IMAGINE REDBRIDGE DAY SERVICES REFERRAL PROCESS Referrals for will be anticipated from Community Recovery Teams, Early Intervention, Home Treatment Team, Outpatients, RAAT and Assertive Outreach. Criteria for Referral for the Full Imagine Service The Service User Must: Live in the London Borough of Redbridge. Must be on a CPA / have a NELFT care co-ordinator or be FAC’s* eligible. Agree to being referred to the service Required Information A completed day services referral form A recent risk assessment Self referrals / Primary Care referrals are not acceptable Referral Process When a referral has been received each individual service user will be assessed for the service and a key worker will be allocated to the service user During the initial meeting a questionnaire is to be completed between the staff and service user wherein the service user identifies the goals they wish to work towards. The service user’s expectations and interests can also be discussed. Imagine can offer the following service for referrals. Recovery Support Planning – time limited one to one support in managing mental health, physical health, living skills and signposting through working on a recovery star model.. Social inclusion – time limited one to one support in expanding community opportunities and expanding friendships; whether cultural, education, leisure, arts activities User lead groups – empowering service users to run their own groups for leisure and recovery purposes. Community based groups - groups/activities supported by staff in the community to encourage social inclusion Peer support groups – Groups run by your peers within the wider community, such as bingo, art, and luncheon clubs. Drop in Service – a two hour drop in service is open 6 days a week for people to attend. Groups and social activities can take place during this time. ( FAC’s * = Fair Access to Care services) QP04-36 OTHER REFERRALS FOR NON-CPA / NON-FAC’S CLIENTS Imagine are able to receive referrals from service users under Secondary Care but who are not on a CPA / Fac’s. Support will be on a short term basis for a maximum 12 week period as service users are lesser severe mental health needs. The Initial Questionnaire forms the base from which goals are set. Note: Star Plan and Grist are not included in this care package. The following services can be offered to this client group. (Please note Drop ins’ are not available to this client group.) The action plan will include realistic goals for up to 12 weeks – this could include practical support and how often you will meet to review these. The service user’s expectations and interests can also be discussed.
