Scholarships for 2015-2016 Academic Year
The deadline for submission of applications is March 2, 2015. Scholarships will be determined by
May 2, 2015. All applicants will be notified by mail. You can download an application on the HBSSW
website under “Scholarship Opportunities” at: www.hbsswscholarships.uwm.edu All applications must
be typed to be accepted.
Some scholarships require that you submit information in addition to this application form. Read all
information about the scholarship(s) for which you are applying, and follow the requirements
accordingly. All materials must be submitted by the deadline. Only fully completed application
packages will be considered by the Scholarship Committee. All information in this application will be
treated confidentially and used only for consideration by the Scholarship Committee. Students who
have been found guilty of academic misconduct are not eligible for scholarships. Availability of some
scholarships may be subject to change without notice.
Please type and include all information requested. Handwritten and incomplete applications will not
be accepted by the scholarship selection committee.
