Eligibility: Students must be cancer survivors diagnosed before age 21 and age 25 or younger when submitting the

Washington or Wyoming, must be accepted without condition to an accredited university, college, community

Eligibility: Students nominated for this scholarship must a senior graduating on his/her appropriate career schedule and a
varsity-level athlete in one of the NIAA’s 16-sanctioned sports. Special consideration will be given by the
selection committee to those student-athletes who participated in multiple sports and/or achieved a great level of
success during their high school athletic career, participated in other school-based
activities/programs/departments, maintained a high level of academic status compared to fellow graduating
seniors from within his/her school, contributed in an outstanding manner to the community through service
projects, public commitments, etc. and exhibited high personal standards (i.e. honesty, integrity, etc.) in all
manners. In essence, the applicant must have served throughout the duration of his/her high school career, as a
positive role model for his/her fellow students. Please submit a 500 word essay on “How I displayed positive
character, and was a role model in my athletic and academic efforts”, high school transcripts, two (2) letters of
recommendation (one from principal or counselor and one from athletic administrator/director) and
sportsmanship statement (to be completed by coach). The student-athlete should deliver a completed

prevention, how it works, and whether current resources in the schools adequately address drug prevention from

Eligibility: Female African-American high school senior during the academic year of 2010-2011. Student must reside in the
states of Alaska, Arizona, California, Hawaii, Idaho, Nevada, Oregon, Utah, or Washington. Students can also
reside through the military installations in Okinawa, Tokyo (Japan), and Seoul (Korea) in which our organization
also services, legal U.S. citizen, less than 21 years of age, GPA of 3.0 on a 4.0 scale, or 2.0 on a 3.0 scale.
Students must receive official admissions from a Historically Black College or University during their senior year.
Students must be enrolled in and attend an HBCU starting the fall of 2011. Scholarship funds will be paid directly
to the HBCU.
