GENERAL ENROLLMENT CONTRACT

The undersigned (collectively, the "Parent"), each being the parent or legal guardian of
("Student(s)"), hereby agree that in consideration of the enrollment of Student(s) and the reservation of a place for Student (s) in the grade and
academic year specified above at Lawton Christian School ("LCS"), the undersigned agree to pay the full annual tuition amount along with all
other applicable fees for re-enrollment/enrollment, morning care, extended care, and all other fees.
The full annual tuition amount shall be paid in accordance with the following Payment Option selected:
OPTION 1 (Annual Payment) (2nd child - 10% discount/3rd (or more) child(ren) - 15% discount): A single payment of $
payable on or before August 1.

OPTION 2 (Semi-Annual Payments) (2nd child - 10% discount/3rd (or more) child(ren) - 15% discount): The first payment of $__________ shall be due and

OPTION 3 (Monthly Payments) (2nd child - 10% discount/3rd (or more) child(ren) - 15% discount: The first payment of $_____________ shall be due and
payable on or before July 1, for 7th - 12th grades and August 1, for K3 -6th grades. Thereafter, monthly payments of $___________shall be due and
payable on or before the first day of each succeeding month for an additional ten months for 7th - 12th grades and an additional nine months for K3 – 6th
grades. The last monthly payment shall be due and payable on or before May 1.

To reserve a space for Student for the above stated academic year, this Contract and the applicable non-refundable re-enrollment fee or
applicable non-refundable enrollment fee must be received by Smart Tuition (a third party billing company retained by LCS), and this Contract must
be signed by an authorized person in LCS's business office. Parent understands Parent's unconditional obligation to pay the full annual tuition for
the 2015-2016 academic year, that after June 1,2015, no portion of fees paid or outstanding tuition shall be refunded or canceled in the event of
absence, withdrawal, or dismissal of Student from LCS. If after June 1,2015, Parent shall notify LCS that Student elects not to attend LCS, Parent
shall still be financially responsible and obligated to pay LCS the full annual tuition amount in accordance with the Payment Option selected above.
Payments made after the 5th day of the month for all Payment Options shall be subject to a $40 late fee (payable to Smart Tuition). Students may be
ineligible to participate in sports or other activities if tuition and fees are not current. Student records and report cards shall not be released by LCS nor
shall Student be eligible to graduate, if applicable, from LCS unless the annual tuition all other applicable fees are paid in full to LCS. Parent also
understands that LCS reserves the right to fill Student's space at LCS with another student upon Parent's failure to make any tuition payment beyond 60
days in arrears, but any exercise of such right by LCS shall not release or discharge Parent from the obligation to pay the full annual tuition amount and
all other applicable fees to LCS. Any exceptions to policy are not handled by administration but are taken up at the next regularly scheduled board
meeting.
LCS believes that a positi ve and constructive working relationship between LCS and Parent is essential to the accomplishment of LCS's educational
mission . LCS accordingly reserves the right to terminate or not renew this Contract if LCS concludes that the actions of a Parent make such a positive and
constructive relationship impractical or in the event a Parent seriously interferes with LCS's accomplishment of LCS's educational purposes.
Further, Parent understands and agrees that in signing this Contract for the 2015-2016 academic year, Parent accepts, and Student shall abide by, all of
the rules, procedures and regulations set forth in LCS's Student Handbook. Parent hereby releases LCS from any and all claims, damages, liabilities,
judgments, actions and causes of action that may arise with respect to any damage to property of, or injury to Student, unless caused by the gross
negligence of LCS or LCS's employees. In addition, Parent hereby consents to the use by LCS of any photograph of Student for use in LCS brochures,
promotional literature, yearbook, website, etc.

Facsimile signatures and/or electronic signatures (including on-line signatures) appearing hereon shall be deemed to be originals and this Contract may be
executed in multiple counterparts, each of which shall constitute an original and all of which together shall constitute one and the same instrument.
This Contract shall be interpreted in accordance with the laws of the State of Oklahoma and shall be amended only by written agreement of the parties
hereto. If any portion of this Contract is deemed unenforceable by law, that portion shall be stricken and the remainder of this Contract shall remain in
full force and effect.
