DESCRIPTION: The Leadership report was established in 1995 at the 60th Grand Council in Memphis, TN. This is a report of the individual and collective Leader-Development and innovative activities by Phi Delta Chi Chapters to promote leadership and improve leadership skills amongst the Brothers. This is to augment the programs Phi Delta Chi has developed in Leader-Development and to show why we are known as “Leaders in Pharmacy.” The award was renamed at the 65th Grand Council in San Antonio after one of the most remarkable Brothers the Fraternity has ever seen. John D. Grabenstein has selflessly offered his leadership to the Fraternity for over three decades. His dedication and commitment are a shining example to the Fraternity and the world of pharmacy (as the “father” of vaccinating pharmacists). There is no more deserving Brother and his example is something that we should all aspire to live up to. WHAT TO SUBMIT: Phi Delta Chi Achievement Award Program (LDS Version) November Report Revised Summer 2015 2 Your report should discuss events that have occurred between December 2014 - November 2015. The judges suggest that you include the following according to the stated criteria: Programs promoting Leader-Development and People Skills Describe Leader-Development programs, which your Chapter implemented to improve the leadership capabilities of your prospective members, Brothers and other students at your campus. Include training programs, activities and any innovation your Chapter has developed. Indicate how many or what fraction of Brother participated in each program. State the number of people served by these programs. Remember that leadership is the ability to influence others to accomplish a task. Only describe programs that increased the ability of people to influence others; do not include programs where leaders simply exerted leadership (all programs have that, so you wouldn’t be describing anything remarkable). If it’s a scholastic project, describe it elsewhere, not in this report. If it’s a professional project, describe it elsewhere, not in this report. Chapter recognition of Leader-Development and People Skills Describe how your Chapter recognizes the leadership achievement of prospective members, Brothers (Collegiate and Alumni) and other pharmacy students through awards, incentives, or other forms of recognition. Outcomes of Leader-Development by the Chapter Quantitative measure of Chapter’s leadership achievement. Include number of Brothers on the roster of Phi Lambda Sigma or Omicron Delta Kappa, officers in other pharmacy organizations (do not list Phi Delta Chi chapter offices), pharmacy student government, University student government, receiving honors or awards based on leadership traits, in comparison to total Chapter membership. Do not include scholarships based on GPA or need, only on leadership. Scholarships listed in the Prescott Scholarship Report should not be listed. Furthermore, only list leadership roles in pharmacy or university organizations, or awards received for Chapter projects and endeavors. Do not include Rho Chi membership unless it relates to a specific office held, lest the judges conclude you haven’t read the instructions. Written Presentation Brevity is the hallmark of a well-written report. Do not let the content of the report exceed 6 pages in length, this limit does not include cover pages, table of contents, pictures, etc. The judges are looking for a professional and concisely written report describing your activities. Creative programs and leadership success are more important than creative writing. Grabenstein Leadership Report Item #17: Founders' Day Activity with Photograph (2 points total) Phi Delta Chi Bylaws, Chapter III, Number 8: "November 2nd shall be known as Founders’ Day and shall be observed with a fitting ceremony by every Chapter, both active and alumni." Suggestions for the event include a Founders' Day Banquet or Luncheon with a guest speaker, a seminar with a pharmaceutical topic, planting a tree, or a memorial ceremony honoring the 11 Founding Fathers. “Regional” Founder’s Day celebrations have become popular in the last few years. Unless your chapter is hosting this type of event or a large majority of your Chapter’s Collegiate Brothers are attending this type of an event, the Chapter is still encouraged to plan a local Founder’s Day celebration so that Brothers who are unable to attend the “Regional” celebration can still have a chance to observe the day. If possible, attach a high resolution digital photograph(s) of your Founders' Day Activity suitable for printing in The Communicator. Captions should be listed below with the names of the Brothers pictured. Please send the highest resolution photo possible. For print publication, it is best to send the new, unedited file directly off the camera. Try not to save the photo using an image handling program such as iPhoto, Facebook or others as they reduce the file size and quality. Any photo file format will be accepted. Formal meetings require use of the complete Ritual opening and closing, as well as Officer's Robes, Regalia, and devices as outlined in the Ritual Book. They will be returned to the Chapter. At the 60th Grand Council (Memphis, 1995) the Fraternity established a National Philanthropic endeavor. St. Jude Children’s Research Hospital in Memphis, TN was designated as the Philanthropic Organization. At the 66th Grand Council (West Palm Beach, 2007), the Executive Council announced a new partnership between St. Jude and Phi Delta Chi called The Prescription for Hope letter-writing campaign. This commitment to raise $200,000 over 4 years resulted in one of the St. Jude hospital pharmacies being named after Phi Delta Chi after only 2 years of participation, along with much needed funding being used to improve the lives of children through unparalleled medical care and research. At the 67th Grand Council (Phoenix, 2009), the Brotherhood voted to renew our commitment to St. Jude and at the Regional Meetings in 2010 it was announced that we would accept a new challenge and raise $1 million in 10 years, that will result in a patient/family room being named after Phi Delta Chi. Chapters are encouraged to continue participating in this campaign to continue to raise money for our National Philanthropy. You should have been contacted by your local St. Jude representative recently regarding your participation in the letter-writing campaign. Go to www.stjude.org/events Click the red Find An Event button Search for “PDC” You will see all current Phi Delta Chi chapters have an event site Each member of Phi Delta Chi can register and begin fundraising. Once a member registers, the website has emails that are already set up which each member can push out to their circle of friends and family. Donors can also simply click the “make a gift” located on the upper right hand corner to donate. At the 69th Grand Council (Omaha, 2013), it was announced that Phi Delta Chi could now have its own teams in order to participate in the St. Jude Give Thanks Walks that were held in cities around the country in November. This walk has been replaced by The St. Jude Walk/Run to End Childhood Cancer that occurs on varied dates in September and in numerous cities across the country. Chapters wishing to participate this way should form teams using Phi Delta Chi and their Chapter as the name. If your Chapter elects to continue previous local traditions in support of St. Jude, please use the online donations form located on our website at https://shop.stjude.org/GiftCatalog/donation.do?cID=13303&pID=14012 to contribute directly to St. Jude. For Chapters that will be writing checks payable to St. Jude, a donations form available from St. Jude can be used to submit the donation. Please be sure to indicate on the form that this is a contribution for the ‘Prescription for Hope’ and identify your Chapter.
