• Have both Medicare Part A and Part B

• Have permanent residence in our service area

• Not have End Stage Renal Disease (ESRD), (kidney dialysis)

Cigna-HealthSpring, referrals are fast and easy. We use the most current online systems to ensure a streamlined, efficient referral process. Your on-staff Referral Nurse is dedicated to setting up referrals for you based on your doctor’s recommendations. And
