SAFE WORK PERMIT GUIDELINES
1.0
PURPOSE
This guideline defines minimum permit requirements, procedures, limitations and provides
forms for the issuance of safe work permits within the facility.
2.0
SCOPE
This guideline applies to all persons including Valero and contractor personnel and defines
permit requirements, procedures, limitations and forms used in the issuance of safe work
permits, including cold work, hot work, vehicle entry, excavation and confined space entry
permits to assure consistent safe work permitting within the facility. A safe work permit is
required for any Maintenance, repair or construction work within the facility. Permits are
generally not required for work performed in office buildings and Maintenance shops except for
confined space entry and hot work on equipment that has been in service. Cold work
performed by owning area personnel does not require a safe work permit.
3.0
DEFINITIONS

Acceptable Entry Conditions - The atmospheric, access/ egress, and work area
conditions that must exist in a confined space before entry is allowed.

Authorized Entrant - A person who is authorized under the terms of the confined space
entry permit to enter and exit the space.

Blanket Safe Work Permit - A cold work permit issued within a defined section (see
Section 5.1 - Permitting for Cold Work and Line Break) to allow work on a grouping of
closely interrelated or similar pieces of equipment (e.g., groups of exchangers, pumps,
vessels and connecting piping, etc.).

Cold Work - Any work activity which does not supply sufficient heat or spark energy to
provide a potential ignition source for a flammable mixture. This includes general work,
scaffolding, cleanup, etc. Hot work, vehicle entry, excavation or confined space entry
are not classified as cold work.

Competent Person - For the purposes of excavations, an authorized person who is
trained in and is capable of identifying existing and predictable hazards in the
surroundings, or working conditions which are unsanitary, hazardous or dangerous to
employees, and who has authorization to take prompt corrective action to eliminate
them.

Confined Space - A space that is:
•
Sufficiently large and so configured that an individual can bodily enter and
perform assigned work;
•
Has limited or restricted means of entry or exit;
•
Is not designed for continuous occupancy; and
•
May contain one of the following:
·
Contains or has the potential to contain a hazardous atmosphere;
·
Contains a material that has the potential of engulfing an individual;
·
Has an internal configuration such that an entrant could be trapped or
asphyxiated by inwardly converging walls or by floors which slope
downward and taper to a smaller cross-section; or
·
Contains any other recognized serious safety and health hazard.
•

Examples of confined spaces include but are not limited to: (a) tanks, (b)
reactors, (c) vessels, (d) towers, (e) drums, (f) sumps, (g) external floating roofs
greater than 12 inches below the maximum fill level, (h) excavations greater than
4 feet deep, and (I) tower skirts.
In addition, temporary shelters used for welding, inspections, etc., where there is
a potential for build-up of toxic materials or oxygen depletion may be considered
a confined space. These must be evaluated on a case by case bases by the
owning department.

Designated Attendant – A qualified individual ("hole watch") stationed outside a
confined space as required by the confined space entry procedure.

Entry - The action by which an individual passes through an opening into a confined
space. Entry is considered to have occurred as soon as any part of the entrant's body
breaks the plane of an opening into the space.

Entry Supervisor - The individual (Superintendent, Shift Supervisor, Lead Technician)
responsible for confined space entry activities. This function cannot be delegated.

Fire Watch - A qualified person designated to monitor the area of hot work involving
welding or cutting, take appropriate action to reduce risk of fire and if necessary
extinguish an incipient stage fire.

Hazardous Atmosphere - An atmosphere that may expose entrants to the risk
of death, impairment of ability to exit, injury or acute illness from one or more of the
following causes:
•
Flammable gas, vapor or mist in excess of 10 percent of the lower explosive limit
(LEL)
•
Atmospheric oxygen concentrations below 19.5% or in excess of 23.5%
•
Atmospheric concentration of any substance which could result in employee
exposure in excess of its permissible exposure limits (PEL)
•
Any other atmospheric condition that is Immediately Dangerous to Life or Health
(IDLH)

Hot Work- Work that causes or requires the use of open flames, arcs, sparks, or other
forms of high temperature ignition sources that could initiate a fire or explosion.
Examples of hot work include welding, burning, soldering, hot tapping/stoppling, drilling,
grinding, abrasive blasting, chipping, the operation of impact wrenches, the operation of
electronic or electrical equipment that is not intrinsically safe, opening explosion proof
electrical enclosures and any other work that may generate sufficient heat that it would
pose a possible ignition source.

Immediately Dangerous to Life and Health (IDLH) - Any condition that (a) poses an
immediate or delayed threat to life; or (b) would cause an irreversible adverse health
effect; or (c) would interfere with an individual's ability to escape unaided from a
confined space. The level of contaminant that would pose an IDLH atmosphere is
substance specific.

Inerting - The displacement of an atmosphere in a confined space by a noncombustible
gas such as nitrogen, to such an extent that the resulting atmosphere will not support
combustion or life.
NOTE - This condition results in an IDLH (oxygen deficient) atmosphere. Inert confined
space entries are not normally done by company personnel.

In-Service Welding - The practice of welding on equipment (e.g., tank, pipe, vessel,
exchanger, etc.) which has not been purged (gas free) and has not been removed from
service through conventional methods. This includes but is not limited to grinding,
burning or welding.

Joint Job Site Visit - A visit to the job area by an Operations person and the person to
do the work (or a representative of the person(s) to do the work) to ensure the safety of
the workers and the operation by identifying the hazards associated with the job and the
measures and equipment to be used to mitigate or eliminate the hazards. Safety
Department personnel may be included in the visit for higher risk or more unusual jobs.
The hazards listed and referred to in sections B – E of the permit form should be
considered, as well as all other potential hazards. When a job extends beyond the valid
date of the first permit, the extent of the Joint Job Site Visit evaluation may be reduced
(as compared to the first permit Joint Job Site Visit) if appropriate in the judgment of the
joint visitors.

Line Break - The intentional opening of a process system that may contain flammable,
corrosive, or toxic material, or a material under pressure or temperature such that an
unplanned opening of the system may result in injury to workers. Examples include
spreading flanges, opening exchangers, pulling pumps, cold cutting pipe, etc. Line
break, depending on equipment used, could be cold work or hot work.

Non-Hazardous Confined Space - A confined space which normally exists without
IDLH conditions. Entry into such spaces requires the issuance of a confined space
entry permit. No blinding is required by the Owning Department as part of the permit
issuance. Non-hazardous confined spaces are floating roof tank tops, tower skirts,
sunken valve and pump manifold areas, cooling tower cells, and fin fans.

Owning Department - The unit/area/department that is responsible for the location in
which the work is performed.

Owning Department Representative - Individuals who have received training and a
certification of competency in the issuance of permits and associated gas testing.
Certification of competency requires passing a standardized written examination.

Retrieval System - The equipment used for non-entry rescue of persons from confined
spaces such as a safety harness and life line.

Testing - The process by which the hazards that may confront personnel performing hot
work, vehicle entry, or authorized entrants of a confined space are identified and
evaluated.

Vehicle Entry - The entry of any motorized vehicle into the battery limits of a process
unit, inside tank farm diked areas, or other restricted areas except for trucks loading or
unloading at racks.

Mounted Enclosure – small enough to prevent complete physical entry (e.g., cabinet,
junction box, analyzer enclosure, etc.).

Walk-in Enclosure – similar to mounted enclosure, but large enough to allow complete
physical entry (e.g., walk-in compressor control equipment).

Non-Pressurized Building Containing Enclosure(s) – building containing purged or
pressurized equipment (e.g., analyzer building, blend building, etc.)

Pressurized Building Containing Pneumatic Controls – pressurized building
containing control equipment that has the potential to release purged air (e.g.,
pressurized control room)

Ensure that the atmosphere is continuously monitored during a
confined space entry as outlined in the Confined Space Permit
Preparation section of this guideline.
Permit Enforcement/Follow up:
When the permit must be updated during the shift while the permit is
valid, enter the update time(s) in the Permit Timing box and initial by
the updated items.
Cancel the permit if conditions of the permit are not being met or if the
area or equipment conditions change requiring either the termination or
re-issuance of the permit.
Ensure that the permit issuance procedures are being followed.
Ensure that the terms of specific permits are being followed.
Reviewing and signing hot work permits.
Ensure that completed entry and confined space entry permits are
returned to the Safety Department.
Ensure that all work carried out by personnel in their work crew is
covered by valid permits.
Ensure that all members of the work crew review the permit
requirements and sign the back of the permit form to confirm their
understanding. If the permit is changed or updated, ensure that all
members of work crew re-sign to confirm understanding of the
change(s).
Review and sign all permits issued for confined space entry, hot work,
hot taps, supplied air work, or work requiring additional safety
equipment such as slicker suits, goggles, face shields, etc., such as
that required for work in acid or caustic areas.
Ensure that a trained designated fire watch and/or designated
attendant is provided when needed.
Ensure that all personnel in their work crew that may enter a confined
space are trained as authorized entrants.
Determine the number of entry portals required for a confined space.
Ensure that the permit form is available at the job site at all times while
the job is underway.
Determine the number of entry attendants needed for multiple portal
entries into a confined space.
Determine the type or method of communication used between the
designated attendant(s) and authorized entrants.
Ensure that the atmosphere is continuously monitored during a
confined space entry as outlined in the Confined Space Permit
Preparation section of this guideline.
Ensure that all personnel in their work crew are aware of and trained in
the relevant chemical and process hazard information for the area in
which they are working.

Assigned Responsibilities
Obtain proper permits from the Owning Department and review and
accept the permit terms and restrictions prior to starting work.
Provide estimated number in work crew.
Assure that a joint job site visit as required in the permit preparation
procedure (Appendix 1) is performed.
Review the permit with the work crew.
NOTE: CONTRACTORS MUST REVIEW THE PROCESS UNIT
OVERVIEW WITH ALL PERSONNEL INVOLVED IN THE WORK
ACTIVITIES DESCRIBED ON THE PERMIT PRIOR TO STARTING
WORK.
 Ensure that the work described in the permit is completed and/or left in
a safe condition prior to leaving the work area, including cleaning the
area up.
 Check in with the confined space designated attendant when entering
and exiting a confined space.
 Maintain their copy of the permit at the work site.
 Ensure the Owning Department fully understands the scope of the work
to be done.
 Sign and return their copy of the permit to a person in the Owning
Department when the work is complete or the permit expires.
 Provide advice/training as required in gas testing.
 Sign initial confined space entry permits. (Non-Hazardous Confined
Spaces excepted, See section 5.5.1)
 Annually review the confined space entry program using completed
permits to determine where improvements, if any, can be made to
better protect confined space entrants (the annual review is
documented).
 Maintain files of completed work permits involving gas tests indefinitely.
 Establish general standards for acceptable entry conditions.
 Establish general standards for equipment required under this SHG.
 Develop and implement procedures and resources for rescue and
emergency services.
 Adhere to the terms and restrictions of the confined space entry permit.
 Check in and out of the confined space with the designated attendant.
 Use the safety equipment specified on the entry permit properly.
 Know the hazards that may be encountered during the entry and the
effects of overexposure to potential contaminants.
 Stay in communication with the attendant during entry and promptly
following any emergency instructions.
 Exit the space based on their own judgment, an order from the
attendant, or an alarm.

When exposed combustibles (wood, paper, rags, dry grass, etc.) are
present within 35 feet of the work area firewatch can only leave the
area after hot work has been stopped for thirty minutes or a relief
attendant has been obtained.
Have NO OTHER RESPONSIBILITIES that would interfere with his/her
reliably fulfilling hole watch duties.
 Know the hazards that may be encountered during entry and the
effects of exposure to the hazards.
 Maintain an accurate roster of personnel who enter and exit the
confined space.
 Remain outside the confined space.
 Monitor the atmospheric testing equipment used for continuous
monitoring and respond appropriately.
 Communicate with entrants and monitor status of a change in
conditions that would create a need to evacuate the confined space.
 Maintain a means of communicating with the Owning Department
and/or Maintenance.
 Order an evacuation of the confined space, performing non-entry
rescue if conditions warrant, or summon rescue or emergency services.
(THE ATTENDANT WILL NOT ATTEMPT ENTRY RESCUE!)
 Ensure that only authorized entrants are allowed into the confined
space.

DESCRIPTION
Upon receipt of a Maintenance work order or request, the Owning
Department will prepare a Safe Work Permit based on:
Understanding the nature and extent of the work to be done.
A review of the impact of the work on the operability/safety of the
process or general area by conducting a job site visit before the permit
is issued. Prior to more hazardous activities this visit is made
collectively with the Maintenance/contract craftsmen BEFORE THE
WORK IS ALLOWED TO BEGIN (see Appendix 1, Section B).
Communicating to the persons who will perform the work, the
personal protective equipment and other precautions that need to be
taken to perform the work safely.
Ensure that the equipment to be worked on is appropriately
prepared (i.e., locked out, drained, steamed, blinded, etc.), and if
being worked on while still operating, what additional precautions are
necessary. (Bring the green copy of the permit in the field for a
reference during jobsite preparation.)
Ensure that all personnel who may be impacted by the work are
aware that the work is commencing.
By the Owning Department representative.
By the work supervisor, designee or contract craftsmen.
By the Owning Department to the Maintenance/contract craftsmen that
will perform the work.
Permit to remain on the job site until completion of the work or the
permit expires, whichever occurs first.

for cold work can be done with prior agreement of Owning Department
and work crew supervisor.

DESCRIPTION
Upon receipt of a Maintenance work order or request, the Owning

 Understand the nature and extent of the work to be done.
 Review the impact of the work on the operability/ safety of the process
or general area by conducting a job site visit before the permit is
issued. This visit is made jointly with a representative of the work crew
BEFORE THE WORK IS ALLOWED TO BEGIN (see Appendix 1,
Section B).
 Performing gas testing of the area as appropriate where the work will
be performed; the boundaries of the area to be tested depend on the
nature of the work to be done.
 Ensure that the results of any gas tests taken are included on the
permit, including the serial number and date of last calibration of the
instrument used.
 Communicate to the persons who will perform the work, the personal
protective equipment and Fire Watch precautions that need to be
taken to perform the work safely.
NOTE - HOT WORK NOT INVOLVING OPEN SPARKS OR FLAMES
MAY NOT REQUIRE THE DESIGNATION OF A FIRE WATCH
ATTENDANT (SEE NOTES IN APPENDIX).

Ensure that the equipment or area is appropriately prepared, i.e.,
sewers covered, equipment drained and cleaned, combustibles
removed, blinds in place, etc. (Bring the green copy of the permit in
the field for a reference during jobsite preparation.)
NOTE: HOT WORK INVOLVING VESSELS OR DRUMS EQUIPPED WITH A
DEMISTER PAD, THE DEMISTER PAD WILL BE REMOVED OR FIRE
PROTECTED. In addition, where pyrophoric materials are concerned refer to
SHG #13.



If a Fire Watch is required, ensure that a fire extinguisher or charged
water hose is available at the scene of the work.
Ensure that all personnel who may be impacted by the work are aware
that the work is commencing.
Ensure that the name of any designated Fire Watch is included on the
form.

DESCRIPTION
By the Owning Department representative and the individual who did
the gas testing. (The name of the person doing the gas testing may
be printed in the space provided.)

Permit to remain at the job site until completion of the work (including
extended Fire Watch) or the permit expires, whichever occurs first.
Permit must be legibly written/completed.
Work described in the permit must commence within 2 hours of gas
testing; if it does not, a new gas test will be required prior to beginning
work.
In work areas where exposed combustibles (wood, paper, rags, dry
grass, etc.) are present within 35 feet of the work area, the firewatch,
or a relief attendant, is required to remain at the work site for a
minimum of thirty minutes after completion of permit-related hot-work
activities before turning in the work permit.
Unless special provisions are made, a new permit is required (and gas
test, if appropriate) at the beginning of each new work shift for the
Owning Department and work crew.

DESCRIPTION
Upon receipt of the request for a vehicle entry permit, the Owning
Department representative will prepare a Vehicle Entry Permit based on:
 Understanding the nature and extent of the work to be done.
 A review of the impact of the vehicle entry on the operability/safety of
the process or general area by visiting the general areas where the
vehicle(s) will enter, drive through and/or be operated.
 A gas test of the areas affected by the vehicle entry.
 Ensure that the results of any gas tests taken are included on the
permit, including the serial number and date of last calibration of the
instrument used.
 Ensure that all personnel who may be impacted by the work are aware
that the work is commencing.
 Ensure that the travel route is defined and communicated (including
pathways, street names and/or gate numbers), and a spotter is
assigned if necessary.

Prior to beginning the excavation and as part of permit preparation, the
Maintenance Department will contact if appropriate local or state
authorities, companies or utilities. Contractors must contact the
Maintenance Department before any excavation work is done.
Upon receipt of a Maintenance work order or request, the Owning
Department will prepare an Excavation Permit based on:
∙ Understanding the nature and extent of the work to be done.
∙ A review of the impact of the work on the operability/ safety of the
process or general area by conducting a job site visit before the permit
is issued. Typically, this will require a visit made jointly with the
company work crew or contractor representative BEFORE THE WORK
IS ALLOWED TO BEGIN (see Appendix 1, Section B). Drawings of
underground tanks, lines and utilities will be reviewed as needed.
∙ Perform gas testing of the area as needed.
∙ Ensure that the results of any gas tests taken are included on the
permit, including the serial number and date of last calibration of the
instrument used.
∙ Communicate to the persons who will perform the work, the personal
protective equipment and other precautions that need to be taken to
perform the work safely.
∙ Ensure that the immediate and surrounding area to be involved is
appropriately prepared, i.e., sewers covered, perimeter barricaded,
equipment drained and cleaned, combustibles removed, a spoil area or
container established, etc.
∙ Ensure that all personnel who may be impacted by the work are aware
that the work is commencing.

beginning work.
Unless special provisions are made, a new permit is required (and gas
test, if appropriate) at the beginning of each new work shift for the
Owning Department and work crew.
Expired permits and permits on work that has been completed are
returned to a person in the Owning Department that issued it.

CONFINED SPACE PERMIT
DESCRIPTION
 Is issued to the space, not to any specific craft or contractor.
 Must describe the method of communication between the attendant and
personnel inside the space.
 Is used by the designated attendant to keep a running tally of entrants
entering and leaving the space.
 Is used by the attendant to keep a record of initial and subsequent gas
tests of the space.
 Must remain with the designated attendant or posted at the entry portal,
normal access ladder/stairs, or other effective location for review.

Entry Supervisor. This action would be noted on the comment section

be marked in the comment section as "REVALIDATED."
 If the Confined Space Permit is canceled, all craft specific permits
referencing this permit must be canceled and reissued as well.
 After completion of the work or expiration of the permit, it must be
returned to a person in the Owning Department for cancellation and
subsequent return to Safety for filing.
NEW CONSTRUCTION:
 Any deviations of confined space entry permitting requirements will be a
joint decision by Maintenance, Engineering, Operations and Safety.
 Artificial lighting or power used within a confined space will meet the
following requirements:
∙ Use a 12-Volt power supply or a 120-Volt system with ground fault
circuit interrupters (GFCI's).
∙ Meet the requirements for Class I, Division 2, Groups C & D rating
where possible explosive or flammable atmospheres cannot be
eliminated.

Grounded air movers will be provided for all vessel confined space
entries where the design of the vessel will allow.
If the temperature within the space is in excess of 105o F, it will not be
permitted for entry without special provisions determined by discussion
between the Operations, Maintenance or Contractor Supervisor and
Safety.
Ventilation systems meeting the requirements detailed below must be
implemented, or supplied air respirators must be worn by welders and
other personnel inside the space when welding in the following confined
spaces:
∙ Work space is less than 16 feet high;
∙ Volume of the space is less than 10,000 ft3 per welder;
∙ Work areas where there are partitions, structural barriers, or other
barriers that significantly obstruct airflow (such as baffles, trays, or
limited access openings)
If the space meets one of the criteria above, one of the following
ventilation options must be implemented or supplied air utilized by all
persons in the space:
∙ Provide at least 2000 ft3 / min of airflow for each active welders; or
∙ Provide each welder with a local exhaust device capable of maintaining
a velocity of 100 fpm toward the air intake.
When arc welding inside of a confined space is suspended for lunch,
breaks or shift change, the electrodes shall be removed from the holders
and the holders located so that accidental contact cannot occur. Also,
the welding machine must be disconnected from the power source or
turned off in the case of a diesel-powered machine.
When gas welding or cutting inside of a confined space the torch valves
must be closed and the fuel-gas and oxygen supply to the torch must be
positively shut off at a point outside the confined space whenever the
space is evacuated for extended work stoppage (i.e. breaks or lunch).
Where practicable, the torch and hose must also be removed.
If cleaning and/or forced air ventilation do not adequately remove air
contaminants from the confined space or the work is such that it will
introduce additional contaminants into the atmosphere within the space,
RESPIRATORY PROTECTION WILL BE USED.
∙ Required respiratory protection will be specified by the Owning
Department Supervisor with advice as needed from Safety.
∙ Entry into an IDLH atmosphere will only be made after alternatives
are explored and found impractical and agreement is reached
between the Operations, Maintenance and the Safety
Manager/Superintendent or HS&E Director.
∙ Entry into an IDLH atmosphere will require that all entrants be
equipped with a positive pressure air line respirator with an
emergency egress pack and a safety harness and lifeline.

 Non-entry rescue retrieval systems will be worn by authorized entrants
unless the retrieval equipment would increase the overall risk of entry or
would not contribute to the rescue of the entrant.
 Authorized entrants will wear rescue retrieval systems including:
∙ an approved harness,
∙ retrieval line where it aids in rescue
∙ Wristlets and a retrieval line can be an acceptable alternative if the
harness is infeasible or less effective.
 The other end of the retrieval line will be attached to a fix point outside

Permitting for "Non-Hazardous" Confined Space Entry
Certain spaces meeting the OSHA definition of "confined space" exist within the refinery
by virtue of equipment of process design or layout. They are "confined spaces" either
due to their limited access or egress and are "not designed for continuous occupancy."
These spaces are identified by warning signs. Non-Hazardous confined spaces are:

Entry into such spaces requires the issuance of a confined space entry permit. However,
no blinding or area preparation is typically required by the Owning Department personnel as
part of the permit issuance.

HS&E personnel do not need to sign the Confined Space Permit for "non-hazardous"
confined space entry. All other signatures and requirements typical of the
preparation and issuance of these and the subsequent confined space entry permits
issued to each work group entering the confined space remain the same.

Permitting For Enclosures That May Contain Hazardous Gases

For Mounted Enclosures, Walk-In Enclosures, and Non-Pressurized Building Containing
Enclosures, appropriate labeling will be mounted warning of the potential hazards of oxygen
deficiency and of the entry requirements, where applicable.
For Walk-In Enclosures, and Non-Pressurized Building Containing Enclosures, access will
be controlled with a lock-and-key, a safe work permit, and monitored with initial air sampling
for oxygen content.
6.0
TRAINING
6.1
Company Employees

Prior to the assignment of duties under this SHG, employees will be trained in the execution
of the duties for which they are responsible.

Records of the training will be maintained in the plant Training Department files.

This training will include classroom and hands on training on the proper use of gas testing
equipment for permitting purposes.

Contractors

Prior to the assignment of duties under this SHG, contractor employees will be trained in the
execution of the duties for which they are responsible.

During their safety orientation, contractors will be informed of Valero’s’ permitting
procedures. Process Unit Overviews are posted at each unit and shall be reviewed with
each employee.

Contractor representatives will participate in the joint job site visits and discuss with the
Valero permit issuer the exact nature and extent of the work planned. Coordination of the
work with the Valero Maintenance and Owning Department will be required. The details of
how this coordination will be handled will be agreed upon with the Valero contractor
representative prior to the start of work.

Valero’s' confined space entry procedure comprises the minimum standards for any entry.
If any measure in excess of these requirements will be used by the contractor, this
information must be communicated to the Valero permit issuer prior to the issuance of the
work permit.

APPENDIX 1 - SAFE WORK PERMIT PROCEDURE
General Instructions on Safe Work Permitting
NOTE: The following sections refer to the corresponding letter on the Permit Form (See
Attachment 1).
A.
WORK SCOPE



This section shall be completed by the Owning Department.
When filling out the Permit, use as much detail as possible to encompass the complete
work scope. The work scope section contains information concerning the types of work
involved to complete the task, the specific location, the equipment to be worked on, the
work to be performed, and the craft/company that will do the work and the estimated
number of employees in the crew.
More than one box may be checked. For example, a work permit for an excavation could
also require a vehicle entry as well as a Confined Space Entry Permit. List each vehicle and
route of entry.
Generally, each craft and company must be issued its own Permit.
B.
POTENTIAL HAZARDS



This section is to be completed by the Owning Department.
BEFORE opening a vessel, Owning Department and Maintenance personnel should also
consider whether pyrophoric iron sulfide or other deposits may be present. If pyrophoric
materials may be present, personnel should follow the refinery pyrophoric hazard safety
instructions as part of preparing and issuing the safe work permit. (See SHG 13)
All of the actual or potential hazards which apply to the specific work activity will be
checked.
The Owning Department is responsible for providing relevant chemical and process hazard
information to contract work crews. Process Unit Overviews and Material Safety Data
Sheets are available to contractors at specified locations.
C.
PERSONAL PROTECTIVE EQUIPMENT

This section is to be completed by the Owning Department and the Maintenance or
contractor work crew representative. Assistance to answer personal protective equipment
needs can be obtained from the Personal Protective Equipment (PPE) Hazard Assessment
forms (Attachment 3) or from the facility Safety Department.
All personal protective equipment necessary to work safely will be checked or listed.
Where rescue by entry is not possible, a mechanical retrieval device will be required to
allow non-entry rescue. The end of the lifeline must be attached to a fix point outside the
permitted space or to the mechanical retrieval device.
H2S monitors are required to be worn by individuals entering areas that may contain H2S
as outlined in SHG #40.




D.
CHECK LIST

This section is to be completed by the Owning Department and the name of the person
verifying the actions indicated on the bottom of the blocked area that includes Section “B”,
“C”, “D”, and “E”.
Check all boxes that apply.
A joint job site visit, i.e., a collective visit to the work site by an Owning Department
representative and a work crew representative, is REQUIRED for hot work, hot tap/stopple
work, confined space entries, excavations, opening lines or equipment, and work requiring
the use of supplied breathing air. The person issuing the permit must determine, by the
nature of the hazards involved, the requirement of a joint job site visit in regards to work not
listed. The purpose of the collective visit is to identify and discuss the work to be done,
procedures to be used, and precautions to be taken. Information generated by this
discussion, in conjunction with the green copy of the permit, is used to prepare the safe
work permit.
Check JSA Completed box if a Formal (written) Job Safety Analysis is done.
The Owning Department representative will review the area for potential or actual hazards
and bring the green copy of the permit into the field for reference as part of the permit
preparation. The Maintenance representative will visit the site of the work to ensure that the
area has been made ready for work activities.
Even for repetitive or continuing type work, the Owning Department representative should
revisit the area each time the permit is reissued or renewed to assure that conditions have
not changed and that adequate housekeeping is being maintained, even during construction
activities. Some examples of repetitive type work are general clean-up or housekeeping
tasks, general preparatory work (i.e., scaffold building, creation of lay down areas, and
multiple day new construction, such as tank/unit construction, etc).





E.
PRECAUTIONS


This section is to be completed by the Owning Department.
The precautions indicated are to be taken by the Maintenance/Contractor before the work is
started.
All designated fire watches and/or designated attendants must be named on the permit and
wear red vests. Fire watches shall be required whenever cutting and welding is performed
in locations where a fire might develop, or where the following conditions may exist:


Appreciable flammable or combustible material is present in the general area closer
than 35 feet to the location of the cutting/burning;
Wall or floor openings are present within a 35-foot radius that do or may expose
flammable or combustible materials; or
Combustible/flammable materials are adjacent to the opposite side of metal
partitions, walls, piping, ceilings, or roofs and are likely to be ignited by conduction or
radiation.
In the event the designated attendant recognizes the need to evacuate the confined space,
an air horn (or any agreed to signal) will be sounded to notify the authorized entrants.

PERMIT VERIFICATION PERFORMED BY

The name of the unit Operator performing the field Check List tasks, Section “B”, and

unit Operator must be obtained ensuring these items have been completed, understood by
the Maintenance/Contractor employees and the area is safe for work to commence.

Maintenance/Contractor representative.
A new permit is required for each Owning Department/Maintenance work shift.

The Owning Department representative (person writing the permit) will sign this section
once all the permit conditions have been identified, verified and/or met.
F.
ATMOSPHERIC TESTING

This section is to be completed by the Operations Lead Process Tech and/or qualified
Owning Department representative.
The Operations Lead Process Tech or qualified Owning Department representative will
perform all necessary atmospheric testing for hot work. The results of the testing will be
recorded on the permit along with the time the test was taken and the serial number(s) and
type of the gas testing equipment used. The name of the individual performing the testing
will also be printed on the permit in the blank indicated "Tested By:".
Gas tests must be taken as part of a hot work permit issuance where the work will be done
in an area that is electrically classified as a Division 1 or 2 area, which includes the following
types of areas:



Within 30 feet of active open process sewers and/or separators;
Within 25 feet of active hydrocarbon conveying pipelines; or
Within any excavation.
Neither hot work permits nor gas tests are normally needed for hot work within plant shops.
Permits and/or gas testing may be needed when process equipment, vessels, drums, and
containers are moved into shop areas for work.

Hot work permits are needed, but gas tests are not normally needed, for hot work
performed within office buildings, labs or control rooms.
The Serial Number and Calibration Date of the gas testing device(s) used will be recorded
on the permit along with the brand or type of equipment.
During the preparation of the hot work permit, it should be determined whether periodic
follow up or continuous testing will be required due to the nature of the work. This decision
is based on whether the work activities themselves or conditions within the work area may
change the atmospheric quality during the work. Questions regarding uncertainties will be
referred to the Safety Department for resolution.
When the qualified Owning Department representative is performing the atmospheric
testing for HOT WORK, the following conditions must be met prior to issuing the permit:

Additional information on vapors or gases that may be present within a confined space may
also be available on Material Safety Data Sheets for materials that have occupied the
confined space.
Determine if these initial conditions could change over time due to the type of work to be
done, a change in confined space conditions, etc.
If the atmosphere quality could change over time, then the atmosphere within the space will
either be periodically or continuously monitored to assure that personnel in the area are not
endangered.
When testing for atmospheric hazards, oxygen will be tested first, then combustible gases
and vapors, and then toxic gases and vapors. (NOTE - The MSA Passport and similar
instruments test all of these parameters simultaneously.)
At a minimum, retesting of the atmosphere is required at the beginning of each
Maintenance work shift. If work does not begin within two hours of a gas test, the area or
space must again be retested.
When checking for combustible gases, the reading may not be accurate if there is
insufficient oxygen. There must be at least 10% oxygen in the atmosphere for the LEL
reading to be correct.
Gas testing in the presence of excessive steam will cause the gas-testing instrument to
provide inaccurate readings and may damage the meter.
If temperatures are in excess of 150o F, readings will not be accurate.
Check the meter to ensure that it responds properly prior to each use. If the meter does not
respond properly, tag it and return it to the designated location within the refinery for
recalibration and/or repairs.
Combustible liquids may be present which do not vaporize and register on the meter.
These hazards must be visually identified.

ADDITIONAL SIGNATURES

Various representatives must sign this section depending on the type of work to be
performed.
FOR CONFINED SPACE ENTRY

This section must be signed by the qualified Maintenance Tech/Contractor Representative
and the Owning Area Superintendent, Lead Process Tech or trained equivalent (i.e., the
designated Entry Supervisor). A representative of the Safety Department must sign ONLY
the initial Confined Space Permit (the permit issued to the space).

The Maintenance Lead Tech/Contractor Supervisor will sign the permit only after he/she
understands all of the conditions and restrictions included on the permit and has field
verified the blinding and/or separation.
For an employee or contractor to enter and work in a confined space under conditions
"Immediately Dangerous to Life and Health," the person must be equipped with a positive
pressure air line respirator with an emergency egress air supply and a safety harness
attached to a life line (if the harness for the respirator does not already provide a ring for a
life line attachment). All other necessary protective equipment must be worn.
FOR EXCAVATION








H.

Permits for excavations deeper than 4 feet that will be entered will be considered a confined
space and must be signed by a Valero or Contractor Competent Person after assuring that
the following precautions have been taken:
The excavation is properly shored or sloped and barricaded.
Proper access and egress is present.
Any water accumulation has been removed and any continuous source has been stopped or
controlled.
All adjacent structures, buildings, walls, etc. have been braced or underpinned to prevent
collapse.
Measures have been taken to remove materials such as rocks or soil from the edges of the
excavation that could pose a falling hazard; all excavated materials are at least 2 feet from
the edges of the excavation.
Appropriate gas tests have been taken.
Procedures are in place whereby the company or Contractor Competent Person will be
contacted to re-inspect the excavation prior to entry after a rainstorm or other hazard
increasing occurrence.
JOB/PERMIT COMPLETION
At the end of the work shift or at the completion of the work (whichever occurs first), a
Maintenance/Contractor representative must return the permit to a person in the control
room or point where the permit was issued - AND check whether the job is complete/
incomplete and whether the work area has been cleaned. Complete the signature, date and
time sections.
If the work was performed as a result of a MOCA and a PSSR is required, the Owning
Department Representative shall ensure the necessary tag(s) is placed and note this on the
permit.
If the permit was a Confined Space Entry Permit, the Owning Department representative
should promptly sign the bottom line of the permit indicating that the permit is canceled.

APPENDIX 2 - CONFINED SPACE PERMIT
(Permits Issued to the Space or Vessel)
GENERAL INSTRUCTIONS ON COMPLETING PERMIT
Note: The following sections refer to the corresponding letter on the permit form.
A.
WORK SCOPE

When filling out the Permit, identify the specific location of the confined space (i.e., refinery
area, the specific equipment to be worked on, and describe the work to be done. A
complete understanding of this information is important to the permit writer since it will often
influence the type of gas testing to be done as well as the decision regarding whether
periodic retest or continuous monitoring of the confined space atmosphere is needed.
Unless special provisions are made, a new permit is required at the beginning of each new
work shift for the Owning Department. This should be reflected in the VALID UNTIL time
frame box provided.

B.
CHECKLIST

The considerations necessary for proper preparation of a confined space for entry are listed
under three headings: vessel draining/purging, vessel isolation, and potential hazards.
Each item under these heading should be considered as to whether this action is necessary
for preparation of the space. A check mark or "x" should appear in one of the three boxes
next to each consideration.
If a consideration does not apply to a particular confined space, the box marked "N/A" (Not
Applicable) should be marked.
If a confined space permit is being written for an excavated hole, most, if not all of the items
under Vessel Draining/Purging and Vessel Isolation will be marked as N/A except for
Access/Egress To/From Space Okay.
IT IS IMPERATIVE THAT FREE ACCESS TO and EGRESS FROM CONFINED SPACES
BE MAINTAINED AT ALL TIMES WHEN ENTRY IS UNDERWAY.
Any potential hazards that may still be present in the space after preparation should be
identified as a reference when preparing the confined space entry permit for a particular
craft or Maintenance group.
BEFORE opening a vessel, owning area and Maintenance personnel should also consider
whether pyrophoric iron sulfide or other deposits may be present. If pyrophoric materials
may be present, personnel should follow the refinery pyrophoric hazard safety instructions
as part of preparing the vessel for entry.
The Entry Supervisor and Qualified Owning Area Representative must field evaluate for the
initial confined space entry and review the Confined Space Permit. The entry supervisor
may delegate to a qualified representative, the field evaluation for non-hazardous confined
spaces and renewed confined space entries. The field evaluation and permit review help
assure that all appropriate precautions and restrictions are considered such as:
All lines entering the confined space are disconnected or blinded (any exception to this must
be approved by the affected Department Manager).
The vessel must be cleaned as thoroughly as possible by draining, water washing,
steaming, forced ventilation or other suitable means before any employee enters the
confined space for additional cleaning or other work.
Irritating fumes, dust and mist, if present or may be generated, will warrant either additional
cleaning, local exhaust ventilation or personal protective equipment.











C.
PRECAUTIONS

When opening confined spaces for entry, danger signs will be posted at the entry portal
indicating " Enter by Permit Only" (or equivalent wording). These signs must be posted as
soon as the confined space is opened and will be removed when closed.
When entrance covers/manways are removed, the opening will be promptly guarded by a
railing, temporary cover, or temporary barrier that will prevent an accidental fall through the
opening and that will protect employees in the confined space from foreign objects entering
the space.
Equipment that is always open to the atmosphere such as tower skirts, tanks with open top
floating roofs, etc., and is identified as a confined space will be identified with the wording
"Enter by Permit Only."
During confined space entry, communications between designated attendants and
authorized entrants must be maintained throughout the entry. Methods of communication
that may be used include visual, air horns, voice, radio, rapping or tapping codes, signaling
tugs on a rope, etc., all of which may be used by the attendant to ascertain that work
activities are continuing normally.
In the event the designated attendant recognizes the need to evacuate the confined space,
an air horn (or any agreed to signal) will be sounded to notify the authorized entrants.
At least one trained designated attendant will remain outside the confined space and be in
constant communication with the authorized entrant inside the confined space and have a
means to summon rescue personnel and/or the Entry Supervisor. If the designated
attendant must leave for any reason, he/she must be properly relieved or the entrant(s)
must exit the confined space. The individual relieving the designated attendant must record
their name on the field copy of the Confined Space Permit.
Forced ventilation (e.g., air horn, air mover, or blower) is recommended for all (except inert
or IDLH) confined space entry, and when used, MUST BE GROUNDED.
Plant air valves used for ventilation equipment must be identified and the attendant must be
aware of its location.
Any mixer or other such device inside a confined space must be mechanically or electrically
disconnected. (See Lock, Tag, & Try, SHG #11)
Any radiation sources affixed to the space must have their shutters closed and locked.
Tanks/vessels that have been in leaded gasoline service must be considered a lead hazard
and all precautions taken until cleaned and a lead in air analysis completed.
As part of the initial entry permit for desalters, crude oil and cat feed tanks, a radiation
exposure measurement should be taken to assure significant levels of NORM (Naturally
Occurring Radioactive Material) are not present. These measurements can be arranged by
contacting the Environmental or Safety staff.
Artificial lighting should consist of equipment suitable for use in a confined space, (e.g.,
sealed flashlights, low voltage lighting with bulb protectors, ground fault circuit interrupters,
etc.).
Prior to IDLH entry, a Safety Department representative or Rescue Team member must be
notified.














D.
ATMOSPHERIC TESTING


Gas testing devices will be bump-tested prior to each shift and results logged.
The Owning Area Supervisor or qualified Owning Department representative will perform all
necessary atmospheric testing for confined space entry work. The results of the testing will
be recorded on the permit along with the time the test was taken and the serial number(s)
and type of the gas testing equipment used. The individual performing the testing will also
sign the permit in the blank indicated "Tested By:"
The Serial Number and Calibration Date of the gas testing device(s) used will be recorded
on the permit along with the brand or model number of the device(s).
Initial testing performed in preparation of the Confined Space Permit must be reviewed with
a representative of the Safety Department who will also sign the permit.

In NO case will a confined space be permitted for entry with >10% LEL.

before entry is authorized.
Determine if these initial conditions could change over time due to the type of work to be
done, a change in confined space conditions, etc.
When testing for atmospheric hazards, oxygen will be tested first, then combustible gases
and vapors, and then toxic gases and vapors. (NOTE - The MSA Passport and similar
instruments test all of these parameters simultaneously.)

Air movers will be shutdown so that natural draft only is present for the INITIAL test of a
confined space. Subsequent tests (including subsequent re-issuance of Confined Space
Permits) can be made with the air mover remaining on.
E.
QUALIFIED OWNING DEPARTMENT REPRESENTATIVE

This individual has been trained and certified through the refinery permitting training
program to issue permits and perform gas testing.
F.
ENTRY SUPERVISOR

This individual is the qualified Valero representative, the Unit Superintendent or Lead
Process Tech.
G.
SAFETY REVIEW

Before initial entry the Safety representative will review the permit for completeness, to
assure that all appropriate gas tests have been taken and documented, and to review the
personal protective equipment specified to help assure that it is appropriate for the tasks
indicated and the expected conditions within the space.
All confined space permits must be canceled. If the signature line is not signed by the
qualified Owning Department representative or the Unit/Area Supervisor, then the permit is
canceled as of the time indicated in the VALID UNTIL blank at the top of the page.
Once the permit has been completed or canceled, it should be returned to the refinery
Safety Department for retention and subsequent review.


