Welcome to Jessop Medical Practice
Jessop Medical Practice is named after William Jessop, a famous
local engineer and founder of the Butterley Company whose
memorial is a well known local landmark.
Our aim is to provide friendly, personal and effective health care.
We endeavour to treat all our patients fairly to meet their need in
an appropriate way and as promptly as possible.
Our staff should treat everyone courteously and we would ask that patients help us by being
polite when they contact us at the surgery.
This new leaflet informs you about us and the services that we offer. You can also order
prescriptions online, cancel appointments, and find out more information about health issues
through the information leaflets and links.
Please let us know if you have any comments about this website, or suggestions for
improvements.
Our Senior Partner, Dr Meakin, left at the end of February 2007 after 30 years at the practice.
We wish him well in his new post as Medical Director of Derbyshire County PCT.
The Senior Partner is now Dr Veale.

Appointments
We offer routine appointments which can be booked for both
morning and afternoon surgeries. During the day we make
provision for patients who need to be seen urgently that day but
you must contact the surgery as early as possible that morning, so
that we can arrange to see you. Our consultations are by
appointment only. You may attend whichever surgery you choose
for an appointment.
Please note that individual doctors have a variable working
pattern, so that each doctor works some sessions each week at both surgeries. This is so we
can offer our patients continuity of care. Several of the doctors work part time. For these
reasons, and because of training needs and holidays, you may find you need to vary the
surgery you attend to see the doctor of your choice at the earliest date. Early or late
appointments cannot be guaranteed.
Opening Times (except Wednesday at Riddings where we close at 4:00pm)
Routine Appointments
A routine appointment is 10 minutes long. Please remember a separate appointment is needed
for each person. If you have several problems to discuss please book a longer (20 minute)
appointment. We do our best to run to time but emergencies or complex cases may cause us to
run late. Our receptionists will try and let you know when your doctor is running late.
Urgent Appointments
For patients with an urgent medical need, please contact the surgery as soon as you can,
ideally, during the morning. Usually, a nurse practitioner will telephone back to find out more
about the problem and to work out the best way to help you. She may give advice on the
telephone, book you in to see the duty doctor or offer an appointment that day when she can
see you herself. You may be asked to visit Riddings surgery to see the duty doctor or nurse
practitioner if your problem is of an urgent nature (except for Wednesday afternoons when the
duty doctor is at Ripley surgery in the evenings).

Prescriptions
Repeat Prescription
Repeat prescriptions need to be requested in writing, and the easiest way to do this is by
using the online prescription request form below. Alternatively you can use the tear-off
slip attached to your previous prescription. Tick each item you need and hand this in at
the surgery or post it to us. If this is lost, simply write down the items you require.
Please allow us two working days before coming to collect a routine repeat prescription from us.
We ask for the request in writing to minimise the possibility of a mistake occurring.
Some local chemists offer a collection service for prescriptions when they collect from the
surgery and make up your prescription ready to collect. Please ask at reception or your local
chemist if you think this may help. If you let us have a stamped, addressed envelope we will
post your prescription on to you but please allow extra time for this.
Please allow 48 hours before collection. If you require items sooner than this please contact the
practice in person.
Staff Details
Staff Page
RONG>Our Practice Team
All members of the team work closely with the doctors and all staff are bound by strict rules of
patient confidentiality.
Practice Managers
Mrs Glynis Blyth is responsible for the day to day running of the Ripley Surgery and Mrs Julie
Maleham has responsibility for the Riddings Surgery. They are available to help with
administrative or non-medical aspects of the Practice.
Administrative Staff
We have a team of receptionists, secretaries and other clerical staff who work behind the
scenes to help support the medical staff in the smooth running of the Practice. One of them may
contact you on behalf of the Practice if necessary or asked to do so by one of the doctors.
Nursing Team
We have three highly trained nurse practitioners who work alongside the doctors. Jan Gregory,
who leads the team, works in the area of Chronic Disease Management, helping to look after
patients with long-term illnesses such as Asthma, Diabetes and Heart Disease. The other two
nurse practitioners, Helen Altimas and Kath Conaghan, are responsible on a day to day basis
for contacting patients who feel they need medical help on the same day and then sorting out
the best way to help. Other members of the nursing team help with this work and in addition see
patients by appointment, offering a range of services including vaccinations, ear syringing,
wound care and cervical smears. The doctors may sometimes ask them to help by checking
blood pressure, giving diet advice or monitoring weight.
District Nurses
Our District Nurse team, who are employed by Derbyshire County Primary Care Trust, is led by
Anna Long. They are able to visit the housebound and people with palliative care needs to offer
a range of nursing care when this is needed.
Pharmacist
The Practice employs a part-time pharmacist, Kim Pearsall, who helps us with our repeat
prescribing and can advise on issues about medication.
Midwifery
Midwives are allocated to the Practice by Derby Hospitals NHS Foundation Trust. They arrange
booking to the hospital of choice for delivery, carry out the majority of antenatal care at the
surgeries and visit patients at home after their baby has been born. Patients are invited to bring
their baby to the surgery when they are 10 days old for a baby check.

Patient Registration
Pre-registration
MB ChB Glasgow 1994 DCH MRCGP
If you wish to pre-register click on the link below to open the form. When you have completed all
of the details, click on the 'Send' button to mail your form to us. When you visit the surgery for
the first time you will be asked to sign the form to confirm that the details are correct.
Pre-registration Form - form automatically adjusts to practice country
When you register you will also be asked to fill out a medical questionnaire. This is because it
can take a considerable time for us to receive your medical records. There is an online version
of this file too, which you may fill out and send to us. When you come to the surgery you will be
asked to sign this form to confirm that the details are correct.
Online medical questionnaire for new patients
Note that by sending the form you will be transmitting information about your self across the
Internet and although every effort is made to keep this information secure, no guarantee can be
offered in this respect.
Change of Address
Please let us know your new address and telephone number as soon as possible. If you move
outside our Practice area you will be requested to register with a more local doctor.
You may use the online form to notify us of a change of address. The form will open in a new
window. Fill out the details and press the Send button.
Change of Address Form
Out of Hours
You should telephone the usual surgery number but your call will be redirected to a trained
operator at Derby Medical Services.
Derbyshire County Primary Care Trust is responsible for providing the out of hours service. You
may be asked to visit a local treatment centre or given telephone advice.
Home Visits
If you are too ill to come to the surgery, please try and contact us between 8.00 and 10.00 in the
morning . Please be prepared to give brief details to our receptionist so that the duty doctor can
assess your request.
The duty doctor may call you back in the first instance. Please only ask for a home visit if really
necessary.
Waiting Area
We do not have any toys in the waiting areas due to the risk of spread of infection, and being a
danger to other patients if left on the floor. Patients may wish to bring a favourite book or toy for
their child.
Telephone Advice
Sometimes a home visit or an appointment with the doctor may not be necessary, but we can
help by talking to you on the ‘phone.
If you specifically would like this kind of help, please ask our receptionist when you call and she
will arrange for a doctor or nurse practitioner to call you back later.
Test Results
We try our best to contact you should results require any further action. However, it is your
responsibility to check for your results.
Please call between 2.00 and 4.00 in the afternoon if you can. You will find it easier to get
through to our reception staff and they will have more time to help.
To maintain confidentiality, you should call the surgery personally. If you are asked to bring a
sample to the surgery, please let us have it by 1.00 p m.
Disabled Access
Both our surgeries have ground-floor access for the disabled and staff will be happy to give
assistance if needed. Please say when you book an appointment if you need to be seen
downstairs and our receptionists will make a note of this.
Please let us know if you have hearing difficulties and require more help because of this. There
is a loop system on the reception desk for the benefit of hearing aid users and an aid for hearing
can be provided for use when being seen by a doctor or nurse.
Suggestions/Complaints
If you have any suggestions or comments to improve our service, please let us know. You can
comment by writing your suggestion down and putting it in the box in the foyer which is also
used for repeat prescription requests. If you put your name on the suggestion, we can get back
to you.
We try to provide the best care we can, but occasionally misunderstandings or mistakes occur.
If there is a problem, we have an in-house complaints procedure to deal with this. Please ask at
Reception or contact one of our Practice Managers.
Derbyshire County Primary Care Trust can offer advice or help with problems in any area of the
NHS via their Patient Liaison Service (PALS) - telephone 01773 525028
Confidentiality
Sometimes audit or research work is carried out for the benefit of the wider health community.
Much of this is initiated by the Department of Health for them to collate information and carry out
surveys, and is therefore not controlled by the Practice.
If you have any concerns regarding this, you should contact the Patient Liaison Service (PALS)
on 01773 525028. We never give information to commercial companies without your consent.
Practice Area
Our Practice area is based on the area around Riddings and the town of Ripley where we have
our two surgeries. We cover the locality from Swanwick and Somercotes in the north to Denby
in the south, from Jacksdale and Ironville in the east to Heage in the west.
We are unable to accept or keep patients on our list if they are living outside our practice area.
Please ask to see a detailed map if you are unsure and please tell us if you move outside the
area so we can give you help by letting you know what local doctors are available to you. If we
find that you are outside the area and have not told us, we will ask Patient Services in Derby to
remove you from our list.
