This form is intended only for current SUNY Canton students who wish to change their major. This includes students who expect to
graduate from their current major and plan to pursue another degree program immediately following graduation, with no lapse in
attendance.
IMPORTANT: If changing major to a selective program, you must submit a Change of Major Request for Selective Program Consideration
instead. Selective programs include: Veterinary Technology (B.S.), Dental Hygiene (A.A.S.), Nursing (A.A.S.) Physical Therapist Assistant
(A.A.S.), Veterinary Science Technology (A.A.S.), and Practical Nursing (Certificate).

the appropriate Dean’s Office. (NOTE: Electronic copies will NOT be accepted).
