DEPARTMENT OF SURGERY WITH ANAESTHESIOLOGY № 2 Address: Klinichna str., 1, Ternopil, 46002 Tel.: (0352) 273341 Staff of the Department The Department includes 2 professors, 4 associate professors, 2 assistants and 2 laboratory assistants The Department was founded on the 1st of June 2009 on the basis of Ternopil regional clinical hospital after the reorganization of the Department of surgery with urology and anaesthesiology. The Head of the Department PhD., Prof. Oliynyk O.V. The Department is headed by PhD., Prof. Olyinyk O.V. , the author and co-author of 80 scientific publications, 7 inventions, co-author of the monograph devoted to the problems of osteoporosis. The Department includes: Associate Professor, PhD Vayda A.R., Associate Professor, PhD Hospodarsky A.Y., Associate Professor, Ph.D. Chepil I.V., Associate Professor, Ph.D. Kostiv S.Y., Professor, MD Hnativ V.V., Associate professor, PhD Kostiv O.I. and Assistant Soroka Yu.V. Hnativ V. V. has 62 scientific works, 4 inventions and 19 innovations. He is one of the authors of the monograph "Craniocerebral Hypothermia in the Treatment of Acute Poisoning'', CD-book and the textbook "Anaesthesiology, Resuscitation and Intensive Care of Emergency Conditions". Today the Department includes 2 professors, 4 associate professors, 2 assistants and 2 laboratory assistants. Education The fundamentals of surgery, anaesthesiology and intensive care are taught for the international students on the basis of the clinic. Interns - anaesthesiologists, students, interns in the following disciplines: family physicians, emergency physicians, surgeons, traumatologists, obstetrician-gynaecologists, neurologists and others are also trained at the Department. Thus, the clinic participates in the academic process of three faculties - the Faculty of Foreign Students, the Medical Faculty and the Faculty of Postgraduate Education. Credit-module system is introduces into the educational process of all courses. Materials for practical classes, presentations of lectures, methodical instructions for students, algorithms of practical skills, educational materials for tutors have been worked out at the Department. According to the concept of the University the Faculty of Foreign Students continue to improve skill-oriented education. As a result, cycle system of education has been introduced at the Department. Correspondingly, surgery and anaesthesiology specialized rooms have been arranged, schedules and curriculum have been corrected. All these improved the efficiency of the educational process. The following stages of the learning information system have been carried out: work programs, methodical instructions, schedules of teacher duties, slides and lecture presentations, lectures for students which contain text material, the necessary images, photos and video are downloaded on the Web-portal of the University. Information system of the teachers rating evaluation, including educational, scientific and clinical work is also downloaded online. The reorganization of Ternopil regional hospital into the Clinical University Hospital helps to prepare students for practical activities and to engage them in the patient examination and to participate in diagnostic manipulations. Examination of patient by the student of the Foreign Faculty under the teacher supervision "Clinical Surgery" manual, translated into English, is the main manual in the training of international students. We have discussed at the departmental meetings and implemented System of students’ knowledge assessment, which includes survey of students, developing practical skills, solving situational problems and control of deontological demands has been discussed at the Chair Meeting and implemented into the educational process. Teachers use different situational tasks in the educational process especially for "Step-2" exam preparation. Many patients were discussed at clinical rounds and clinical conferences for better understanding of the subject and diagnosis. The Department constantly updates and improves themes of lectures and practical classes. About two thousand situational tasks and tests with pictures, approximately six thousand text tests for clinical and theoretically knowledge evaluation in the electronic system "Moodle" for the IV, V and VI year students of the Foreign Students Faculty have been created at the Department. The manual “Surgery” for 1056 pages edited by Prof. NAMS of Ukraine, Honoured Worker of Science and Technology of Ukraine, MD, Professor. L.Ya.Kovalchuk, for medical universities of III-IV levels of accreditation was published in 2010. The staff of the Department also participated in the creation of this. Clinical work Surgery with Anesthesiology Department №2 provides specialized surgical care of patients with vascular, surgical and proctologic pathology; provides anesthesia for surgery in Ternopil University Hospital. Doctors annually perform more than 1000 complex operating procedures. Clinical basis of the Department is the Department of vascular surgery and the Department of anaesthesiology and intensive care. The staff of the Department continuously develops and implements modern methods of treatment. In clinical practice we have been able to introduce methods of prevention of reperfusion syndrome after reconstructive operations on the aortic-hip segment in patients with chronic critical ischemia of lower extremities, a method of differentiated approach for the prevention of thromboembolic complications, not only in patients with vascular pathology, but also in patients with common surgical pathology. During surgery on the hip-popliteal segment Associate professor Vaida A.R. and Associate professor Kostiv S.Ya Professor Venger I.K. According to the modern standards, doctors perform low invasive operations including endoscopic subfascial dissection of veins in patients with postthrombophlebitic syndrome. During minimally invasive surgery in the operating room MD Kostiv O.I. Diagnosis and differential diagnosis diseases of the arteries and veins have been significantly improved thanks to ultrasonic duplex scanning of vessels implementation into clinical practice. Surgeries in injuries of blood vessels, embolism and thrombosis the arteries, acute venous thrombosis are among the most urgent surgeries. The Department widely uses inhalation endotracheal anaesthesia sevorane with the help of the anaesthesia apparatus «Leon», uses the membrane plasmapheresis procedure, acute hemodialysis in the relevant categories of patients, drug-induced fibrinolysis in thromboembolic complications, uses different methods of noninvasive ventilation by means of «Drager» device which are controlled by volume and pressure. In the wards of intensive therapy constant observation of patients, which includes cardiomonitoring, monitoring of blood gas and respiratory gas, acid-base investigation states is made. Professor Chepil I.V. examines seriously ill patients in the Intensive Care Department The clinic staff performes intensive infusion-transfusion therapy, correction of fluid and electrolyte, acid-base status, parenteral and enteral nutrition, antibiotic therapy, uses different methods of detoxication therapy. New methods of optimizing presurgical preparation, intraoperative correction and postoperative treatment of patients with peptic ulcer disease have been developed in the clinic. Methods determining the oxygen saturation of the bowel wall and its viability in colonic obstruction, methods of its prevention and treatment failure of primary anastomosis during colon and rectum surgery have been elaborated and implemented. Clinical workers regularly visit and treat patients in regions. Along with this, clinical staff has the duty in emergency medical care. All teachers of the Department passed postgraduate study at the University Hospital of South Carolina (USA), Vienna Medical University Hospital (Austria), Prague Hospital (Czech Republic), the leading clinics of Russia and Ukraine. The Department has close working relationship with Anaesthesiology and Intensive Care clinics in (Wroclaw University, Poland) and with West London University Hospital (London, UK). The staff of the Department presented their reports at the XXII Congress of Surgeons of Ukraine (Vinnytsa, 2010), at the III Congress of Vascular Surgeons of Ukraine (Donetsk, 2010), at the III Congress of Coloproctologists of Ukraine in which with Central and Eastern Europe also took part (Odesa, 2011), at the III and IV conferences with international participation "Angiology and vascular surgery "Suharyevsky reading"" in 2010 and 2011, national and regional surgical conferences. During the training in the centre of robotic surgery in the hospital «Na Homolce» (Czech Republic) Assoc. prof. Hospodarsky A. uses “Da Vinci” system device. Research Scientists have published 10 monographs, 5 textbooks, over 450 scientific works, 16 patents. One Doctoral and four Candidate dissertations are being worked out in the clinic, English textbook in anaesthesiology and intensive care is being finished. In May 2011 clinic organized scientific-practical conference "Galician anaesthesiology reading: issues of anaesthesiology and intensive care," in which over 300 scientists and practical anaesthesiologists from 11 regions of Ukraine and from Poland, Great Britain, USA took part. Such conference is planned to be organized annually. Perspective fields of the clinic are: development of minimal invasive methods of treatment of the aneurysm of the abdominal aorta and peripheral arteries and new methods of operating of extracranial arteries, the introduction of standards for treatment of emergency in anaesthesiology, intensive care, research and development of diagnostic and treatment efficacy of hypoxic conditions in the practice and experiment; development of minimally invasive methods of treatment of proctologic patients, prevention of early postoperative complications in operations on the colon. Workers of the Department plan to continue the development of inhaled anaesthesia methods, implement the use of BIS-monitoring during anaesthesia, total parenteral nutrition "three in one" perfect extended noninvasive ventilation with various kinds of laryngeal masks. The staff of the Department plans to train in the Wroclaw Medical University (Poland) and in the hospitals in London (UK). The Department staff does not stop on the current achievements and planning future scientific and medical professional growth.
