HILBERT HIGH SCHOOL SCHOLARSHIP COMMITTEE PROCEDURE (Do not include this page with your completed application) 1. Students apply for all scholarships by completing one scholarship application form. (see attached) 2. Students must attend a post-secondary institution within the next academic school year (either beginning at the fall or spring semester) or will forfeit any scholarships awarded. 3. Teacher committee reviews the application and makes the selection of the scholarship winners. 4. Scholarship winners are announced at the Academic Awards banquet. 5. Scholarships are awarded on the basis of subjective judgment of the teacher committee, meeting specific criteria established by the donor. 6. CRITERIA FOR CLAIMING YOUR SCHOLARSHIP: Depending on your enrollment status (see #2 above), scholarships are awarded to students as follows: A. If you enroll for the fall semester: Upon the successful completion (good academic standing) of the fall semester attended as a full-time student. Check your individual college or technical college to determine full-time student status. (UW-Colleges require 12 credits to be considered full-time). Re-enrollment for the spring semester as a full time student with good academic standing is also required. Proof of spring semester registration and a copy of fall semester report card or transcript are required. B. If you enroll for the spring semester: Upon the successful completion (good academic standing) of the (spring) semester attended as a full-time student. Re-enrollment for the fall semester of your 2nd yr. of schooling as a full-time student with good academic standing is also required. All information given on this application must be true both at the time of application and at the time of the award. 2. In order to receive an award the student must be a full time student and successfully complete their first semester at the college or tech. Also, the student must be enrolled in the next semester as a full- time student. 3. The scholarship committee reserves the right to withhold the scholarships if it is in the best interest of the scholarship program to do so. 4.
