Locally Determined Assessment Procedures Please remember that having a student take the LDA to meet the graduation requirement is an IEP team (including general education teacher, student, and parent) decision and no decisions shall be made outside this process. Teams must keep in mind the following conditions: 1. Student must be receiving Special Education services (specific qualifying content areas no longer applies) 2. Be enrolled in CEDARS as a 12th grader and on track to meet all other state and district graduation requirements by the end of his/her planned graduation year 3. Participated and/or attempted a state assessment IEP Teams must follow the LDA process below: 1. The IEP team will meet and evaluate the student’s assessment history. Items to consider: Has the student ever earned a Level 3 score in that area? Has the student consistently earned Basic Level 2 scores? Has the student participated in the Off-Grade Level assessment? How many attempts has the student participated in? Is there a scored recorded for each of the required areas of testing? 2. The IEP team verifies that the student has met or is on track to meet all other state/district or IEP graduation requirements (standard credits, assessments, Senior Portfolio, IEP determined requirements) 3. If the LDA Option is being considered by the team, has the team: Verified if the student has taken any of the approved published standardized achievement tests in Grades 11 or 12 for Reading, Writing and Math Consulted with the school psychologist, or designated person, to administer one of the approved standardized achievement test(s), if needed. Verified whether or not the student¹s scores on the standardized achievement test are equal to or greater than the established minimums, if already taken. 4. After the IEP team meeting, the case manager will document the team’s decision whether to move forward with the LDA and then notify the student and parent with a PWN. An IEP Amendment is not necessary if the data and the team’s decision are clearly documented in the PWN. 5. If the student’s annual IEP is held after Senior Portfolios, then the team can simply document the LDA testing option in the testing section of the IEP. The IEP case manager should also note the team’s decision to administer the LDA within the Team Planning section of the IEP and within the PWN in the Other Factors section. Next, the school psychologist will complete the LDA Registration form, the case manager will sign the form and then hand deliver it to Pam Cooley, Office Manager. 7 Then, the District Special Education Director reviews and signs the application form and forwards the form to the District Assessment Coordinator. 8 The District Assessment Coordinator reviews, signs, and faxes the application form to OSPI. 9 OSPI will document that the student met the applicable high school standard(s) in the student¹s state assessment record. 10 Once the approval from OSPI is documented, the case manager may include the results of the LDA acceptance in the PWN that is included with the Summary of Performance paperwork. The acceptance of the LDA should be documented in the Other Factors section of the PWN. Learning Support Services March 25, 2015
