Abstract Title: CARCINOMA IN SITU ARISING IN A LATERAL CERVICAL CYST: THE MISSING LINK

The existence of branchiogenic cleft cyst carcinoma(BCCC) is extremely debated entity and cystic
metastases to cervical lymph nodes originating from oropharyngeal squamous cell carcinoma occurs
far more commonly and should be suspected particularly in adult patients presenting with cystic
masses in Level 2 or 3 of the neck. A diagnosis of a BCCC can be made only if criteria laid down by
Martin et al and modified criteria by khafif et al are fulfilled.
A 51 year old male presented at our hospital with lateral cervical cyst. A fine needle cytology aspirate
was performed which showed dysplastic squamous cells and a possibility of metastatic carcinoma was
given. No other lesion was detected in the oropharyngeal/nasopharyngeal region on extensive physical
and radiological work up. Thoracic and abdominal work up was also normal. Excision of the cyst was
performed. Extensive sampling showed only Carcinoma in situ in the stratified squamous lining of the
cyst wall. There was no evidence of invasion. A diagnosis of Carcinoma-In-Situ arising in lateral
cervical cyst was given. There are very few case reports of carcinoma-in-situ arising in lateral cervical
cyst in published literature and molecular/genetic basis of this phenomenon needs to be deciphered
at large, however we strongly believe that this represents the missing link between BCCC arising
from the branchial cyst.
