all fees that have been voted on and approved by Michigan Tech students, currently [enter current sum of all

As a supported student, [percentage] percent of the annual cost of your University-approved health insurance or
[enter current dollar amount of University-funded portion of health insurance cost], will be covered by Michigan
Tech. The remaining [percentage] percent will be your responsibility. All supported graduate students must
either purchase the University-approved health insurance or provide proof of comparable coverage. All
University graduate students receive access to the Michigan Tech Student Development Complex (SDC) which
maintains state-of-the-art sport facilities. This membership is currently valued at [enter current annual SDC fee]
per year. Graduate students also receive discounted or free access to facilities and events including but not
limited to Mont Ripley Ski Hill, Portage Lake Golf Course, Gates Tennis Center, visual and performing arts
events, University hockey games, and intramural sports.
Tuition and stipend levels for each academic year, beginning with the fall semester, are determined by the University’s Board of
Control each July. Both are typically increased by a modest amount each year.

GPA of 3.0 or better, take required exams on schedule, make satisfactory progress in your research (if
applicable), and perform all the duties for which you are receiving support.
During the time period covered by this offer, the [enter department or school name] may recommend that
support be terminated in cases in which dereliction of duty, endangerment of students or facilities, or failure to
make satisfactory academic progress can be documented. The dean of the Graduate School must review and
approve all recommendations for mid-contract termination of support before they occur.
