall and have patients in Axbridge Court Nursing homes. Within our area are also care homes for the elderly and a residential home for people with learning disabilities. Each week we provide a Baby clinic either at Winscombe surgery or the Children’s Centre in Banwell. These clinics are held collaboratively with the Health Visitors, GP and Nurse and patients come for checks, injections and advice. We are also the Doctors for Sidcot School which is the local boarding school. We provide a wide variety of services in the surgery, in house clinics including chronic disease management, minor operations, joint injections, Warfarin monitoring clinics, Support to stop smoking, ear micro-suction, cryotherapy, antenatal care, travel clinic and palpitation diagnosis. Clinicians who visit the surgery to provide services for our patients include Counselling, Physiotherapy, Podiatry and Retinal Screening. We also provide a Forget me not service via The Alzheimer’s Society for those who are concerned about memory loss and a bladder and bowel service. Our Banwell Surgery also has an integral pharmacy. The majority of staff work at both Winscombe and Banwell surgeries. We have 5 partners (3 full time and 2 part time) and 2 part time salaried GP’s. All are dynamic, forward thinking and enthusiastic with their main focus being excellent patient care for their local community. All of the partners are Clinical Trainers and 3 are Educational Supervisors, all with a wealth of knowledge to share. All clinical members of the practice are willing to assist with your training. We have 3 part time Practice Nurses providing treatment room services and chronic disease management. We also have 3 HCA’s who provide our warfarin and phlebotomy service and also other treatment room services. Page 1 of 4 WINSCOMBE AND BANWELL FAMILY PRACTICE The Practice has a team of hard-working, helpful and friendly staff which include the Practice Manager and Deputy Manager, Secretaries, a Reception Supervisor and her team of Receptionists, and Administrative staff. We pride ourselves on working as a team and this supportive network enables us to cope with the challenges of General Practice today. A good sense of humour and frequent cups of tea/coffee also help us to maintain an enjoyable supportive working environment. The surgery is open from 8am until 6.30pm after that the BrisDoc Out of Hours (OOH) Service takes over care of our patients. 4 of our Partners work for the OOH service in the evenings and at weekends. We also provide extended hours surgeries on 3 mornings per week from 7am -8am. The surgery is currently closed evenings, weekends and Bank Holidays. We have a close working relationship with the Strawberry Line Community Team of District Nurses, Clinical Lead Nurses, Physiotherapists and Occupational Therapists. We also have strong links with an NHS Physio and Midwives from Weston General Hospital, a Palliative Care Nurse from Weston Hospice Care and other health professionals. General Practice is currently going through a period of major change not least the introduction of Clinical Commissioning Groups (CCGs) and the National Commissioning Board (NCB) to replace the Primary Care Trusts (PCT). We are a member of the North Somerset Clinical Commissioning Group (NSCCG) part of the wider Bristol, North Somerset and South Gloucestershire Commissioning Group (BNSSG). Along with Care Quality Commission Inspections (CQC) this means that life in the surgery is never boring and there are always new things to learn about General Practice alongside medical learning. We have consistently achieved excellent Quality Outcome Framework (QoF) results. The GP’s are responsible for coordinating specific QoF clinical areas, with the Practice Manager and an Administrative Assistant coordinating the managerial and administrative aspects. Winscombe and Banwell Family Practice is a training practice which trains GP Registrars and foundation grade Doctors. As well as being a training practice we also have regular medical student teaching sessions and encourage the registrars to be involved in teaching if they would like to. We are a Research Practice and are a Research Ready Accredited Practice and are a research practice within the South West Primary Care Research Network (PCRN). All trainees are encouraged to attend the weekly Practice and Clinical meetings. Practice Meetings are held with the Partners and Practice Manager to discuss business matters and development of the surgery. Clinical meetings are held with the GP’s and nurses to discuss patient issues and welfare. To maintain good communication links our Community team, (District Nurses, Community Clinical Lead, Physiotherapist, Occupational Therapist and Palliative Care Nurse) join us at a monthly Community Clinical meeting. We are currently using the EMIS LV clinical system but are changing to EMIS Web in October 2014. We use a Crescendo digital dictation system for all referrals and letters and workflow incoming correspondence by DOCMAN, document management system. Our intranet provides access to essential documents across both sites and the IQ medical CQC toolkit provides easy access to our policies and procedures. Our website provides information regarding the surgery and enables patients to book appointments and order repeat prescriptions. Dr John works full time at the surgery and also for BrisDoc as an Out of Hours GP. His interests include Musculo-skeletal including Joint Injections, Child Health (GP covering Banwell Baby Clinic) and he is the Safeguarding Lead and Contraceptive Implants. He is also the Caldicot Guardian for Information Governance. DR KATHRYN RUDDELL BSc, MBBS, DRCOG, MRCGP, DFSRH Partner and Clinical Supervisor. Dr Ruddell works at the surgery part time and also for BrisDoc as an Out of Hours GP. Her interests include Gynaecology, Women’s Health and Contraception. She is joint lead for Infection Control with Dr Caroline Stovell. DR JOHN JACKSON MB, ChB, MRCP, MRCGP, DCH, DRCOG, Dip. Occ.Medicine Partner and Educational Supervisor. Dr Jackson works full time at the surgery and also for BrisDoc as an Out of Hours GP. His interests include Dermatology, Respiratory Disease (Asthma, COPD etc.) and Occupational Health. Page 3 of 4 WINSCOMBE AND BANWELL FAMILY PRACTICE One of our previous Registrars has kindly provided some feedback for us Winscombe Surgery – Registrar view Location Firstly don’t be put off by the distance on a map. If you live in Bristol the commute is made much easier by the fact that all the rush hour cars are going the opposite direction. My drive to work from Bristol centre took just over 30 mins, most of which was in traffic free countryside. Alternatively live down there and have a nice time in the country! There are 2 surgeries in 2 adjacent villages, both pretty rural and mostly not highly deprived. Having said that there is still a significant amount of poverty and social problems – enough to learn about it, but not too much to get bogged down. Facilities Both surgeries have pleasant rooms for the sole use of the registrar. The library at Winscombe is a good size and in combination with the kitchen make it is a home from home. Training Without doubt the high point of a registrar year at Winscombe. David John manages to give excellent education to the registrar without you realizing it happening. The secret seems to be the way he tailors his methods to the individual registrar without making it seem contrived. He is always available for advice (even if late for surgery or bogged down in paperwork). You never feel taken advantage of – something that cannot be said for all surgeries. If something is not in your educational interest then you’ll not end up doing it. The other partners at the surgery are also highly approachable and add to the healthy learning atmosphere. You’ll probably be told that out of their 4 registrars all have passed MRCGP at first attempt, 3 with merit and one with distinction. I don’t believe that this is a fluke, or that Winscombe happened to get the best registrars. The whole learning environment is nurturing without being protective and sets you up well, not just for the exam but for your future general practice. Workload Although rural, the practice is busy. However, your consultation times will not be cut down until you and Dr John feel that you are ready. The home visit schedule is also busier than most other training practices (averaging 3 visits per day) some of this due to the distances that people live from the practice, some due to the practice culture. Although this does take up a significant part of the day it is a useful part of General Practice and gives good insight into disease in its social context. Staff Another huge benefit to the surgery is the friendliness of the team. From district nurses to admin staff the atmosphere is warm and non-hierarchical. Summary Would I train there again? Yes without question. It was the best year of my medical career. Get over the distance factor and the feeling that inner city GP is the place to train and get yourself to Winscombe. Page 4 of 4
