Health Information Specialist Page 1 of 3 POSITION TITLE: Health Information Specialist EFFECTIVE DATE: 2010 POSITION STATUS: Non-Exempt REVISED DATE: 1/11, 1/14, 7/14 SUPERVISOR: Clinical Services Executive POSITION SUMMARY: The Health Information Specialist provides support functions to maintain an effective and efficient Health Information (HIM) department that is compliant with all state, federal, accreditation and internal policies related to agency documentation and records. The position provides general clerical and administrative support for the Health Information department. This position works with confidential clinical records to fulfill regulatory standards for accuracy and completeness of hospice documentation. In addition, this position assists the Admissions Team to assure that clinical records are handled in a prompt and efficient manner and that the daily workflow within the HIM department is the most efficient use of agency resources. ESSENTIAL JOB FUNCTIONS: 1. Completes the paperwork and computer data entry for new admissions. Assures that all forms have required signatures. Sends admission letter to attending physician. 2. Understands health information systems and maintains a working knowledge including upgrades. Maintains networking related to HIM as a resource and expertise in the area of Health Information Management – collaborates with IT and Clinical Education to ensure quality monitoring and education to clinical staff regarding the patient record. 3. Coordinates the gathering, monitoring and filing of ongoing clinical regulatory paperwork. Reviews all forms for completeness. 4. Ensures appropriate diagnostic coding according to hospice regulations for reimbursement. 5. Prepares interdisciplinary team meeting agendas and monitors paperwork from meetings. 6. Maintains tracking logs – census, patient transfers, renewal physician orders, benefit certifications, etc. 7. Communicates verbally and electronically in an effective manner with internal and external customers to complete regulatory requirements. 8. Completes a chart audit of all charts at time of patient death/discharge. Sends a death/discharge letter to Attending Physician. 9. Monitors daily workflow within the HIM department and delegates as needed. 10. Maintains and assures daily scheduling and coverage for HIM department. 11. Provides resource to the HIM department staff as appropriate. Seeks out development and learning opportunities for HIM department in collaboration with agency goals and with direction from the CSE. 12. Works in a consistently positive and cooperative manner with fellow team members 13. Tracks physician licensure and physician disciplinary actions and updates appropriate internal resources. 14. Works with external agencies to develop new processes for delivery of patient documents per regulatory requirements. 15. Coordinates with external agencies for release of Protected Healthcare Information (PHI). 16. Maintains clinical record after patient discharge/death per regulatory requirements. 17. May provide healthcare information management support to off site locations. 18. Participates in the organization’s quality assessment and performance improvement (QAPI) program at a departmental or agency-wide level. 19. Minimum 2 years experience in Health Information Management and most recent levels of coding certification required. 2. Effective organization and interpersonal skills. Excellent verbal and written communication skills and the ability to work independently with minimal direction. 3. Attention to detail and excellent computer skills required. EMR experience required. 4. Ability to contribute to the organization’s vision and mission, maintain confidentiality of all PHI 5. Maintain appropriate personal and professional boundaries, interact effectively as a team member, and work in a self-directed and dependable manner. 6. Dependable personal transportation with Colorado driver’s license and proof of current auto insurance. 7. Must be able to speak, read and write in English. Spanish speaking ability beneficial 8.
