Present Research Activities: This investigator has had his primary basic research training and experience in the study of degenerative changes in connective tissue. The relationship of these changes to blood 22 flow has been studied in bone, tendon, and articular cartilage with aging and osteoarthritis. Use has been made of both laboratory models and human clinical material. In the realm of clinical research his major emphasis has been on the study of degenerative disease of the spine as well as degenerative arthritis of the hip and knee. Anatomic and pathologic studies have been performed to expand the body of knowledge in these areas of the skeleton in normal and diseased states. This knowledge has then been related to new methods of surgical treatment of the age-related disorders of the spine, hip and knee. A continuum of research has also been established on the new and evolutionary diagnostic modalities used to study the spine, hip and knee with clinical pathologic correlations. Current Basic Science Research Proiects: Studies are being performed on the changes in connective tissue, biochemical composition of the intervertebral disc and articular cartilage with aging and variational stress. Anatomic studies are being conducted on the gross and microvasculature supplying the spinal cord and Cauda Equina. The relationship of this blood supply to pathologic disease states is also being evaluated. A laboratory model of osteoarthritis has been produced in the canine and rabbit knee with subsequent evaluation of the changes in vascular bed and blood flow. A major bioengineering project is currently being conducted on the initial fixation of uncemented hip implants in cadaver femora. Using an Instron testing device both axial and rotational fixation is being evaluated with a variety of hip implants as a measure of fixation strength and safe parameters for fixation forces. Instrumentation is being developed to allow In vivo measurements of both torque and axial fixation to improve the degree of operative stability achieved. A series of experiments are being conducted to study the cellular function at the interface between uncemented implants and adjacent bone in laboratory animals. A variety of stimuli are being evaluated to better understand the processes related to bony ingrowth. The effect of a variety of anticoagulants including aspirin, Coumadin, and Heparin are also being studied in regard to porous ingrowth. Current Clinical Research Projects: A randomized prospective evaluation of lumbar spine surgery for degenerative diseases of the spine to evaluate decompression, fusion, and the use of interposition membranes is being conducted. A correlative study of MRI and CT scanning is being conducted in terms of evaluation of lumbar spinal stenosis and lumbar disc herniation in conjunction with the Department of Radiology. 23 The effectiveness of low dose Coumadin is being evaluated to prevent thromboembolic complications of hip reconstruction. An FDA approved investigational study is being conducted on the use of Gentamicin impregnated methylmethacrylate in the treatment of infected total hip replacements. An FDA approved study is being performed evaluating uncemented femoral prostheses and their effectiveness in the treatment of degenerative arthritis of the hip. A major long term prospective study of all total hip and Knee replacements performed at Pennsylvania Hospital and Thomas Jefferson University Hospital is being performed. A series of studies is being conducted in regard to the factors affecting long term loosening in cemented hip implants. These include femoral head size, cement technique about the socket, and extreme age. Survivorship analysis is being studied in regard to all of these parameters in an effort to ascertain their effect. Intensive review of all of those patients who have developed either symptomatic or asymptomatic pulmonary emboli is being conducted to isolate those factors prejudicing their development of this complication as compared to the large population sub-group free of thromboembolic disease. PCA - patient controlled analgesia is being evaluated as a method of pain relief in the immediate postoperative course after hip replacement surgery. A prospective randomized study is being conducted to compare the Hardinge approach versus trochanteric osteotomy. Pain, surgical technique as evaluated by x-ray, and blood loss are being evaluated. Muscle strength postoperatively is also being evaluated utilizing Cybex. A series of outcome studies are being conducted measuring effectiveness of joint reconstruction on patient independence, emotional well being and social independence. 24
