REGISTRATION AND ENROLLMENT FOR SECONDARY SCHOOLS QUICK REFERENCE GUIDE March, 2015 Registration 1. Residency Requirements A student can have only one residence. The student must live in one of the following: 1) The residence of the student’s parent/guardian who resides within the boundaries of the school district 2) A licensed foster or group home 3) The home of a caregiver adult that is located within the boundaries of the school 4) A state hospital located within the boundaries of the school Students whose custody is shared by both parents may attend school only in the residential area of one parent. Dual enrollment due to custody issues is strictly prohibited. 2. Residency Verification Verify address with utility bills, property tax documents, rental or lease agreements, official government mail (Cal Works, Social Security). Other documents may be considered as proof of residence. However, many of these documents may not be as reliable as those listed above and therefore, may require additional verification. Examples of these documents include, but are not limited to: Escrow papers, Current Bank Statements (no personal checks accepted), Moving Company Receipts, and Verification of Residency Service Letter from a utility company. Any documentation offered as proof of residence must show the name of the parent/legal guardian, a current date, and the residence address. LAUSD does not accept telephone bills or driver’s licenses as proof of residence. In the absence of specific documentation, a prospective parent may provide a written affidavit to verify his or her residence. Enrollment 1. Establish Student Identity Establish student identity with a birth certificate, baptismal certificate, Passport or court order. If the parent is unable to provide one of these documents, the parent must complete Affidavit for Proof of Age of Minor. 2. Obtain Proof of Immunizations o TB Skin Test is no longer required for kindergarten or first grade. In addition, any student entering in any other grade level who has not attended a California school will not be subject to a mandate for TB skin testing. (See MEM-5718, Updating Policy Regarding Tuberculosis (TB) Testing for School Entry, dated February 29, 2012) o Provide the parent or guardian with the Person Beliefs Exemption to Required Immunizations, Attachment C from Bul-1660.7, for students who are not immunized due to person or religious beliefs. Follow instructions on the form. o Copy yellow immunization card 3. Request, accept, and review the transfer form for student entering from another LAUSD school. If a transfer form is not available, school staff is to call the previous school to notify them. The school shall immediately issue a transfer form and fax it to the new school within one business day. NOTE: Under no circumstances shall parents/guardians be required to return to the previous school of attendance for a PAR. 4. Establish parent/Legal Guardian Identity Identify and verify the relationships of the adult to the student with: Birth Certificate and State Identification (driver’s license, DMV Identification Card or Passport) a copy shall be filed in the student’s cumulative record, or be stapled to the completed enrollment packet. A baptismal certificate and a state identification A court order establishing the parent/guardian relationship If parent/legal guardian does not have any legal identification, they are required to complete Affidavit of Parent/Legal Guardian Identification If the student is not residing with the parent/guardian and guardianship has not been legally established the Caregiver’s Authorization Guidelines and Affidavit shall be given to the caregiver for completion. The Caregiver’s Affidavit is sufficient proof necessary to determine that a student lives in the caregiver’s home, unless there is a suspicion that the student is residing elsewhere. 5. Determine grade placement Review the transfer, report card, or transcript Contact the last school of enrollment Use Chronological Age Calculator 6. Provide parents/legal guardians with the following registration forms to be completed: District Enrollment Form (9661214321) Two Student Emergency Information Form (rev. June 16, 2014) with at least two work telephone numbers Temporary Health Card (Form 34-EH-27), if student is new to the LAUSD. Immunization History Meal Application Student Residence Questionnaire (BUL 1570.2, Enrollment of Homeless) Ethnicity/Race Identification Form (REF-4718.2 Ethnicity/Race Identification for Students) Migrant Education Program for Secondary (MEM-6351.0, dated August 15, 2014) American Indian-Alaskan Native Letter Questionnaire (MEM-5282.5, dated August 1, 2014) Financial Responsibility for Damaged School Property Letter 7. Also provide parents with the following: Parent/Student Handbook California School Nurses Organization health update about Pertussis Vaccine Letter Master Academic School Calendar District and school attendance policy and procedures related to absences, tardiness, and truancy. School rules, behavior standards, policies, school map including location of restrooms, bell schedules, pedestrian routes, etc. CHAMP Program Brochure Family Educational Rights and Privacy Act (FERPA) REMINDER: When a student enrolls late in the semester, the school administration must ensure that the parent/legal guardian and student receive all of the information listed above. 8. Registration & Enrollment: How to complete the paper process! Information must be entered or transferred over into MISIS. 1. If the student is new to LAUSD then all data must be entered new. 2. If a student is coming from another LAUSD school, transfer student information within MISIS and update information (address, phone number, etc.) using the Student Search option in the Enroll Student tab under the Admin Menu in MISIS. IMPORTANT NOTE REGARDING IMMUNIZATION BEGINNING JULY 1, 2012 (MEM-5718 Updated Policy Regarding Tuberculosis (TB) Testing for School Entry, dated February 29, 2012– Student Health and Human Services, Deputy Superintendent of Instruction “Effective July 1, 2012, schools shall not require a TB skin test for kindergarten or first grade entry. In addition, any student entering in any other grade level who has not attended a California school will not be subject to a mandate for TB skin testing. Instead, entering students will be screened for risk of disease as part of their regular well child care and school entry exam. Provides of well child and school entry exams will target their testing to those that are at increased risk for TB. Screening procedures and guidance on this topic is available to providers through the Child Health and Disability Prevention Program. Los Angeles County Department of Public Health, and other national organizations.
