National Family Caregiver Support Program (Title III E) Specifications of funding Introduction and Administration on Aging Guidelines The National Family Caregiver Support Act (Title III, Part E) permits Area Agencies on Aging to contract with entities to provide services that support family caregivers. This funding must be used to supplement not supplant existing services. This funding may be used to provide a variety of supportive services including respite and care management for caregivers Land of Sky Regional Council's Family Caregiver Support Program Funding for Caregiver Directed Respite Land of Sky Regional Council's Family Caregiver Support program (FCSP) contracts with aging services providers to provide respite that is caregiver directed. Caregivers may select the type of respite that meets their need. Agencies that receive funding must allow family caregivers to determine how they will use their respite funds. Caregivers must be able to select the type (in home care, adult day services, short term facility care, group respite, or a combination of these services) of respite they will use these funds for. The caregiver rather than the care recipient is the client . Funding is available for: Caregiver Directed Respite Funds Caregiver respite services offer temporary, substitute supports or living arrangements for the person the caregiver is supporting in order to provide a brief period of relief or rest for caregiver. Agencies receiving these awards will administer a caregiver directed respite program for eligible caregivers. Caregivers will be able to choose home care, adult day services, short-term facility care or a combination of these. The contractor will partner with or purchase services from other providers or persons if necessary to allow these options. Contactor Responsibilities The contractor will partner with or purchase services from other providers or persons if necessary to allow caregivers to choose home care, adult day services, short term facility care or a combination of these services. 1) Grant Manager will ensure that all clients enrolled are eligible for NFCSP funded respite services In order to receive NFCSP funded respite clients must also meet these requirements. 2) Caregiver is an adult age 18 or older Care recipient is age 60 or older with impairment in at least 2 activities of daily living or individuals of any age with Alzheimer's disease and related disorders with neurological and related brain dysfunction. Traumatic brain injury is not a related disorder. Contractor will ensure that the NC Division of Aging and Adult Services (DAAS) Client Registration Form 101(CRF) is completed and signed by the caregiver at enrollment and updated annually. The registration form must also be updated and entered into Aging Resource Management System (ARMS) whenever there is a change in status. 3) Contractor will ensure that enrolled caregivers are informed about the consumer contributions policy, their client rights and their right to confidentiality. 4) Contractor will ensure that the client registration form and service units received per month are entered into the ARMS reporting system monthly. Caregiver Case Assistance Funds Case Assistance is a consultation with the family caregiver to identify their needs and inform them about community resources and develop a care plan for the caregiver. The contractor will assist the caregiver in accessing these services when necessary. Follow-up will be provided to conclude the service. The contractor receiving these awards will complete a thorough caregiver assessment, develop a care plan for the caregiver and follow up with the caregiver. Contactor Responsibilities 1) Grant Manager will ensure that all clients enrolled are eligible for NFCSP funded caregiver case assistance Caregiver is an adult age 18 or older Care recipient is age 60 or older or has Alzheimer’s Disease or a related dementia. 1) The contractor will inform caregivers about community services and resources including information about the various respite options and providers. 2) Contractor will ensure that the NC Division of Aging and Adult Services (DAAS) Client Registration Form 101(CRF) is completed and signed by the caregiver at enrollment and updated annually if carried over. The registration form must also be updated whenever there is a change in status and entered into the Aging Resource Management System (ARMS). Units are to be entered under services code 823 3) Contractor will complete a thorough caregiver assessment, develop a caregiver care plan and follow-up. A face-to-face assessment visit is recommended.. 4) Contractor will ensure that enrolled caregivers are informed about the consumer contributions policy, their client rights and their right to confidentiality. 5) Family Caregiver Support Program funds should be used to supplement not supplant existing services. These funds should be reserved for families that do not receive other federal or state assistance.
