At the beginning of each sport season, the principal/designee of each member school shall prepare an eligibility certificate listing the
students eligible under OHSAA bylaws to represent the school in each sport. Eligibility certificates shall be prepared for each sport in
which the school participates and filed in the office of the principal/designee and available for inspection upon request.
Corrections, additions and deletions may be made as the sport season progresses and, must be filed in the office of the
principle/designee.
Students must be eligible in accordance with all sections of Bylaw 4-Student Eligibility.
Column three is the date the student first enrolled in your school since leaving the eighth grade or another school. Place an asterisk *
next to the names of students who do not attend your school (i.e., home educated, non-public school students, etc.)
High School-List students alphabetically by number of semesters in high school since entering Grade 9 for the first time
(1-2, 3-4, 5-6, 7-8).
Place students who are on home-education, those who attend non-public schools that do not sponsor the sport, and those who
attend Community or STEM schools into the correct semester category. Use the date of the student’s first participation in this sport
as the “date first entered your school” and denote those students with a ##..
