GSIS SCHOLARSHIP PROGRAM TERMS AND CONDITIONS 1. SUBMISSION OF NOMINATION FORM & OTHER REQUIREMENTS The member-grantee (winner) shall be given 30 days from the first day of publication of winners in newspapers of general circulation within which to submit the Nomination Form and other requirements namely: Certified True Copy of Birth Certificate of Scholar-Designee Scholar-Designee Information Sheet with 2 pieces of latest 2 x 2 I.D. pictures Enrollment Assessment Form (if already available) The nomination forms shall be made available at the different GSIS Regional and Branch Offices or may be downloaded from the website. Failure to submit the Nomination Form within the given time shall render the grant as forfeited. 2. SUBSTITUTION OF NOMINEES Substitution of scholar-designee will be allowed once and within the 30-day period given for submission of requirements only. 3. SCHOLARSHIP TERMS & CONDITIONS 3.1. ACADEMIC LOAD 3.1.1. The scholar-designee shall enroll in the semester immediately following the award of the scholarship, and shall carry the full load of academic subjects for each semester as prescribed by his/her program of study. No deferment of enrollment shall be allowed. 3.1.2. The scholar-designee may carry an overload of one subject with the approval of the authorized school official. 3.1.3. The scholar-designee may enroll in summer class, provided that the actual tuition and miscellaneous fees shall be at the expense of the scholar-designee. 3.1.4. The scholar-designee shall be required to enroll and finish NSTP/CWTS and PE subjects within the duration of the course. 7.2 SHIFTING OF COURSE 7.2.1 No scholar-designee shall be allowed to shift to another course except for meritorious reasons, and only upon approval of the GSIS and the school’s officials, e.g. Registrar or College Dean. 7.2.2 In no case shall the scholar-designee be allowed to shift course more than once and beyond the first semester of his/her third year in the university. 7.2.3 Shifting must enable him to finish the course within the duration specified in his/her scholarship grant, unless earlier terminated due to his failure to comply with its terms and conditions 7.3 TRANSFER OF SCHOOL 7.3.1 The scholar-designee who wants to transfer to another school must submit a written request addressed to the Manager, Training and Scholarship Department, GSIS, Financial Center, Pasay City, for proper recommendation to the GSIS Management. 7.3.2 Approval of request shall be subject to any of the following conditions: The school shall submit a certification that for reasons beyond its control, it can no longer offer the course where the scholar-designee is enrolled under the scholarship. The school is located in a troubled area. 7.4 DROPPING OF SUBJECTS 7.4.1 The scholar-designee must not drop any subject except for a valid cause. 7.4.2 The scholar-designee may drop only one subject in one semester, regardless of the number of units. The same subject, however, may not be dropped twice. 7.4.3 Dropping without approval of the GSIS is a valid ground for termination of the scholarship grant. 7.5 SCHOLASTIC REQUIREMENT 7.5.1 The scholar-designee must maintain a weighted average of 85% with no failing grade in any subject. In case of indefinable Quality Point Index (QPI) grades, the GSIS shall require the submission of additional documentary requirements from the School/University Registrar to show proof of grantee’s compliance with above-mentioned grade requirements. 7.5.2 The scholar-designee must not incur a grade of “Incomplete”` in any subject per semester, except if there is documentary proof coming from the School/University Registrar showing the grade was not of scholar-designee’s failure to comply with requirements but due to the school’s prescribed completion of the subject at a succeeding semester. 7.5.3 The scholar-designee must not commit any misconduct or violation of school regulations during his/her stay in the university. 7.6 DOUBLE GRANTS The scholar-designee shall not have any other scholarship simultaneous with the GSIS scholarship grant. Acceptance of other scholarships automatically terminates the scholarship grant. 7.7 SUPERVISION The scholar-designee shall be under the supervision and guidance of the school where he/she is enrolled and the Scholarship Staff of the Training and Scholarship Department, GSIS. 7.8 DURATION OF SCHOLARHSIP The scholarship shall be in force for the duration of the 4- or 5-year course, unless earlier terminated due to failure of the scholar-designee to comply with the GSIS and the school’s scholarship terms and conditions. 2 7.9 SCHOLARSHIP BENEFITS 7.9.1 Actual amount of tuition and miscellaneous fees up to the maximum amount of P20,000.00 per semester only; 7.9.2 A monthly stipend of P2,000.00; 7.9.3 All payments of the benefits shall be coursed through the school where the scholar is enrolled upon receipt of report of grades and billing statements. Any amount in excess of the P 20,000.00 limit for tuition and miscellaneous fees shall be for the account of the scholar–designee. In case of reimbursement, wherein payment of tuition fees has already been made, the scholar-designee shall be required to submit a certified true copy of registration form, billing statement, and official receipt as proof of payment in order to effect reimbursement of expenses up to a maximum of P20,000.00 only. 7.9.4 The scholar must submit within sixty (60) days, after each semester, the certified true copy of all his grades for the just concluded semester and the school billing statement and official receipts of the current semester to the GSIS Training and Scholarship Department for evaluation and processing. Failure to comply with the given 60-day requirement will automatically forfeit his claim for that billing period. 4. SIGNING OF SCHOLARSHIP CONTRACT Upon submission of complete documentary requirements and formal awarding of the scholarship grant, a Scholarship Contract will be executed between the GSIS and the member-grantee, scholar-designee, and parent/guardian of the scholar. If the signatories are residents of different cities or provinces, they will have to acknowledge their signatures separately before different notaries public. 5. LETTER OF GUARANTY WITH PARTNER-SCHOOLS To signify its willingness to enter into an agreement with the GSIS under the terms and conditions of the scholarship program, a Letter of Guaranty (LOG) shall be undertaken by the partner-school and the GSIS. The LOG states the scholarship benefits which the GSIS agrees to pay the partner-school and the scholar per semester for the duration of the scholar’s chosen course, upon receipt of the scholar’s grades and billing statement, and official receipt. 6. MISCELLANEOUS PROVISIONS 6.1. The scholar–designee shall enroll in any school of his/her choice. 6.2. The scholarship slot awarded to a scholar–designee is non–transferable. 6.3. In case of willful abandonment of the scholarship, the scholar-designee should reimburse to the System the total amount spent by the System in connection with his/her scholarship grant. 3
