COOPERATIVE EDUCATION APPLICATION FORM
BRAMPTON CIVIC HOSPITAL
Non-Clinical Placements – 2015 Semester 2
We are pleased to offer an exciting opportunity for high school students delivered through a unique partnership
between the Dufferin-Peel Catholic District School Board, the Peel District School Board and William Osler Health
System. Students will be working in an office environment within the hospital with limited patient interaction. They will
be interacting with hospital employees, visitors and volunteers depending on their placement.
General Information
 Students must be 16 years of age or older at the start of their co-op placement. A positive attitude, punctuality
and commitment to placement are essential.
 During the interview students should be prepared to talk about what they hope to gain from their hospital
experience as well what they have to offer the hospital clinic they may be working in.
 Students will be required to have their doctor complete a medical form and have a 2-step TB test completed
BEFORE they begin their co-op placement.
Placements Available For Semester 2 2015: (see attached descriptions on the following pages)
 Volunteer and Community Resources (1 full time position at Brampton Civic - will consider 0.5 students)
 Finance (2 full time positions available at the Airport Road location)
 Occupational Health (3 full time positions available – will consider 0.5)
 Diagnostic Cardiology Clinic (1 full time position available – will consider 0.5)
 Diversity Services (l half day position available)
 Hospital Customer Service (1 full time position available- will consider 0.5)
 Data Analysis/Gift Shop Business (1 full time position – will consider 0.5)
 Gift Shop (1 full time position available – will consider 0.5)





Students will:
be provided with a generic hospital identification badge/parking card and the use of a day locker(excluding the
Finance position)
be required to wear a current school photo ID badge
be required to purchase a co-op t-shirt for $15.00 to be worn at all times during their placement (excluding the
Finance position)
attend a hospital orientation
not be allowed to begin their placement until all the necessary medical history forms and TB, tetanus, diptheria,
polio, and rubella shots are completed and documented. This medical information will be forwarded to the
students, after acceptance for placement, and must be submitted to the hospital before their placement begins

NON CLINICAL CO-OP PLACEMENT POSITION DESCRIPTION
TITLE: Volunteer and Community Resources Co-op Student
DEPARTMENT: Volunteer and Community Resources
REPORTS TO: Volunteer Resources Staff
TIME AND PLACE: Monday to Friday, during business hours; Half day or full day students accepted. Located in the
Volunteer and Community Resources office at the Brampton Civic Hospital
PRIMARY FUNCTIONS: General Administrative / Receptionist Duties.
RESPONSIBILITIES:
1. To provide office assistance to the Volunteer and Community Resources staff
2. To assist with receptionist duties (particularly over the lunch hour)
3. To answer the telephones
4. To welcome volunteers and visitors
5. To assist with inquiries about volunteering
6. To distribute volunteer application forms
7. To assist with photocopying, filing and other administrative duties
8. To assist with distributing supplies throughout the hospital to volunteers
9. To learn key volunteer placement areas and fill in shift if the volunteers are absent (Lobby Connections,
Emergency Connections)
10. To help monitor attendance of student volunteers
11. To assist at orientation of new volunteers
12. To assist with the intake process of new volunteers i.e. position description, sign in procedures, uniforms etc.
13. Other duties as assigned
QUALIFICATIONS
 Strong customer service and people skills
 Strong communication skills, both written and verbal
 Strong work ethic; ability to work independently and problem solve
 Must be detail oriented, conscientious, mature, display professional appearance
 Proficient in computer skills: Excel and Word programs etc.

 General co-op orientation provided by VCR staff’
 Training on specific co-op duties will be provided by VCR staff

NON CLINICAL CO-OP PLACEMENT POSITION DESCRIPTION
TITLE: Finance Clerk Co-op Student
DEPARTMENT: Finance Department
REPORTS TO: Supervisors, Accounts Payable, Accounts Receivable Finance department
TIME AND PLACE: Monday to Friday during normal business hours; Full day students only; Located at the Airport Road
Campus – 9445 Airport Road, Brampton (see map on the following page)
PRIMARY FUNCTIONS: General office duties
DUTIES:
1. Assist with opening incoming departmental mail
2. Tracking of return mail; mailing of invoices and statements/sorting and distribution of mail
3. Photocopying/ typing assignments in the receivables/payables areas
4. Filing documents in numerical and alphabetical sequence;
5. Data entry/ obtaining patient information from the patients/families for billing purposes;
6. Other duties as assigned.
QUALIFICATIONS:
 Languages – moderate level of English, verbal and written skills
 Physical Abilities – good hearing/visual ability
 Clerical Skills – interest in and ability to learn clerical skills, previous office experience / skills an asset
 Interpersonal Skills – Focus on patient/customer oriented service. Co-op students are part of the health-care
team working in a busy, sometimes stressful environment
 Courses - Completion of Financial Accounting Fundamentals BAF 3M or Accounting Essentials BAI 3E
 Future Plans - A strong interest in pursuing a career in the financial sector
TRAINING:
Training on specific co-op duties will be provided by the Finance department staff
General co-op orientation will be provided by the Volunteer and Community Resources department

TITLE: Occupational Health Co-op Student
DEPARTMENT: Occupational Health
REPORTS TO: Occupational Health staff
TIME AND PLACE: Monday to Friday during normal business hours: Full day or half day students accepted. Located in the
Occupational Health department the at the Brampton Civic Hospital
PRIMARY FUNCTIONS: General office, administrative duties
DUTIES:
1. Assist the Occupational Health Department as required
2. Occupational Health project work as required
3. Answering phones, filing, and preparing new charts
4. Job shadowing to learn the various aspects of the Occupational Health department
5. Other duties as assigned
QUALIFICATIONS:
 Excellent organizational skills
 Ability to work independently
 Punctual and good attendance
 Customer service oriented
 Must be conscientious, mature, display professional appearance
 Detail oriented
 Excellent communication skills – both written and verbal
 Good computer skills, i.e. efficient in Excel and Word

TITLE: Diagnostic Cardiology Clinic Co-op Student
DEPARTMENT: Diagnostic Cardiology
REPORTS TO: Clinical Care Leader
TIME AND PLACE: Monday to Friday during normal business hours; Half day or full day students accepted. Located in the
Diagnostic Cardiology department at the Brampton Civic Hospital
PRIMARY FUNCTIONS: General Office and Administrative Duties.
RESPONSIBILITIES:
1. To provide general office assistance to the various Program Assistants
2. To assist with preparing mailings and processing mail
3. To make reminder phone calls for clinic patients
4. To welcome and direct visitors
5. To direct patients into examination rooms and change paper linens in between patients
6. To assist with office filing and photocopying
7. To assist with ordering office supplies
8. To assist with preparing packages for Cardiac Rehab department
9. To assist with set up of lecture/educational classroom for Cardiac Rehab
10. Other duties as assigned
QUALIFICATIONS
 Ability to work independently and posses strong organizational skills
 Dependable, patient and friendly.

TITLE: Hospital Customer Service Co-op Student
DEPARTMENT: Volunteer & Community Resources
REPORTS TO: Volunteer Resources department staff
TIME AND PLACE: Monday to Friday during normal business hours; Half day or full day students accepted. Located in
various main entrances at the Brampton Civic Hospital
PRIMARY FUNCTIONS: General Customer Service and Way finding duties.
RESPONSIBILITIES:
1. To provide pro-active customer service at the main entrances of the hospital
2. To greet, direct and escort visitors and patients throughout the hospital
3. To look up information for visitors and patients
4. To resolve or refer to the appropriate person any problems or concerns as they are received at the main entrance
5. To provide way finding assistance to patients and visitors in areas throughout the hospital
QUALIFICATIONS
 Strong customer service and people skills
 Excellent ability to learn quickly, adapt to frequent structure change, and use a variety of skills
 Ability to work independently and problem solve
 Dependable
 Must be conscientious, detail oriented, mature, display professional appearance

NON CLINICAL CO-OP PLACEMENT POSITION DESCRIPTION
TITLE: Diversity Services Co-op Student
DEPARTMENT: Diversity Services
REPORTS TO: Administrative Assistant
TIME AND PLACE: Monday to Friday during normal business hours; half day students accepted. Located in the Diversity
Services department at the Brampton Civic Hospital
PRIMARY FUNCTIONS: To provide general office assistance to the Diversity Staff in planning workshops, in-service and
training activities in Diversity Services
RESPONSIBILITIES:








Assist with printing and disseminating E-Learning Certificates
Assist in updating the staff database
Update the Community Resources Guide
Participate in planning process of Diversity/Ethics Forums
Participate in any presentations/in-services/meetings etc
Participate in the interviewing process of staff in the Dialysis/Renal units at Osler (exposure to the clinical side of
the house)
To assist with office filing and photocopying
Other duties as assigned

TITLE: Service Excellence Data Analysis/ Gift Shop Business
Co-op Student
*This role involves a mix of various placement areas
DEPARTMENT: Service Excellence/ Volunteer Association
REPORTS TO: Data Analyst/ Gift Shop Manager
TIME AND PLACE: Monday to Friday during normal business hours; Half day or full day students accepted. Located in a mix of
various departments at the Brampton Civic Hospital
PRIMARY FUNCTIONS: General Customer Service and Administrative Duties.
RESPONSIBILITIES: Data Analysis
1. Preparation of Recognition Friday names for inclusion into Frontline Heroes
2. Preparation of Recognition Friday names for the creation of the Leader Board
3. Running of the Monday update report
4. Miscellaneous data entry through Excel or Survey Monkey
5. Participation in analysis
6. Preparation of PowerPoint presentations
7. Participation in Service Excellence quarterly town hall meetings (if student volunteer is available)
8. Special projects as they come up.
RESPONSIBILITIES: Gift Shop Business
1. Retail sales – to assist customer and sell shop merchandise
2. Merchandise as required.
3. Unpack and price received stock
4. Replace sold stock (from Storage or Stock Room)
5. Prepare Baby Baskets
6. Take phone orders and deliver gifts
7. Blow up balloons and balloon heads
8. Keep book cart in order and replace books from storage room as necessary
9. Organize stock on shelves and empty boxes in stock room
10. Fill Nevada box as needed and set up Nevada for sale
11. Dust, clean, sweep and dry mop store
12. Gather all garbage and take to garbage room in basement daily

Gift Shop at Brampton Civic Hospital
PRIMARY FUNCTIONS: This position will offer the student and opportunity to learn retail sales skills and techniques,
retail point-of-sale systems, and the opportunity to work with a buyer/merchandiser to learn how best to display the
merchandise.
