Doctors are consulted by appointment. Surgeries are throughout
the day and during the usual opening hours.

telephone number and your call will be directed to the out of hour’s

Welcome to Church Walk Surgery
Thank you for registering with our practice. The partners and staff of this long established
practice are committed to providing the highest level of patient care. This booklet contains
information about the facilities and services we provide & we hope you will find it useful &
keep it handy for reference. We are a training practice and have GP registrars with us for 4
month to 12 month periods. The registrar is a qualified doctor and can prescribe and refer for
hospital appointments. The registrar will have surgeries on most days.
We are a group practice and patients may see any of the doctors in the partnership.
However, please be aware that you may have to wait a little longer to see the GP of your
preference. We are an undergraduate training practice and regularly have student doctor’s
sitting in with our GP’s for consultations.

EMERGENCIES
For Life-threatening emergencies such as severe crushing chest pain, collapse or
unconsciousness or severe bleeding telephone 999 for an ambulance IMMEDIATELY
If you feel that you need to be seen urgently on the day by a Dr, please tell the receptionist
the nature of your request to assess the urgency.
HOME VISITS
These should be reserved for patients who are genuinely too ill to attend surgery. Please
telephone before 10.00am and be prepared to answer a few questions to help the doctor’s
plan their calls and to give the doctors an idea of urgency. Please remember the doctor can
see at least four patients in surgery in the time taken to do one visit.
ACCESS FOR THE DISABLED
The surgery has a ramp for wheelchair access. A lift is available for use by disabled patients
for access to some clinical rooms. The patient toilet is adapted for use by disabled patients.
A wheelchair is available for use in the surgery if required, please ask a member of staff.
NEW PATIENTS
It is the policy of this practice that you are given a health check by the practice nurse, shortly
after you register to discuss any general health issues. Please make an appointment for this
service when you request to register. The new patient registration forms can be printed off our
website. www.churchwalksurgery.nhs.uk Complete and sign these and bring them into the
surgery to register with the surgery. We will require sight of your ID, so please also bring
photographic and address ID for the surgery to copy for your medical records.
OUT OF AREA REGISTRATIONS
For patients who chose to register with our practice as a new patient but DO NOT live in the
catchment area that we cover, we will offer the patient to be registered as an ‘out of area’
registered patient WITHOUT HOME VISITS. This will mean that we are aware that you live
outside the practice area (catchment area) and when you registered at the practice it was
made aware to you that we are not required to provide you with a home visit.
You may on occasion, develop an urgent illness or injury at home that means attending the
GP surgery as normal would not be appropriate. If you require a GP, please contact the
practice in the first instance. If we determine you need access to services local to where you
live we may ask you to call NHS 111.
In these circumstances NHS 111 will direct you to the local service that has been established
by NHS England for patients such as you. This local service could be a GP practice near to
where you live, the local walk-in or urgent care centre, A&E or minor injuries unit.
This local service will then decide if you can attend for an urgent face to face appointment
with a healthcare professional or if a home visit is needed which will be based on your
individual circumstances.
You should be aware that we will provide (if requested) the GP or healthcare professional with
basic patient details in order that they can carry out their consultation safely and effectively.
By registering as an ‘out of area’ registered patient you are giving your consent to this
information being provided.

If this is in the out-of-hours period when GP surgeries are normally closed – between 6:30pm
and 8:00am weekdays and during weekends – NHS 111 will direct you to the local out-ofhours provider.

PATIENTS MOVING TO AN AREA OUTSIDE OF OUR CATCHMENT AREA
It is the practice policy that if patients that are already registered at the practice move from
living within the practice area to outside the practice area they will be de-registered from our
practice list on the grounds that the patient has moved out of our area. We will however,
invite the patient to re-register under the new arrangements as detailed above for OUT OF
AREA REGISTRATIONS WITHOUT HOME VISITS. It will be the patient’s choice whether
they wish to re-register with the practice again without the home visit service or to register at a
new GP surgery local to where they are now living to receive full GP medical services which
would include home visits.
REPEAT PRESCRIPTIONS
These are produced by computer and we require 48 hours notice for your prescription to be
processed, with the exception of weekends and bank holidays where extra time must be
allowed. You will receive a ‘tear off’ slip with each prescription which lists the medicines you
receive by repeat prescription. PLEASE CHECK THE ITEMS CAREFULLY TO ENSURE
THEY ARE CORRECT. When you require repeat medication you should leave the request in
the prescription box in the reception area. Please enclose a stamp addressed envelope if you
wish it to be posted back to you. We also accept FAX requests for prescriptions. FAX No –

COMMENTS, PROBLEMS AND SUGGESTIONS
Sarah Petter is our Practice Manager and she may be able to help you with any
administrative or non-medical aspects of your health & treatment. She is also available to
discuss any comments, suggestions or complaints. Our aim is to give you the highest
possible standard of service and we try to deal swiftly with any problems what may occur.
Please do not hesitate to contact the practice manager for further details.
RECEPTION STAFF
The receptionists are here to help you. Please try to be considerate and patient because at
certain times they are extremely busy but will always do their best to assist. Try not to feel
offended if they ask you a few questions to help assess the urgency. They will be able to help
you with many routine enquiries and may ask you to see the practice nurse for other advice.
MEDICAL SECRETARIES
We have a team of four Medical Secretaries who provide a full and comprehensive secretarial
service. They deal with all referrals sent from the practice on to secondary care, liaising with
hospitals and consultants on behalf of GP’s and patients when necessary. They are also the
point of contact within the surgery for any issues relating to the “Choose & Book” referral
system.

TRIAGE NURSE PRACTITIONER
Our Advanced Nurse Practitioner has clinics every Tuesday, Wednesday and Thursday
morning & Thursday afternoon. It is an ‘Emergency on the day clinic’. Please contact the
surgery on the day for an appointment. Appointments are not usually taken for advance
bookings without prior agreement by the Nurse Practitioner.
HEALTH CARE ASSISTANT
 Phlebotomy clinic is held every morning to take bloods.
 Health promotion matters such as blood pressure & diet
 Smoking cessation clinic

 Health checks, including well person and over 75 health checks

DIABETIC PATIENTS
We have a special service for diabetics, held regularly by our Practice Nurses.
FAMILY PLANNING & CONTRACEPTIVE ADVICE
For all family planning and all contraceptive advice, please book an appointment with the
practice nurse.
Coils and contraceptive implants are done in a special surgery held by Dr Reshmi Dickson
and Dr Emma Sawyer. To book for a clinic appointment for this procedure you must 1st make
an appointment with either of these Dr’s in normal surgery to discuss this and see if this will
be suitable for you.
HRT (Hormone Replacement Therapy)
Our Practice Nurses monitor the progress of women who are already on HRT can give
information to women who may be considering this type of treatment.
CERVICAL SMEARS
The practice nurses will do cervical smears. You will receive a reminder letter from the NHS
screening department when this is due. Please make an appointment in the Practice Nurse
Clinic when you received your reminder letter.
BLOOD PRESSURE MONITORING
Our Health Care Assistants and Practice Nurses are available for blood pressure
measurement and monitoring. Please book an appointment with the Health Care Assistant.
ANTENATAL CLINIC
For the care of expectant mothers. The community midwives have clinics both at the surgery
and at the Surestart centre, Hilltop, Eastwood.

Postnatal examinations are undertaken by the doctor at the surgery. An appointment will be
sent for this.
TEENAGE PATIENTS
The practice nurse can discuss with patients any problems relating to the teenage years,
including any general health & sexual advice and ensure immunisations are up to date. The
surgery will automatically send a letter to the patient around the 14th birthday.
MINOR SURGERY
Minor surgical procedures, such as removing skin cysts and moles and joint injections are
carried out by the doctors by prior agreement with the doctor.
CHILD HEALTH CLINIC / HEALTH VISITORS
Screening for health problems, especially in growth and development, in children under 5
years of age will be undertaken by Health Visitors who are based at Eastwood Health Centre,
Nottingham Road, Eastwood, Nottingham NG16 3GL. To book an appointment phone: 01773

HOLIDAY VACCINATIONS FOR FOREIGN TRAVEL
Complete the Travel Questionnaire and leave with the surgery for the nurse to review what
you require and when. Save time and print the form from our website and bring into surgery
CHILD IMMUNISATION
These will be carried out by the practice nurses within the Nurses normal surgery by
appointment.
DISTRICT NURSES
District nurses are based at Eastwood Health Centre.

Appointments are made via referral by your GP.
ACUPUNCTURE
Dr Lim does an acupuncture clinic. An appointment with him in normal surgery to discuss if it

Violence and abuse
Church Walk Surgery is a NHS Zero Tolerance Zone. Zero Tolerance is a nationwide
campaign to stop violence against staff working in the NHS. Violence against the doctors,
staff or other persons present on the practice premises will not be tolerated. Violence against
staff working in the NHS is a crime and Individuals behaving violently will be reported to the
police.
Patient information
All information at the Church Walk Surgery General Practice is held, retained and destroyed
in accordance with NHS guidelines and under the data protection act 1998. If you require any
further information of who has access your patient information please contact the practice
manager.
From time to time patient records may be reviewed by a healthcare professional who is not an
individuals own GP for the purposes of clinical audit. This would only be for the purpose of
quality assurance and each healthcare professional is bound by a code of conduct with
respect to confidentiality. If you do not wish your records to be reviewed for such purposes
you should inform a member of staff. The surgery will then record that you, the patient has
not given your consent and ensure that your records do not form part of any external review.
When the surgery is closed
If you or your family need urgent medical care when the surgery is closed, please phone the
usual daytime surgery number and you will be given instructions of what you should do
depending on the nature of the call or emergency. You may be asked to put down the phone
and dial 111. When you call 111 your needs will be assessed and you will either be given
advice or arrangements will be made for you to be seen by a healthcare professional.
You can also call the NHS 111 number immediately rather than dialing the surgery number in
the 1st instance. NHS 111 is has been introduced to make it easier for you to access local
NHS healthcare services. You can call 111 when you need medical help fast but it’s not a 999
emergency. NHS 111 is a fast and easy way to get the right help, whatever the time.

NHS Walk in Centre
At the NHS Walk-in Centre at London Road, Nottingham, the 8am-8pm Health Centre on
Upper Parliament Street and Clifton Nurse Access Point in the Clifton Cornerstone, you
can see an experienced nurse for treatment of minor injuries and illnesses.
Nottingham Walk-in Centre is open every day including weekends and Bank Holidays
between 7am and 9pm.

Patient Participation Group
If you would like to know more about the Patient Participation Group, or would like to discuss
the possibility of becoming a member please give your details to any of the reception staff.
Church Walk supports and encourages patient participation and consultation.
We also welcome feedback and comments on our services as part of our learning and
services development. Our Patient Participation Group (PPG) has been operating for some
time now, and is attended by volunteer patient representatives, GP’s and other practice staff.
The PPG meets about six times a year, usually in an afternoon for two hours, and is
concerned with any aspect of services offered, practice policy and patient interests.
