These tables show the number of residents in preliminary programs, core programs, and subspecialty programs in
2000, 2003, 2006 and 2009.
For the purpose of these tables, a core specialty is one that may be entered with no prior graduate medical education,
or with one preliminary year. A subspecialty is a specialty that normally requires more than one year of prior graduate
medical education as a prerequisite. Surgical specialties such as colon and rectal surgery, vascular surgery, thoracic
surgery and plastic surgery are counted as subspecialties of surgery, except when they are in integrated programs that
combine specialized training with the preliminary years.
A preliminary program is one with start- and end -dates that indicate a duration of 400 days or less.
Subspecialties that do not have at least 100 new residents or 200 residents on duty or 1,000 physicians in practice are
not listed separately, but are included in the subspecialty count in the row containing the primary specialty count. For
example, 1,366 persons began psychiatry training in 2009, and 618 began fellowships in psychiatry subspecialties. The
subspecialty count included 394 in child and adolescent psychiatry (shown separately) and an additional 224 beginning
fellowships in a smaller subspecialties of psychiatry (not shown separately).

Notes on Tables 4A and 5A:
If the degree of subspecialization were not changing, the number entering subspecialties in
the current year would equal the number of residents beginning training in the current year
who would eventually subspecialize. Although we know that such a steady state does not
exist, we do know that subspecialization rates are only slowly changing. We therefore
make the simplifying assumption that the ratio of those entering subspecialty training in the
current year to the number entering the primary specialty in the current year is the rate of
subspecialization. Thus, for example, we can subtract the 3,941 entering internal medicine
subspecialty fellowships in 2009 from the 6,551 entering general internal medicine
residencies in 2009 to estimate that 60% of those beginning internal medicine will
eventually subspecialize.
This approach allows us to gain early estimates of the degree of subspecialization that is
occurring. Of course, those beginning subspecialty training in the current year are not the
same individuals who are beginning primary training in the current year. For exact
determinations of subspecialization, cohort studies are necessary. Unfortunately, the cohort
studies that have been done show that residents completing a primary specialty in internal
medicine and surgery may return for subspecialty training two, three or even four and five
years after completing the primary residency. Thus, for a definitive determination of the
subspecialization of residents who began a primary specialty in 2009, we would have to
wait until 2019 or later. This simplifying assumption allows us to obtain more current
estimates of the subspecialization rates. However, they could be conservative estimates of
subspecialization trends.
Data for the number of residents expected to become general surgeons in 2009 is not
reliable and 2008 data have been substituted. Cohort studies have shown that residents in
preliminary surgery programs are often permitted to convert to categorical general surgery
programs. Some of the residents we have counted in preliminary surgery (and therefore
excluded from general surgery) may, in fact, end up in general surgery programs. This
artifact does not exist for earlier years, because subsequent reports to GME Track would
have corrected the end date to indicate a full five-year program. The same artifact may be
present for the most recent internal medicine figures also. In order to show trends, we
substituted 2008 data for the 2009 general surgery estimate. Because of this artifact, we
report the greater 2008 figure for general surgery in Table 5 instead of the 2009 figure
affected by the artifact.
